[Foreign body granuloma of the head of the pancreas caused by a fish bone--a rare differential diagnosis in head of the pancreas tumor].

Despite highly developed scanning methods there is no absolute certainty of delineating malignant pancreatic tumor from chronic pancreatitis. Pancreatitis caused by foreign bodies has occasionally been mentioned in literature. Our report is on the first case of a foreign body granuloma of the pancreatic head caused by a fish-bone transduodenally perforating the pancreas. On preoperative CT and ultrasound as well as by intraoperative inspection and palpation the lesion appeared malignant, so we saw the indication for Whipple's operation. Although the histological examination showed a benign state, taking into account the generally bad prognosis, in case of suspected malignant pancreatic tumor we plead for resection as the only possible form of curative therapy.

A comparison of the prognoses for deep and subcutaneous sarcomas of the extremities.

We evaluated 172 patients who had a soft-tissue sarcoma of the extremity in order to determine whether the prognosis for a subcutaneous sarcoma was better than that for a deep sarcoma. At a median of thirty-six months after the biopsy or definitive operation at our hospital, six of the fifty-two patients who had had a subcutaneous sarcoma had died and one had had a local recurrence; in contrast, forty of the 120 patients who had had a deep sarcoma had died and eight had had a local recurrence. Twenty-five (48 percent) of the subcutaneous sarcomas were malignant fibrous histiocytomas, and thirty-eight (73 percent) were small (five centimeters or less in the largest dimension). The three-year estimates of disease-free survival were 85 percent for the patients who had a subcutaneous sarcoma and 54 per cent for those who had a deep sarcoma (p = 0.002). Although the survival estimates remained significantly different when the groups were matched for histological diagnosis and for intracompartmental location (p = 0.0001 and 0.0006, respectively), they were not significantly different when the groups were matched for the size of the tumor (p = 0.42). A Cox proportional-hazards model confirmed that a tumor size of more than five centimeters and the histological grade are the most significant prognostic factors (p = 0.0007 and p = 0.004, respectively): a tumor size of more than five centimeters was associated with a relative risk of 3.5 (95 per cent confidence interval, 1.7 to 7.3), and a higher histological grade was associated with a relative risk of 4.0 (95 per cent confidence interval, 1.6 to 10.3). Subcutaneous location, when considered separately, was not a significant prognostic factor (p = 0.45). The data indicate that a tumor size of more than five centimeters is a more important prognostic indicator than histological diagnosis, depth, or intracompartmental location.

Token reinforcement in the psychosocial rehabilitation of individuals with chronic mental illness: is it effective over time?

The main purpose of this project was to determine whether the addition of the token economy reinforcement to the regular treatment modalities (medication therapy and psychotherapy) improved the following outcome measures: re-hospitalization rate. NYPCC (agency) therapeutic goals, symptomatology, social integration activities and ADL skills. The research was carried out over a period of 18 months. Subjects were 617 individuals diagnosed as having chronic mental illness. They lived in three adult homes in New York, two of which were on a token economy programme, while the third served as a quasi-control group. The findings suggest that while medication therapy and psychotherapy have different effects in respect to the various outcome measures, the addition of the token economy programme resulted in positive, albeit marginal, gains to all outcome measures.

[Determination of gallstone ileus using emergency gastroscopy].

It is reported on the diagnosis of a gallstone ileus in a 73-year-old female patient. In an otherwise quiescent anamnesis an increasing vomiting has been since 10 days. The X-ray examination of the stomach with visotrast 370 resulted in the suspicion of a pyloric stenosis. By means of emergency gastroscopy an independent disease of stomach and duodenum was excluded, but in the duodenal bulb 2 gallstones of the size of a hazel-nut and a kernel of the hazel-nut with references to a cholecystoduodenal fistula were established. In this case the emergency gastroscopy resulted in the indication to the immediate operation on account of gallstone ileus with previously unclear symptomatology of an ileus. Additionally a nearly hen's egg-sized stone was removed aborally 30 cm from Treitz's fascicle.

[Percutaneous antegrade pyelography with a fine needle].

