[Overview of the current status of measurable parameters of cartilage metabolism in various body fluids].

The human cartilage and bone is characterized by a remodeling during the life, well balanced by neosynthesis and degradation of matrix components. In different joint diseases, it becomes imbalanced and the destruction of the cartilage supersedes the repair. In tissue processes in disease and in normal turnover of the matrix, these molecules are fragmented and released into surrounding fluids, in the synovial fluid, and then in the blood and the urine, where they can be detected. The quantitative measurement in the synovial fluid is more specific than in the other body fluids. The research process in recent years has suggested that these molecular markers of cartilage and bone matrix metabolism can be used to determine diagnosis, the disease severity rather than its presence or absence, the prognosis, and the response to therapy. They should help to identify the disease mechanism in different joint diseases not only on the tissue but also on the molecular level. The specific cartilage matrix markers promise to become useful tools in the future in clinical use. The research in this area is still in the early stages, with most results dated from the end of the 1980s and the 1990s.

[Backache--orthopedic diagnosis and special therapeutic possibilities].

Back pain is a wide-spread complaint in modern society and in part an adverse effect resulting from present-day lifestyles. To date, too little attention has been drawn to efficient prevention. The treatment of patients affected with back pain today calls for an in-depth pathophysiological knowledge about the mechanism occurring on the spine. Diagnostic methods and non-operative treatment have been complemented specifically by chirotherapeutic methods. A systematic classification of the vertebragenous pain syndromes and the related treatment strategies have proven to be successful. Irrespectively of that, the individuality of diagnosis and the complaint's progress in each patient should be taken into account at all events. Cost-intensive diagnostic imaging should only be undertaken with a clear indication. Today, the overwhelming majority of back pain patients undergo a non-operative treatment. In case the conservative applications prove inefficient, it is possible to successfully operate on patients a with disc prolaps, degenerative instabilities as well as osseous spinal foraminal stenoses. A precondition is a precise causal diagnosis and a clear indication for operative intervention.

Synovial oxygen partial pressure after experimental microtraumatic joint affection.

We have developed a method allowing the direct determination of the synovial oxygen partial pressure (pO2) in vivo by using puncture electrodes. The investigations were carried out on the knee joints of young adult rabbits. Immediately after a defined microtraumatization of the left knee joints, there was a decrease in the synovial pO2 down to an average of 72% of an initial value obtained from untreated control animals. But 42 and 84 days after loading, a significant rise in the synovial pO2 was detectable. The unloaded contralateral joints showed a slight consensual reaction. The dynamics of the synovial pO2 after the microtraumatization of joints is under discussion on the basis of results of micromorphological studies of the articular cartilage and the synovial membrane.

[Chondrocyte proliferation in joint cartilages in beginning arthrosis--experimental studies].

The cellular reaction of the articular cartilage in the beginning of the arthrosis has not been investigated very often up to now. An arthrosis model of little progression was used for this investigation by operative production of a valgus position of the proximal tibia at an angle of 30 degrees in the knee-joint of adult rabbits (in connection with Reimann, 1973). Proliferative processes of the chondrocytes are in the foreground with the beginning of arthrosis. This could be observed by means of histological and autoradiographical investigations with 3H-thymidine. The mode of the cell division is discussed. Mitotic divisions after modulation of the chondrocytes in a chondroblastic phase are probable.

[Microtrauma and cartilage injury].

In addition to macrotraumas resulting from single and heavy impact, recurrent microtraumas are probably frequently the cause of cartilage damage. The risk of a microtraumatic cartilage damage exists especially in activities involving a high load frequency. Epidemiological studies and dynamometric measurements in race events support this assumption. Based on experimental investigations it is shown how the different components of the articular system-cartilage, synovial membrane and subchondral bone react to microtraumatic stress. In the elimination of microtraumatically induced joint damage special emphasis should be placed on prophylaxis.
