[Comparison of residues of glutaraldehyde and formaldehyde in urologic instruments after sterilization in aseptic conditions or in a formaldehyde gas sterilizer].

The aldehyde residues of thermolabile urological instruments, after the formaldehyde sterilization or sterilization in aseptic preparation from a 2% glutaraldehyde solution will be examined and compared. The results are discussed together with experimental toxicologic work from the literature. It is concluded that after sterilization in the 2% glutaraldehyde solution, the glutaraldehyde residues on the experimental materials are safe.

[Aspects of roentgen ray burden in endourologic surgery].

In 1987 a total of 322 endourological procedures with punctual use of x-rays were performed at our ultrasonographic-radiological working table. The personal-dosimetric supervision of 4 surgeons who performed the interventions was analyzed in dependence of our local dosimetric realities. The radiation exposure of the surgeon and the possibilities of supervision and reduction of the exposition are presented.

Influence of growth factors on an insulin-producing cell line (RINm5F).

The influence of IGF-I, insulin and epidermal growth factor (EGF) as well as glucose as control was studied on both growth and function of RINm5F cells, an insulin-producing cell line derived from rat insulinoma tissue. [3H]thymidine incorporation and DNA content (fluorometrically) were measured for estimating cell growth, insulin release and biosynthesis [( 3H]leucine incorporation) as parameters for cell function. There was a significant increase in both [3H] thymidine incorporation and DNA content into RIMm5F cells by glucose already at very low concentrations. IGF-I and high concentrations of insulin too were able to stimulate cell growth (insulin also in additional experiments with isolated rat islets). EGF, however, was without growth effect on this cell line (in contrast to previous own results in isolated islets). Insulin secretion and biosynthesis were also increased by very low glucose concentrations. IGF-I and EGF were ineffective regarding these functional parameters in RINm5F cells. Besides for glucose, our results demonstrate a role for IGF-I and insulin, but not for EGF, in regulating growth of these cells.

Stimulatory effect of insulin-like growth factor-I on [3H]thymidine incorporation, DNA content and insulin biosynthesis and secretion of isolated pancreatic rat islets.

The influence of insulin-like growth factor-I (IGF-I) on both the growth and function of isolated pancreatic rat islets was studied. As a measure of islet cell replication, [3H]thymidine incorporation and DNA content was estimated at the same time as parameters for (pro)insulin biosynthesis and secretion as a measure of the functional capacity of B cells. Incorporation of [3H]thymidine was measured after tissue solubilization; DNA was determined fluorometrically. (Pro-)insulin biosynthesis was determined by incorporation of [3H]leucine after immunoprecipitation and binding to protein A-Sepharose. There was a significant increase in both [3H]thymidine incorporation and DNA content after the culture of islets with IGF-I. IGF-I resulted also in an increase in insulin biosynthesis and secretion from isolated pancreatic islets. These results demonstrate a role of IGF-I in regulating the growth and function of pancreatic islets.

[Clinical value of suprapubic prostate and urinary bladder sonography].

With the help of a prospective study of 100 patients, in whom an operation of the prostate was taken into consideration (TUR, adenomectomy, radical operation), the clinical value of the suprapubic sonography of prostate and urinary bladder is presented. The determination of the size of the adenoma and the residual urine is particularly dealt with. Clinical, transurethral and radiological examination methods served as reference methods, in addition to this the weight of the specimen. The study resulted in a high accordance between sonography and reference methods. The fully painless and undangerous suprapubic sonography and prostate and urinary bladder become an integrated constituent of the urological diagnostics and replaces a large part of hitherto performed transurethral examination methods.

[Tumor marker diagnosis in non-seminomatous testicular tumors using human chorionic gonadotropin (HCG) and alpha fetoprotein (AFP)].

