Effect of selective proximal and truncal vagotomy on canine gallbladder bile composition.

In 6 dogs the influence of consecutively a selective proximal vagotomy (SPV) and a truncal vagotomy (TV) on gallbladder bile composition was studied. Following SPV no significant changes in biliary lipids were observed. On the other hand, after TV an increase of deoxycholic acid (D) and a concomitant decrease of cholic acid (C) occurred, resulting in a marked increase of the D/C ratio. Although a positive correlation was found between the D/C ratio and the lithogenic index of bile, the change in bile acid composition after TV was accompanied with only a slight, statistically not significant increase of the lithogenic index. It is concluded that in the dog neither SPV nor TV is a definitely lithogenic procedure.

Permanent cardiac pacing with sutureless myocardial electrodes: experience in first one hundred patients.

This report concerns our experience with a sutureless myocardial lead which we have used in 103 patients for establishment of permanent cardiac pacing. The electrode was inserted through a subxiphoid approach in all patients. Results to date indicate that this technique compares favorably with the presently popular transvenous technique as far as immediate or long-term morbidity is concerned. Complications were related entirely to pericardiotomy; no lead failure was seen during a maximum follow-up period of four years.

Zinc deficiency syndrome versus glucagonoma syndrome.

In this paper two patients with uncommon syndromes, viz. acrodermatitis enteropathica-like eruption due to acute zinc deficiency, when on long-term intravenous hyperalimentation for Crohn's disease, and necrolytic migratory erythema as a consequence of a malignant glucagon secreting alpha-cell tumour of the pancreas (glucagonoma syndrome) are reported. Attention is paid to the many common features of the skin lesions in both syndromes. This is discussed in detail. Both patients passed through a catabolic stage. Laboratory investigations, however, failed to demonstrate a common biochemical mechanism which might be responsible for the skin lesions. Administration of zinc in the first patient and surgical treatment of the second patient results in rapid clearing of the skin lesions and other symptoms.

Acute Disease↗

A late complication of a diagnostic mediastinoscopy.

A 69-year-old man with a moderately well differentiated squamous-cell carcinoma of the lung, underwent a mediastinoscopy. All histological examinations of the mediastinal nodes were negative. The patient was treated with a curative left pneumonectomy. Three months after this negative mediastinoscopy the patient developed a metastasis in the mediastinoscopy scar. A report of this case and a review of the literature are presented.

Lymphangiectasia of the small intestine as an acute surgical problem.

Two cases of lymphangiectasia of the small intestine in elderly women are presented. The condition manifested itself in one case by a sudden perforation of the jejunum; in the other case by ileus of the terminal ileum. In both cases the disease process pursued a relentless course that ultimately led to the death of the patients. Elective surgery in patients with lymphangiectasia has been performed before. Acute surgery of lymphangiectasia of the small intestine with complaints of acute abdomen, obstruction or perforation as presenting signs of the disease has not been described previously.

Curative and palliative surgical treatment of pancreaticoduodenal adenocarcinoma.

In the period 1965 to 1976 inclusive, 55 patients suffering from duodenopancreatic adenocarcinoma were surgically treated at the Free University Hospital. Twenty of these patients underwent a 'curative' Whipple pancreaticoduodenectomy for a resectable tumor. The difference in survival time makes a differentiation important between carcinomas occurring in the head of the pancreas and those of the peri-ampullary region. In our series, 11 of the former were found and 9 of the latter group. Of each group one patient died within 30 days of the operation. The average survival time of 9 patients, operated on for a tumor in the head of the pancreas, was 10.2 months, with a range from 2 to 23 months. One patient is still alive 23 months after the operation. The average survival time of 5 patients with a periampullary tumor was 22 months with a range from 6 to 52 months. Three patients are still alive, one more than 12 years and 2 more than 19 months after operation. Considering the bad results, and the fact that 30 percent of pancreatic carcinomas are multifocal, it would seem that the classical Whipple operation is a thing of the past. One can then make a plea for total pancreaticoduodenectomy or even for a regional pancreatectomy. Thirty-five patients underwent a palliative procedure, consisting of a biliary bypass combined in a number of cases with a gastroenterostomy. Five patients died within 30 days of the operation. The average survival time for the remaining patients was 9.5 months with a range of 1.5 to 30 months.

Adenocarcinoma↗

Endoscopy in lye burns of oesophagus and stomach.

Upon ingestion of lye and its compounds severe corrosive lesions may develop not only in the oesophagus but also of the stomach. These are frequently not recognized until life-threatening complications have set in. Two case histories are reported to point to the need for early endoscopic examinations. Brown-black discoloration of the mucosa of the stomach signifies necrosis through the thickness of the wall which is an indication for immediate resection.

First results obtained with an epicardial screw-in electrode.

A description is given of the introduction of an epicardial screw-in electrode (Medtronic, model 6917) by the infrasternal approach in 22 patients with severe cardiac conduction disorders. Special attention is given to the surgical technique, the electrode and the measurement of the threshold value. Comparison of the various techniques of pacemarker implantation leads to the conclusion that this method of treatment offers many advantages.

Idiopathic retroperitoneal fibrosis in multifocal fibrosclerosis.

A case of rare combination of retroperitoneal fibrosis and Riedel's thyroiditis is presented. The term multifocal fibrosclerosis is used in the literature to denote such combination and others which include idiopathic retroperitoneal fibrosis, mediastinal fibrosis, sclerosing cholangitis, invasive (Riedel's) thyroiditis and pseudotumor of the orbit.

Histoplasmoma of the lung.

A review of all granulomatous surgical lesions in the files of the Free University Hospital revealed four cases of histoplasmoma in which Grocott-staining procedures provided histologic confirmation of the presence of Histoplasma capsulatum bodies. The case histories have been presented briefly. A short review of the literature follows.

Tracheal reconstruction with hydroxyapatite tracheal prosthesis.

A porous hydroxyapatite tracheal prosthesis was implanted in rabbits after resection of a 15 mm circular segment of the cervical trachea. Postoperatively the animals were followed by radiography, bronchoscopy, and microscopy at the time of autopsy. The longest survival time was 22 weeks. All the rabbits died of infection at the prosthetic site or of progressive stenosis at the anastomoses. The prosthesis was incorporated by surrounding tissues, but no overgrowth of ciliated epithelium on the luminal surface of the prosthesis was observed. Although a hydroxyapatite tracheal prosthesis is tolerated in rabbits, covering of the luminal surface of the prosthesis with a non-porous layer to prevent infection and to permit mucosal overgrowth is necessary to achieve better long-term results.

Similar hypocholesterolemic effect of ileal bypass and ileal excision in pigs.

We have compared the effect of partial ileal bypass versus ileal excision on the level of serum cholesterol in cholesterol-fed pigs. Both surgical procedures drastically lowered serum cholesterol in spite of continuation of cholesterol feeding. No evidence was obtained for a differential hypocholesterolemic effect of ileal bypass and excision.

[The use of a myocardial screw electrode for continuous stimulation of the heart with a pacemaker (author's transl)].

In 50 patients with serious heart conduction disorders the authors describe the use of a new epicardial screw electrode (Medtronic models 6917 and 6917-A), inserted through a substernal route. The operative technique, the electrode and the authors' experience are described in detail. By comparison with others, the present technique offers many advantages.
