Disseminated Hodgkin's disease with bilateral orbital involvement.

Hodgkin's disease was diagnosed in a patient by lymph node excision. Orbital infiltration was demonstrated by echography and computerized axial tomography. Subsequent orbital biopsy specimens showed lacrimal gland involvement with definite Reed-Sternberg cells.

Peripapillary choroidal melanomas. Extension along the optic nerve and its sheaths.

We reviewed 26 cases of peripapillary melanoma, 21 (80.8%) of which extended into the optic nerve or its meningeal sheaths. Most melanomas that extended into the optic nerve were necrotic and of the mixed or epithelioid cell type; all invaded the overlying retina and occurred in blind eyes with secondary glaucoma. The extension of melanoma cells into the subarachnoidal space was only influenced by the presence of the melanoma around the optic nerve head (peripapillary). The prognosis of these patients was highly dependent on the presence of orbital extension. A long piece of optic nerve should be excised when eyes with peripapillary melanomas are enucleated.

Cryotherapy of herpes simplex keratitis.

Twenty-one cases of epithelial herpetic keratitis were treated with cryotherapy and 14 cases were treated with Iodoxouridine (IDU). Cryotherapy was found statistically superior to IDU. The average healing time was 2.4 days for cryotherapy and 6.2 days for IDU. The advantages of cryotherapy are emphasized.

Prognostic factors in choroidal and ciliary body melanomas.

We reviewed 293 cases of choroidal and ciliary body melanoma with a followup period of five years or more. Nine factors influenced significantly the prognosis: Age of the patient at the time of enucleation, location of the tumor and of its anterior border, largest tumor diameter in contact with the sclera, height of the tumor, integrity of the Bruch membrane, cell type, pigmentation, and scleral infiltration by tumor cells. Some of these factors can be evaluated clinically before the enucleation. Seven sets of three factors each were determined in order to have better prognostic values based on the clinical data and the cell type. The largest tumor diameter is the single most important clinical and pathological prognostic factor. The prognosis is relatively good when this diameter is 10 mm or less and becomes poor when it exceeds 10mm.

Bilateral choroidal melanomas. Case report and incidence.

A 55-year-old woman had bilateral choroidal melanoma. To our knowledge, this is the second reported case in the United States. To calculate the incidence of bilateral cases, we determined the incidence of unilateral choroidal and ciliary body melanomas among the white population in Iowa. We also studied the incidence of these melanomas by different age and sex groups. One of 2,500 whites will develop a choroidal melanoma during his or her lifetime. Based on these values and those of life expectancy in patients harboring a choroidal melanoma, we calculated the risks of developing a second primary melanoma in the other eye. In a population of 50 million whites, 1 person will develop a bilateral choroidal melanoma during his or her lifetime. In other words, a bilateral case is expected to occur once every 18 years in the United States.

Sympathetic uveitis following glaucoma surgery.

We had two cases of sympathetic uveitis after filtering procedures were performed on blind, painful eyes. A review of the literature shows that it is not the type of antiglaucoma operation that has to be incriminated in precipitating sympathetic uveitis, but the condition of the eye undergoing a filtering procedure. The risk is much higher when the glaucoma is absolute. There is much danger in operating on blind, painful eyes.

Choroidal melanoma and retinal tear.

The patient's eye contained a choroidal melanoma and a horseshoe retinal tear. Clinical recognition of the tumor led to the correct diagnosis and enucleation, avoiding unnecessary and potentially detrimental retinal detachment operations.

Orbital extension of Choroidal and ciliary body melanomas.

In a review of 432 choroidal and ciliary body melanomas, histopathologic evidence of orbital extension was found in 45 cases (10.4 percent). The occurrence of orbital extension was influenced mainly by the cell type (P less than .05) and by the size of the choroidal mass (P less than .001). Local orbital recurrence occurred in 23 percent of cases with evidence of extrascleral extension. Survival rates were independent of cell type, pigmentation, and size and shape of the extension. Patients treated by exenteration had significantly better prognosis than nontreated patients or patients treated by chemotherapy, radiotherapy, or local excision (P equals .0002). Early exentration is the best treatment for choroidal melanomas with orbital extension.

False-positive results with the radioactive phosphorus test.

A chorioretinal granuloma that contained acid-fast bacilli and a choriodal nevus that consisted of benign nevus cells yielded false-positive radioactive phosphorus tests. The beta emission exceeded that of the control areas by more than 100% in each case. The granuloma had infiltrated the sclera, permitting inflammatory tissue to be in closer proximity to the counting probe than was the normal choroid. The reason for the increased metabolic activity of the nevus cells remains unexplained.

Eosinophilic granuloma of orbit.

Three cases of orbital eosinophilic granuloma are reported. In all 3 the diagnosis was established histologically. In the follow-up period, ranging between 5 and 20 years, the disease has taken a benign course; incomplete surgical removal in 2 of the patients and radiation of the lesion in one have been equally effective in promoting healing. The first mode of therapy may be preferable in the region of orbit in order to avoid damage to the globe or optic nerve.

Intraocular pressure, cup-disc ratio, and steroid responsiveness in retinal detachment.

A review of the literature failed to indicate whether the high incidence of open angle glaucoma in patients with nontraumatic rhegmatogenous retinal detachment is due to common genetic determinants. The diurnal variation of intraocular pressure, the cup-disc C/D ratio, and the intraocular response to topically applied corticosteroids were studied in 30 subjects with unilateral, nontraumatic, rhegmatogenous retinal detachment. The results indicate that 20% of the subjects were high steroid responders and that 53% had a C/D ratio greater than 0.3. We shall discuss the relationship of these findings to the multifactorial inheritance of open angle glaucoma.

Cloxacillin penetration.

We investigated the intraocular penetration of cloxacillin in 14 patients undergoing surgery for senile cataract after intramuscular and subconjunctival injections of the drug. No antibiotic level was detected in the aqueous humor when cloxacillin was administered intramuscularly in a dose that ranged between 1 and 4 g. With subconjunctival injection, therapeutically effective cloxacillin activity was detected in the aqueous humor only after a dose of 250 mg.

Outcome of traumatic hyphema.

One hundred and twenty-seven cases of traumatic hyphema are reviewed and discussed according to a definite system of grading. Grade iii hyphemas have definitely a poorer prognosis than Grade ii and Grade i hyphemas. Rebleeding occurs more frequently when there is a delay in treatment but does not appear to affect the outcome of a traumatic hyphema. Blood staining of the cornea could be avoided by an adequate treatment started immediately after the trauma, thus decreasing the percentage of blindness following a traumatic hyphema.

Bilateral corneal perforations in Stevens-Johnson syndrome.

A case of Steven-Johnson syndrome was treated with topical and systemic corticosteroids. The patient developed bilateral simultaneous staphylococcal corneal ulcers. The ulcers eventuated in bilateral corneal perforations within 48 hours, in spite of topical, subconjunctival, and systemic antibiotic therapy.

Endogenous E. coli endophthalmitis.

A case of Escherichia coli septicemia with associated metastatic en dophthalmitis and endocarditis is presented. The ocular signs and symptoms were the initial manifestations of sepsis. Irreversible damage to the eye occurred in less than 24 hours. The pattern of metastatic bacterial endophthalmitis has changed since the introduction of potent antimicrobial agents, with an increased incidence of Gram-negative bacillemia. E. coli endophthalmitis carries a poor prognosis. Early diagnosis and systemic treatment will prevent the life-threatening complications of sepsis.