In 28 patients with non-seminomatous testicle tumour the tumour markers human chorionic gonadotropin (HCG) and alpha-fetoprotein (AFP) were determined radioimmunologically and enzymeimmunologically, respectively. While tumours with chorionic carcinoma (n = 8) always were marker-positive, in the embryonic carcinoma in 2 out of 10 cases falsely negative findings appeared. On 5 patients the biochemical monitoring of the course of the testicle tumour disease is demonstrated in detail by means of HCG and AFP and estimated as very helpful method. Advantages and problems of the marker diagnostics are shown and discussed. The positive marker findings were absolutely evident for a metastasation. On the other hand, marker negativation was not always to be equated with absence of a tumour and demanded a further control of the patient by means of all other available methods of diagnostics up to the second-look-operation.

[Sterilization with formaldehyde vapors in a hypobaric procedure: microbiologic and toxicologic aspects].

The use of formaldehyde gas is an alternative for the sterilization of thermolabile instruments. A report is given on the effectivity of a sterilization medium the production of which is not dependent on any imports, and on the suitability of sterile packaging materials produced in the GDR for this process. In further studies the formaldehyde residue on catheters and tubing used in urology following sterilization is measured. The question of whether formaldehyde residue on catheters is of toxicological significance is examined.

[Orienting studies of the formaldehyde residue in urologic instruments following sterilization with formaldehyde gas].

At an orientating investigation the formaldehyde residue at catheters and tubing was defined after formaldehyde gas sterilisation and a varying dependence of the residue of the concentration of the agent solution, the sterilisation time and the tube resp. catheter material was found. The formaldehyde residues lay in the region of micrograms/cm2. They are higher as in the judgement of the equipment producer. The necessity of the investigation of toxicological importance of the residue is mentioned.

[Sterile packing materials for formaldehyde sterilization].

Apart from the already widely applied sterilization by ethylene oxide, the sterilization by gaseous formaldehyde is another reliable procedure for the sterilization of thermolabile instruments. An essential advantage of the sterilization by gaseous formaldehyde over the so-called wet sterilization is that it can be performed in a recontamination-proof sterile packing. Packing materials produced in the GDR, the germ-tightness and toxicological safety of which were already known, were tested to evaluate their suitability for sterilization by gaseous formaldehyde. An appropriate packing variant was found and tested for microbiological reliability.

[Treatment of non-seminomatous testicular tumors in combination with vinblastine/bleomycin and adriamycin/cisplatin].

Following orchiectomy and retroperitoneal lymph node dissection (RND) 11 patients with non-seminomatous testicular cancer were treated alternately with combination chemotherapy consisting of vinblastine/bleomycin and adriamycin/cis-dichlorodiammineplatinum (II) (DDP). A total of 39 course of vinblastine/bleomycin and 36 course of adriamycin/DDP were administered. The two patients with disseminated testicular cancer came into complete and partial remission respectively. 1 patient with stage N3 and minor residual disease after RND, 7 patients with stages N1-2 and complete RND and 1 patient with stage N0, but chorio-carcinomatous portions in the testicular tumour show not sign of recidivation at present. The period of observation after orchiectomy ranges from 4 to 21 months, the median being 13 months. The side effects of chemotherapy are discussed in detail and both combinations are judged to be aggressive. The avoidance of DDP-nephrotoxicity by forced diuresis and the distinct and rapidly beginning intensification of the effect of phenprocoumon due to both regimens are stressed.

[Suppression of testicular testosterone production by irradiation of the testis in prostatic cancer].

Irradiation of the tests of 11 patients with carcinoma of the prostate in the age from 68 to 78 years with 20 Gy (5 X 2 Gy per week) decreased the plasma testosterone levels to an average of 70,3% of the initial values. After operative orchidectomy, however, a decrease of the plasma testosterone level to only 5% the values before operation was observed. Therefore, the operative orchidectomy cannot be replaced by the irradiation of the testes. Higher irradiation doses may cause skin injuries and there is the possibility of being a real carcinogen noxe for the irradiation region too.
