[Long-term experience with a PACS subsystem].

Our experiences with a PACS-Subsystem have shown, that it would be more usefull and less expensive, first to install digital archiving systems in single areas. After a certain time of errorfree working the subsstems should be integrated into a superior information system. Fast data networks and userfriendly viewing stations are a basic requirement. Common standards of archiving, independent of the manufacturer, should be considered.

[Clinical staging of malignant tumors by the TNM system. Contribution of modern imaging procedures in patients with testicular tumors].

For classification of testicular tumors in the TNM-System several modern imaging methods are available: sonography, conventional radiography, lymphography, computer-tomography, nuclear magnetic resonance and scintigraphy. Sonography--usually the first method to be used for reasonable classification--is significantly inferior to lymphography and CT for N and M staging. It remains to be seen if the NMR-methods will reach the good informative standard of CT. Only in the case of extensive lymphatic metastasis (bulky disease) are i.v. pyelogram and cavography still used to verify displacement of the ureter or infiltration of the tumor into the cava. In the nuclear medical field bone scintigraphy plays the main role for testicular tumor metastases.

Diagnostic Imaging

[Familial sarcoidosis. 4 cases in 2 families].

Among the patients of the Bundeswehrzentralkrankenhaus Koblenz two new cases of familial sarcoidosis with two cases each became known from Nov. 1977 to Feb. 1978. These cases are reported with typical x-ray-findings. The above mentioned short period of time for finding two new cases of familial sarcoidosis seems extraordinary, as in the world literature on sarcoidosis until today only 114-150 cases are reported. "Sarcoidosis" is described referring to the newer literature. Possible causes for acquiring sarcoidosis are discussed. Specific research by the disciplines social medicine, genetics, and hygiene of the families suffering from familial sarcoidosis is proposed, in order to analyze genetic disposition and/or environmental factors. Long-term supervision of the families afflicted with familial sarcoidosis is proposed.

[Pathophysiology of lung resections (author's transl)].

Changes of adaptation and complications which may appear after extended lung operations must be known both to the surgeon and to the doctor responsible for the after-treatment. They are divided into pulmonary, cardiac, and intestinal early and late sequelae. Pathophysiology, clinical aspects and therapy of the most frequent changes are briefly demonstrated in this paper, sometimes with the aid of our own illustrations and experimental results.

[Radiation monitoring in the area of metallic dental replacement during cobalt 60 gamma radiation and in electron radiation of oral tumors].

In the radiotherapy of malignant tumors of the oral cavity, the exact distribution of radiation dosage in the neighborhood of metallic dental replacement has been uncertain so far. Many authors therefore asked that crowns and bridges be removed before radiotherapy. By means of lithium fluoride thermo-luminescence dosimetry, punctate measurements of doses are possible for the first time. In view of the dose values obtained, removal of metallic dental replacement before the beginning of radiotherapy as a prophylactic measure to prevent radioosteomyelitis appears not to be necessary.

Cobalt Radioisotopes

[Survival time, capacity of action and radiological diagnosis after gunshot wounds of the cranium (author's transl)].

Three cases of suicidal temporal gunshots. Different circumstances in the clinical course and in the ability of action.--In one case there was a bullet tract in the lower temporal region with considerable destruction of the base of the skull, followed by immediate inability of action and early death. Another case with a transtemporal gunshot without radiological signs of a laceration of the base of skull showed severe disturbance of cerebral functions for some days and remaining blindness. In the third case there was a pistol shot in the temporal region with retained missile, full ability of acting and undisturbed consciousness. Causative for the differences in the effect of the gunshots are differences of weapons and amunition and the anatomical position of the wound track in the temporal region. Discussion about the role of concussion of the brain by different rate of transgression of energy from the missile to the skull. Reference to the importance of X-ray analysis and to a radiographic documentation of the findings in such cases.
