Evaluation of anterior knee joint instability with the Rolimeter. A test in comparison with manual assessment and measuring with the KT-1000 arthrometer.

We report on a comparative measurement of anterior knee joint laxity on 30 healthy test subjects and 30 patients with anterior knee joint instability using a new knee testing device (Rolimeter) and the KT- 1000 arthrometer compared to manual assessment of the anterior tibial translation in 30 degrees flexion (Lachman's test). We wanted to establish the reliability of the Rolimeter device compared to manual assessment and to the KT- 1000 arthrometer. The statistical evaluation by means of an analysis of variance (ANOVA) at a significance level of 5% showed no significant difference in the exactitude of measurement between the Rolimeter and the KT-1000 arthrometer, but altogether a significant difference between both instrumental methods compared with manual assessment. Thus the Rolimeter provides an economic, exact and simply operating device for quantifying anterior knee joint instability.

[The semitendinosus tendon as replacement for the anterior cruciate ligament].

This article describes the history, advantages, biomechanical properties, operative technique, and results of replacement of the anterior cruciate ligament using the semitendinosus tendon. After arthroscopic ACL replacement using the doubled semitendinosus tendon in combination with an extraarticular lateral repair in 65 consecutive patients (20 acute, 45 chronic instabilities) we found after 2 years in 51 patients (78%) an excellent or good result with less than 3 mm difference in the Lachman test measured with the KT-1000 arthrometer (MMD). After 6 years 40 out of the 45 patients with chronic instabilities could be examined again. Only 28 patients (70%) showed a difference in anterior translation of less than 3 mm. A positive pivot shift sign could be seen in 11% of the patients after 2 years and in 20% after 6 years. The Tegner score increased from 6.0 after 2 years to 6.2 after 6 years, the Lysholm score decreased from 93 to 91 in the same interval. 89% of the patients had a free range of knee motion after 2 years and 92.5% after 6 years. There were 10% normal, 50% nearly normal, 25% abnormal, and 15% severely abnormal knees in the IKDC evaluation after 6 years. In the last 4 years we are using the quadrupled semitendinosus tendon in an arthroscopic single incision technique for ACL replacement with femoral Endobutton fixation, because the doubled semitendinosus tendon seems to elongate over time. After 23 months on average (12-36 months) the preoperative anterior translation of 7.5 mm could be decreased to 1.9 mm in 35 patients.

[Imaging procedures in the diagnosis of nodular goiter].

A prospective examination of 80 patients with struma nodosa resulted in an approximately equally high reliability of scintigraphy (98.8%) and sonography (97.5%) with regard to the evidence of focal changes. Due to the well-known advantages and the present availability the sonography should above all be used in children, adolescents, pregnant women and iodine-contaminated patients. Since from the scintigram additionally can be reported on the functional condition (autonomous adenoma), this technique cannot generally be replaced by sonography. The advantages and the limits should be known to the physician, in order to avoid unnecessary manifold examinations.

The value of transurethral sonography in tumours of the urinary bladder.

The authors report on their first experience obtained in the field of transurethral sonography in cases of tumours of the urinary bladder. Sonography is a method without complication and risks. TNM classification of urinary bladder tumours is possible. A special advantage of this method of investigation is that sonography can be performed directly before transurethral tumour resection. Moreover, during the postoperative period, sonography can be combined with any kind of control cystoscopic investigation. The determination of the depth of infiltration and extension of the tumour is a prerequisite of therapeutic procedures in cases of cancer of the bladder.

[Ultrasound diagnosis of malignant lymphomas].

The most effective treatment of malignant lymphomas requires an exact diagnosis of the spreading of the tumour in the organism (staging). In this respect the combination of various radiological methods, actually including both sonography and computed tomography stood the test. However, in order to put the ultrasound into the right place of the diagnostic-step programme one has to know the advantages as well as the limitations of this method: the sonography ought to be used at the beginning of the diagnosis and in controlling the success of the treatment of malignant lymphomas.

Codeine and its alternates for pain and cough relief . 4. Potential alternates for cough relief.

In this report-the fourth of a series on codeine and its alternates for pain and cough relief-an attempt is made to evaluate, on the basis of experimental and clinical data, and wherever possible in comparison with codeine, the effectiveness of a number of antitussive substances currently in clinical use. In the discussion of the undesired side-effects particular attention is paid to the risk of dependence and abuse.

This report-the second of a series on codeine and its alternates for pain and cough relief-contains a detailed evaluation of experimental and clinical data on newer substances having analgesic properties comparable to and in approximately the same range as those of codeine. The data are discussed under the headings: analgesic effects in animals; clinical usefulness; side-effects with particular reference to dependence and abuse liability.

This report-the third of a series on codeine and its alternates for pain and cough relief-presents a detailed review of the physiology and pathophysiology of cough, the methods for the experimental and clinical measurement of the antitussive action of drugs, possible mechanisms of action of antitussive agents, and includes a compilation of experimental results and clinical experience with codeine as an antitussive.

Codeine and its alternates for pain and cough relief. 5. Discussion and summary.

This chapter concludes the survey of experimental and clinical data on the analgesic and antitussive properties of codeine and its potential therapeutic alternates. From an evaluation of their effectiveness on the one hand and the side-effects, including tolerance, dependence and abuse liability on the other, it would appear that the therapeutic goals of codeine could be achieved by other substances, except perhaps where analgesia, cough relief, and sedation are required simultaneously. The use of these other substances would, however, result in no particular gain and probably no particular loss.

Codeine and its alternates for pain and cough relief. I. Codeine, exclusive of its antitussive action.

This report-the first of a series on codeine and its alternates for pain and cough relief-presents a detailed evaluation of experimental and clinical data concerning the analgesic action of codeine (the antitussive action will be assessed separately). The authors discuss the pharmacology of the drug, including side-effects and toxicity; effects on the respiratory, circulatory, digestive and urinary systems; tolerance, dependence and liability to abuse; metabolic effects; and mechanism of action.Though codeine is generally more toxic than morphine to animals on account of its convulsant action, it is less toxic to man, possibly because it produces less respiratory depression. Again, tolerance to its analgesic effects has been demonstrated in several animal species, but dependence in man is observed far less frequently than it is with morphine, and the abstinence syndrome is less intense. From their extensive review of the evidence available, the authors conclude that codeine is a good analgesic and that little risk to public health is likely to arise from its clinical use to relieve pain.
