[EUROPOP--European Programme of Occupational Risks and Pregnancy Outcome--part III].

As continuation of the previous paper ther authors submit the second part of the results of the European programme EUROPOP which evaluates the relationship between work, living conditions of pregnant women, prenatal care and medical data on the outcome of pregnancy. The authors compare groups of women, who had premature deliveries (22nd-36 week of gestation) with control groups 1/10 deliveries in term (> or = 37 weeks). Groups of abortions in the second trimester which were investigated were relatively small (Czech Republic 49 and all-European study 229). To keep the paper within reasonable limits the tables are not presented.

[Correlation of umbilical artery erythropoietin levels during labor with traditional indicators of neonatal hypoxia].

OBJECTIVE: To assess a correlation between the erythropoietin (Epo) level in umbilical blood during delivery and three "classical" markers of hypoxia: the Apgar score, pH in the umbilical artery and lactate concentration. MATERIAL AND METHODS: The above parameters were assessed in 109 deliveries divided into three groups-normal deliveries, deliveries with neonatal pathology at term and premature deliveries. RESULTS: Based on calculation of Spearman correlations which seemed most suitable with regard to the nature of the experimental data no significant relations were found between Epo and any of the mentioned parameters, while the mutual correlations between the Apgar score, pH of the umbilical artery and lactate were highly significant. CONCLUSION: Epo is an independent marker of foetal hypoxia and supplementation of "classical" markers by this examination could be an asset to the evaluation of the prognosis of postnatal development.

[Erythropoietin and erythropoiesis after kidney transplantation].

In a group of 15 patients after the first transplantation of a cadaverous kidney the authors monitored for the period of one month the serum erythropoietin concentration (Epo), haemoglobin levels (Hb), the haematocrit (Ht) and serum creatinine (Cr). The objective of the investigation was to evaluate in relation to serum concentrations of Epo and the function of the graft. Erythropoietin was examined by the authors own method before and after transplantation during the first 10 days daily and then on the 12th, 15th, 20th, 25th and 30th day. Normal values are 22-44 mU/ml. The mean Epo value before transplantation was 41.9 +/- 18.8 mU/ml. After transplantation the highest mean value of Epo was recorded on the second day when it reached a value of 102.8 +/- 144 mU/ml. The mean Epo serum concentration at the end of the study was 45.9 +/- 20.9 mU/ml and did not differ significantly from values recorded before transplantation. Between Hb and Ht values and Epo serum concentrations no correlation was revealed. Conversely, statistical analysis revealed a close inverse relationship between the drop of creatininaemia and the rise of Hb and Ht values. From the results of the investigation ensues that the decisive condition for the onset of the erythropoietic response after renal transplantation is adjustment of the milieu interiéur, induced by the restored excretory function of the graft which creates prerequisites for a normal response of the bone marrow to Epo.

Development and validation of an in-house radioimmunoassay for erythropoietin.

A sensitive radioimmunoassay (RIA) for the determination of erythropoietin (Epo) in blood serum and amniotic fluid has been developed and validated. The assay uses a polyclonal antibody obtained in rabbits immunized with recombinant human Epo by one of the authors. Tracer and second antibody are commercially available. With 100 microliters of sample, the assay can detect 3 U/l. The reproducibility of the assay compares well with commercial Epo RIAs. The normal values estimated in a group of blood donors are slightly higher than those reported for various RIA determinations. The clinical usefulness of the assay was demonstrated by observing the expected increases or decreases of Epo levels in various clinical states known to alter Epo production. The assay meets all requirements of a routine clinical assay at significantly reduced costs, which makes it particularly suitable for use in research applications.

Terguride in the treatment of hyperprolactinemia.

Thirty-two women with ovarian dysfunction due to hyperprolactinemia were treated with a new derivative of lisuride-terguride. Twenty-three patients were treated for infertility. A microadenoma was confirmed in five, and three other patients had had a macroprolactinoma surgically removed. The finding in one of the patients was diagnosed as the syndrome of empty sella. Galactorrhea was present in 18 women. The duration of treatment ranged from 2 to 33 months. The determination of therapeutic dosages was based on individual responses on the prolactin levels within a range from 0.1 to 4.5 mg per day. Increased prolactin levels were successfully normalized in twenty-one treated patients. Regular periods were reappeared in 59% of the women. Thirteen (56%) became pregnant, seven gave birth to healthy babies, two of the patients aborted in the first trimester. Four women are still in later stages of pregnancy. Galactorrhea disappeared in 56% of the patients, being markedly inhibited in the remaining ones. In two cases, microadenoma disappeared after treatment, and in those after surgery the postoperative findings were decreased, in one patients there is no alteration in the pathology. Side effects were seen in 34% of the patients, being mostly mild in nature, and including in most cases nausea, headache and stomach pain. The complaints were transient, receding after prolonged treatment.

Periodontium and vitamin E and A in pregnancy.

The evaluation of the clinical condition of periodontium by means of the epidemiological indexes and the level of oral hygiene in two weeks intervals in the course of physiological pregnancy in 39 women in good general health revealed the maximum of inflammatory changes of periodontium in the 8th month of pregnancy with the amelioration shortly before delivery. Simultaneous follow-up of the physiological levels of vitamin E and A in four weeks intervals showed the decline of the mean levels of both vitamins in the course of the 8th month and their marked elevation shortly before delivery; therefore remains questionable, if this elevation reaching over their physiological range, contributes to the amelioration of the condition of periodontium observed at the same time.
