Managing digital mucoid cysts by cryosurgery with liquid nitrogen: preliminary report.

Fourteen patients with digital mucoid cysts were treated by cryosurgery with liquid nitrogen using an intermittent spray technique. The cysts were punctured and the contents expressed preoperatively. Treatment was well tolerated without local anesthesia. Cysts ranged in size from 0.4 to 1.5 cm. Average follow-up was 18.6 months. Twelve patients were cured to date, in some instances after many prior unsuccessful attempts with other treatment modalities. In one patient a cyst recurred after 10 months and was retreated by cryosurgery. In another patient a much smaller cyst recurred after 12 months. Cosmetic results were gratifying, with absent or minimal scarring. From these preliminary observations it may be concluded that liquid nitrogen cryospraying of digital mucoid cysts is a valuable therapeutic modality in these notoriously recalcitrant lesions.

Granuloma annulare with follicular perforation.

An instance of perforating granuloma annulare was observed in a 42-year-old female patient in which the necrobiotic material in the dermis was eliminated by a hair follicle. This had previously been described by the author in only 1 other case of perforating granuloma annulare. Transepithelial elimination may occur by the transepidermal and/or transfollicular route. Both pathways are possible in perforating granuloma annulare. Some factors are discussed which may be responsible for the predominance of one route over the other in various disorders of transepithelial elimination.

[Generalized perforating folliculitis in chronic kidney insufficiency].

A 24-year-old female patient is reported, in whom severe renal insufficiency following renal transplantation for Goodpasture's syndrome was associated with a persistent generalized perforating folliculitis of the trunk and extremities. In addition to the pathologic renal parameters a hypervitaminosis A was found. On the basis of this observation as well as similar recent reports in the literature it is suggested that a subset of an unusually extensive perforating folliculitis may occur with renal insufficiency and, albeit rare, represents a clinical marker for this associated condition.

[Unilateral nevoid telangiectatic syndrome].

In recent years the unilateral nevoid teleangiectasia syndrome (UNTS) has been mainly reported in the Anglo-American literature. This rare disease is not well-known in the German literature, where hitherto it has not been published. Based on the observation of a new case the clinical and histologic features of UNTS are reviewed and the pathogenesis discussed in detail.

[Nosologic position of demodicidosis in humans].

In 20 patients with rosacea and rosacea-like eruptions Demodex folliculorum mites were found in high numbers within lesional skin. In one patient Demodex folliculorum could be demonstrated in pits and burrows in the epidermis of the scalp. In other instances the mouth parts of the mites were closely related to the follicular epithelium. Recent parasitologic findings showed that in spite of their name (Demodex = "sebaceous worm") the mites derive their nutritive requirements from cellular proteins obtained by epithelial destruction. To clarify the pathogenetic significance of hair follicle mites in man as suggested by these histological findings as well as their known ability to form organisational centers of foreign body granulomas, further epidemiologic and immunologic studies are needed. Topical antiparasitic agents did not offer any therapeutic advantage over traditional preparations used for rosacea and seborrheic dermatitis.

[Dowling-Degos disease with involvement of the scalp].

Dowling-Degos disease is described in a 40 year-old man, involving the nape of the neck, axillary regions, scrotum, intergluteal fold and also the scalp. There was no associated hair growth anomaly. There was extensive pitting of the entire face. Dowling-Degos disease appears to result from a genetically determined imbalance of epithelial-mesenchymal regulatory mechanisms causing increased epithelial proliferation, as well as a disturbance of the maturation and differentiation of the proliferating keratinocytes.

Granuloma annulare with transfollicular perforation.

In recent years the phenomenon of transepithelial elimination has been found to be the underlying mechanism in a variety of skin disorders. "Perforating dermatoses" may show transepidermal or transfollicular elimination or a combination of the two. A case of papular granuloma annulare is reported, where in one lesion the pilary apparatus actively participated in the elimination of necrobiotic collagen from the dermis. To our knowledge, perforating granuloma annulare with a transfollicular pathway has not previously been described. Also, a classification of perforating dermatoses is presented.
