[Bretylium tosylate as an anti-arrhythmic drug].

With the help of an analysis of the world literature pharmacological, pharmacokinetic and electrophysiological properties as well as the possibilities of the clinico-therapeutic use of Bretylium tosylat are demonstrated. The pharmacon is of increasing interest as antiarrhytmic, especially in the commandment of otherwise therapy-refractory ventricular disturbances of rhythm. A positively inotropic effect is proved. More severe side effects, such as arterial hypotension and increase of the pulmonary vascular resistance have an objection to routine application. The antifibrillatory effect needs particular consideration. This effect might promote this pharmacon to the "chemical defibrillator" in the emergency situation.

[Clinical experiences with bretylium tosylate].

Because of its antiarrhythmic and antifibrillatory qualities as well as its positively inotropic effect bretylium tosylate increasingly finds interest. The first results of a clinical and haemodynamical mechanocardiographic investigation with bretylium tosylate are presented. In doses between 1.6 and 6.2 mg/kg body weight intravenously and intramuscularly after an initial increase of blood pressure a clear decrease of the systolic system blood pressure develops. The resistance of the peripheral vessels decreases by 10 to 20%. It is an increase of the heart index and the stroke volume index by 10 to 30% to be proved as an expression of a positively inotropic effect. In orientating investigations the antiarrhythmic effect could be confirmed. The side-effects observed cause us to give a strong indication and to observe the patients carefully. The main area of application of bretylium tosylate might be seen in the possibility to achieve a pharmacological defibrillation of the ventricles.

[Clinical-pharmacological effective data on 3',4',-dipropionylhelveticosol].

In experiments of treatment carried out on 39 patients with cardiac insufficiency with tachyarrhythmia for the partially synthetic, new heart glycoside 3',4'-dipropionyl helveticosol the following clinico-pharmacological data of efficacy were approximately established on an average: level of full efficacy 2.7 mg, daily coefficient of disappearing 40%, coefficient of enteral absorption 30%, daily maintenance dose intravenously 1.1 mg and orally 3.7 mg.

[Experimental model of coronary thrombosis in the closed thorax in swine].

Coronary thrombosis was induced in 14 miniature pigs by means of direct current. The induction electrode was introduced via the common carotid artery into the coronary artery. The intensity of current (1 mA) and the duration of current flow (60 min) were kept constant in all the experimental animals. 6 miniature pigs survived the period of observation (48 h). In all instances, an injury to the intima was the morphopathological location factor. With the exception of one animal, which died after an induction time of 60 min, coronary thrombosis was stated in all the miniature pigs. The experimental arrangement which produces closed-thorax coronary thrombosis is suited for cardiosurgical, physiopathological and pharmacological experiments, especially because the anatomy and the degree of development of the porcine heart are most similar to those of the human heart.

[Experimental coronary thrombosis in dogs. Morphologic investigations (author's transl)].

By means of electric induction without opening the chest in 23 dogs coronary thromboses were produced with the aid of a standardized method based on a test model of Salazar (1961). The morphological results are reported. 13 of the dogs showed coronary thromboses. At the same time in 12 animals a myocardial infarction could be registered. Altogether, we observed 17 myocardial infarctions. The electric field led to differently severe lesions in the walls of the coronary arteries. The significance of this test model is discussed.

[Morphologic investigations on experimental coronary thromboses in pigs (author's transl)].

With the aid of a standardized method basing on a report of Salazar (1961) coronary thromboses in pigs were produced by means of electric induction without opening the chest. 12 of 13 pigs showed thromboses in coronary arteries, in 8 animals with embolization of the thrombotic material. At the sametime in 8 pigs a myocardial infarction could be registered. The significance of these experiments discussed.

[Possibilities of a routine prevention of arrhythmia in the early phase of acute myocardial infarct].

The frequency of the electric heart death in the early phase of the acute myocardial infarction demands a prophylactic use of antiarrhythmic remedies. In a guarded ward for coronary diseases since 1972 all patients with a fresh myocardial infarction or suspicion of infarction were treated with a continuous intravenous lidocain drip in a dosage of 100 mg/per hour for 48 hours. In 266 patients in a certain examination period ventricular fibrillation was observed only once and ventricular tachycardias twice. In accordance with literary data the prophylatic medication of lidocain gives an effective defence against threatening disturbances of the ventricular rhythm. The indications for a passagere electrostimulation in bradycardiac disturbances of rhythm associated with acute myocardial infarction are demonstrated.

[Preinfarct and prehospital care of myocardial infarction].

In intensive therapy units, especially in guarded wards for coronary diseases, the lethality in acute myocardial infarction could be reduced by ca. 50%. However, these favourable results are nowadays of importance for the patients concerned only then, when the diagnosis myocardial infarction or the tentative diagnosis infarction are made in a short period and already prehospitally adequate measures are begun. The following measures are in the centre of prehospital care: Immediate home visit when a suspicion of infarction is present, immediate hospitalisation into an in-patient facility, alleviation of pain, immediate treatment of complications (disturbances of cardiac rhythm, shock, pulmonary oedema, cardiac arrest), prevention of disturbances of the cardiac rhythm. According to the modern knowledge is to be assumed that about 50% of the patients with infarction undergo a premonitory stage which lasts for hours, days or weeks. It is possible that here develop concrete approaches to an infarction prophylaxis. In the first place there are an increase of frequency, intensity and duration of the attacks of angina pectoris, insufficient responsiveness to nitrangin, provocation of the attacks by slight causes and changes of the ECG as they are typical for the inner layer and outer layer ischaemia and the so-called rudimentary infarction. The treatment of the preinfarction process should immediately be begun, at best under clinical conditions.

[Clinico-pharmacological studies with Nifedipine--a new coronary substance].

On 19 test persons (10 healthy persons, 9 patients with heart diseases) mechanocardiographic and haemodynamic examinations were carried out with the new preparation Nifedipine (05-072) which effects on the coronary vessels. Insignificant positively inotropic effects with invariable heart rate and cardiac output, decreasing heart action and decreased oxygen consumption were found. Arterial blood pressure and peripheral resistance were only slightly diminished. Own results and reports from literature show that the application of Nifedipine in the ischaemic heart disease appears to be successful.

[An experimental model of electrically-induced coronary thrombosis in the closed chest].

The authors report on an experimental model of coronary thrombosis, electrically induced in the closed thorax. By this technique they succeeded inducing coronary thrombosis with myocardial infarction in 13 cases out of 21. In three cases just disseminated myocardial necrosis was proved. All of the other animals showed vast myocardial infarctions. Coronarography, arterial pressures, typical shifts of serum enzymes, decrease of thrombocytes and fibrinogen and elevation of spontaneous fibrinolysis during the first hours proved to be valuable for the confirmation of successful inducing of thrombosis; in individual cases, however, these estimations yield no absolute certainty.
