The use of electrodiagnostical investigations became more and more accepted in obstetrics within the last decade; namely rheobase measurements as a means of recognising a threatening premature labour and of checking the effectiveness of tocolytic treatment, and further as an indication of a change in the neurovegetative excitability immediately before delivery. Not only the excitability of the uterine musculature increases before onset of labour but also at the same time, the excitability of the striated skeletal musculature is raised. In obstetrics the electrical excitability usually is tested on the left musculus tibialis anterior but there is a difference in applying the electrodes (unipolar and bipolar technique). Both techniques and the basis of Electrodiagnosis are reviewed in this paper. The advantages and standardisation of the bipolar technique shall be shown.

In 12 primiparae who--on term--did not have spontaneous contractions but a Bishop score greater than 7, Rheobase was measured and there after labour induced by amniotomy. A second evaluation of Rheobase followed one hour after amniotomy. Results show that one hour after rupturing the membranes the Rheobase values decreased significantly (p less than 0,01). The correlation between initial Rheobase values and duration of delivery was still more significant (p approximately 0,001). The possible importance of Rheobase evaluation in the selection of pregnant women on term for elective induction (baby by appointment) is discussed.

Electrodiagnostical determination of Rheobase and Chronaxy has been carried out on 10 women during pregnancy, beginning labour and puerperium. Rheobase and Chronaxy were marked in a Strength-Duration Curves scheme as well as some other points within that scheme. As a base of rise of the gained curves the actual rise at the point of the chronaxy was calculated. Our results show that with all 10 women the t-Difference-Test showed a significantly much lower curve during labour than that obtained during pregnancy. The same applied to the rise of the curve obtained during labour, which was significantly much lower than that during puerperium. There was no significant difference between the curves obtained during pregnancy and puerperium. The significance of the decline of the actual rise at the point of the Chronaxy of the Strength-Duration Curve before starting of labour as a physiological method of functional diagnostic will be discussed there under.
Electrodiagnostical investigations measuring the Rheobase and the Chronaxy have been carried out on 100 women, 36 during pregnancy, 21 during labour, 29 in puerperium and 14 with impending early delivery. As the results show, the rheobase values, which were determined during pregnancy, were significantly much higher, than those during labour or impending early delivery. The rheobase values were significantly lower during 19th--30th week of pregnancy than during 30--40th week, and the latter were significantly much higher, than those during labour or impending early delivery. In puerperium there was no significant difference of the rheobase values to those during labour, but a significant difference to the values obtained during pregnancy. The chronaxy values show no significant differences during pregnancy and labour and during pregnancy and impending early delivery. Only the chronaxy values in puerperium were significantly lower than in pregnancy as well as in labour.
A retrospective study was performed in the context of 1,047 cases of internal foetal monitoring, using direct foetal electrocardiography and/or foetal scalp blood sampling. Seven minor foetal complications have been observed, including six cases of infection and one of haemorrhage, representing a rate of 0.66 per cent. The advantages obtainable from internal foetal monitoring appear to outweigh the low risk associated with these methods.
Including the deliveries in 1956, 1961, 1967, 1970, 1971, 1975, 1976 the development of obstetrics at the 2nd Department of Obstetrics and Gynecology, University of Vienna, Medical School, is shown. The frequency of vaginal operative deliveries was reduced, the indicence of ceasarean sections increased from 1.6 to 6.9%. In premature newborns it was possible to gain a higher birthweight, perinatal and postpartum mortality fell from 4.4 to 2.8% and from 3.3 to 1.5% respectively. The connections between these positive results and the new methods and possibilities introduced at the department are discussed.
A case of Mobin-Uddin-Filter embolisation (28 mm filter) into the right pulmonary artery is presented. Via a right thoracotomy filter and thrombi could easily be extracted. Indication, avoidance of malposition, and the question of whether the filter should be extracted after embolisation are discussed.
Following a description of the three different functions of the placenta (respiratory, nutritive, endocrine) and their failures the most common function tests are presented. Special emphasis is put on the evaluation of a recently described pregnancy-specific protein (SP1) in the maternal serum. The amount of the substance produced by the placenta correlates well with the fetal weight and well-being in cases of EPH-gestosis and idiopathic placental insufficiency; correlation was not as well in mild EPH-gestosis, diabetes and RH-incompatibility. The validity of measuring SP1-production seems to be similar to that of HPL. The method of quantitative determination is simple and possible even in a smaller hospital.

In our experience with five cases of Crohn's disease of the esophagus, the endoscopic appearance has been demonstrated. Corresponding to the basic pathological changes, the findings are very different, but two stages may be differentiated: Stage I in which inflammatory changes predominate as a mild or more often erosive-ulcerative esophagitis. Stage II is a stenosing form similar to a peptic stenosis or to a stenosing tumor. The morphological changes are predominantly limited to the lower part of the esophagus with a tendency to extend to the proximal regions. The diagnosis may be established endoscopically only in special cases with shallow ulcerations within a normal mucosa or with cobble-stone relief which is usually seen in the colon. In all other cases, a specific macroscopical appearance of Crohn's disease of the esophagus does not exist and no specific differentiation is possible from other forms of esophagitis. Only by a combination of endoscopy, radiology and histology can the diagnosis be suspected. Guided biopsies are not able to confirm the diagnosis histologically. The exact diagnosis of Crohn's disease of the esophagus is only possible by histological examination of the resected esophagus.
Ligamentous injuries of the ankle joint can be demonstrated radiologically by special techniques using lateral and A.P. projections. A device is described for obtaining the requisite views. It enables accurate projections to be achieved and a standardised technique is assured by applying a pull of constant weight. The problem of muscle spasm, resulting as a reflex from pain, is discussed.

The authors report a case of silent rupture of uterus during delivery. The whole delivery was monitored by intrauterine pressure recording and fetal scalp electrode. Severe fetal bradycardia induced cesarian section. The retrospectively study of the papercover of fetal monitoring indicates the rupture of uterus.
12 cases of induction of labour with prostaglandin F2 alpha and 8 with prostaglandin E2 were compared with 14 cases in which induction was undertaken with oxytocin. All inductions were successful, the induction--delivery intervals being slightly shorter in the prostaglandin groups than in the oxytocin group. Both with prostaglandin F2 alpha and with prostaglandin E2 the cardiotocogram showed uterine hyperactivity in most of the cases with an unexpected, episodically-occurring increase in basal uterine tone and remarkable tachysystoly. Uterine hyper-activity led to fetal heart rate alterations of the "dip 2" type in about 50% of the cases. According to these results prostaglandins cannot be considered superior to oxytocin for the induction of labour at term.