In 15 patients the percutaneous antegrade pyelography with thin needle was performed for the clarification of a mute kidney and a urinary stasis kidney. The two groups of diseases could not be clarified by means of the methods of the conventional urography. The retrograde pyelography was intentionally given up. By means of the percutaneous antegrade pyelography we could clarify a mute kidney in 8 cases and in 4 cases a urinary stasis kidney. In 3 cases with mute kidney we did not succeed in the demonstration of the renal calyces. No complications appeared. In strong indication the percutaneous antegrade pyelography is an evident diagnostic measure by great security, little stress for the patient and little cost. Despite modern investigation methods including sonography and computer tomography is referred to the fields of application of the percutaneous antegrade pyelography.

[Modification of glucose resorption by bulk materials].

40 test persons were randomized for testing the influence on the glucose resorption by ballast substances. The four groups consisted of 10 persons with healthy metabolism, 10 diabetics with diet, 10 with maninil 5 and 10 with B-insulin. The oral glucose tolerance test with 100 g glucose was used. The examination time after application of the test meal was 120 minutes. As ballast substances the hemicelluloses supercol (galactomannan) and manucol (sodium alginate) were tested. For comparison an examination with only 100 g glucose (experiment A) and one with a placebo substance (experiment D) were performed. Of supercol (experiment B), mannucol (experiment C) and of the placebo substance 20 g were added to the sugar solution. Using supercol we could get unequivocally lower mean blood sugar curves in non-diabetics and in all groups of diabetics compared with the examination without addition of substance and with addition of placebo. When manucol was used, a decrease of the mean blood sugar curves could be observed, however, not proved. Thus the swelling substance supercol leads to a retardation of the glucose resorption.

[Hepatologic dispensaire for blood donors].

By means of hepatological examinations of 119 blood donors who became conspicuous by liver screenings in 57 cases we could prove an adipose degeneration without, in 16 cases a fatty degeneration of the liver without, in 9 cases an adipose degeneration with and in 15 cases a fatty degeneration of the liver with mesenchymal reaction. Only 9 blood donors had normal liver findings. 90% of the punctured blood donors had an overweight up to a maximum of 35 kg in comparison to the ideal weight. 30% of the punctured patients admitted a clearly increased daily consumption of alcoholic beverages. By means of repeated punctures on 24 blood donors after 8 to 32 months after an adequate consultation in three cases a normalisation and in eight cases an improvement of the liver findings could be proved. In 13 cases the findings remained unchanged, also the anamnestic and clinical findings corresponded to this. In 2 blood donors by means of a threefold liver puncture in the observation period of 48 months also a clear retrogression of the liver findings was proved. With this was also connected the reduction of overweight and the avoiding of alcohol. Thus our investigations confirmed that overweight and abuse of alcohol are essential factors for the development of a fatty degeneration of the liver. Of an infectious hepatitis reported in the anamnesis in 10 blood donors histologically in no case residues were established. Thus the liver dispensary is of great importance for the care of blood donors who become conspicuous by screening tests. On the one hand the importance is referred to the blood donor himself, on the other hand to the group of donors. Blood donors taken out of the group could again be included in the special group, when after the exclusion of known noxae the retrogression of unspecific liver changes took place.

[Gynecomastia--an indication of a malignant chorionepithelioma in the male].

The course of the disease in a 26-year-old patient showed that also an endocrinological clinical picture may be the basis of a gynaecomastia. Here the symptom gynaecomastia was bilaterally conditioned by a malignant chorion epithelioma. By this tumour a feminisation with gynaecomastia, atrophy of the testicles and positive HCG-test developes. The ascertainment of the diagnosis is possible with the help of the HCG-test. In the early diagnosis of a malignant chorion epithelioma new therapeutic consequences arise by cytostatic drugs.

[Recurrent metabolic alkalosis due to pyloric stenosis].

After an inability to work lasting 15 months with 9 treatments in an inpatient department in several institutions in a 33-year-old patient the relapsing metabolic alkalosis in hypopotassiaemia and relapsing increase of the creatinine level could causally be clarified by establishment of a pylorus stenosis in chronic duodenal ulcer. Due to the Billroth II operation a complete clinical and objective improvement developed. In this case the transient retention of substances normally contained in the urine was conditioned by a hypokalaemic nephropathy. The cause of the hypokalaemia was the vomiting by pylorus stenosis. The histologically ascertained glomerulonephritis had no causal significance for the pathological process.
