[Patella dislocation. Therapeutic principles and prevention of late complications].

In this review we have compared the different kinds of treatment of acute dislocation of the patella regarding the effects and the complications. The literature describes a variety of treatments and no standard treatment exists. Most studies are not randomised prospective studies and the follow-up period is often short. We conclude that there is a need for randomised, prospective studies with a large number of patients and with a long follow-up period in order to find the best kind of treatment. Treatment modalities for acute as well as recurrent dislocation are described in the general section of the review. In order to achieve the best results and decrease the number of complications it is necessary to have a clear diagnosis of APD and knowledge of predisposing factors.

[Acute and chronic patella dislocations. Therapeutic strategies in Danish orthopedic departments].

BACKGROUND: In review of the literature there is no standard treatment of acute- as well as recurrent dislocation of the patella. Most investigations are retrospective or not randomised. Approximately 1500 patients/year with the diagnosis luxatio patellae are seen in Danish hospitals. METHODS: Using a questionnaire we wished to find out how these patients were treated in Danish orthopaedic departments. RESULTS: We found that there was no consensus on the treatment of patellar dislocation. CONCLUSION: We concluded that there is a need for prospective randomised studies involving a large number of patients, with a long follow up time.

[Spontaneous rupture of the left external iliac vein. A complication of deep vein thrombophlebitis?].

A 52 years old woman was hospitalized on suspicion of left femoral venous thrombosis. Within 24 hours she went into shock, and laparotomy was performed on suspicion of a ruptured abdominal aortic aneurysm. The operative finding was a ruptured iliac vein, which was repaired and anticoagulant treatment was started. Unfortunately the patient never stabilized and died from cardiovascular collapse within 12 hours.

[Intraabdominal hemangiopericytoma. Diagnosed during surgery for suspected acute appendicitis].

A 39 year-old febrile man with lower abdominal pain and peritoneal reaction was operated on suspicion of acute appendicitis. During the past eighteen months he had been hospitalized twice due to similar symptoms, both times with spontaneous remission. The operative finding was a large multilobular vascular tumour which on pathological examination was a haemangiopericytoma, with no degree of malignancy. The tumour was removed in toto.

[Skating injuries. A study of an injury sample in a Danish community skating rink].

A review is presented of injuries sustained at an ice rink and treated at the emergency room, Odense University Hospital in Denmark, during 1993. Of the 105 patients treated, 67 (64 per cent) were 10-19 years of age. Most patients had been injured during a fall, no other participant being involved. About 25 per cent of the injuries were lesions the face or head. In seven cases the patient had been cut on the hand by the blade of another participant's skate, two patients had tendon lesions and in 16 cases the patient had sustained a fracture of the forearm or hand as the result of a fall. Many of the injuries could have been prevented by the use of protective gear (helmet, gloves and knee and elbow pads).

[Tibial pseudoarthrosis. Treatment using the Ilizarov technique].

In the period from the 1.1.1991 to the 31.5.1993 17 patients with a tibial pseudoarthrosis were treated by the Ilizarov method. The mean age was 35 years (13-88 years). Mean time from fracture to operation for pseudoarthrosis was 14.6 months (3-39 months). There were two hypertrophic, 14 atrophic and one defect pseudoarthrosis. Six of the pseudoarthroses were infected. After stable external fixation the treatment was compression over the pseudoarthrosis in four cases and alternating distraction and compression in eight cases. In five cases the treatment was supplemented by bone transport for the defects between 15 and 50 mm. Mean time of treatment with the external fixator was 5.2 months (2-11.5 months). The overall treatment time was 9.8 months (3-19 months). At the follow-up 14 pseudoarthroses were fully consolidated, one patient was still using an orthosis, and three patients were in need of reoperation with bone transplantation. During the period of fixation thirteen patients were hospitalized for short periods, and some of them several times. The treatments were for correction of the distraction, replacement of fixation of pintrack infection or treatment of pain. The Ilizarov method of treatment of pseudoarthrosis show a good stimulation of healing, but experience with the fixator system and aggressive treatment of various minor complications are essential for a successful outcome.

[Bone lengthening of the extremities using the Ilizarov method].

We describe the indications and the results in 18 bone lengthenings carried out on 14 patients in our department during the period January 1991 to September 1993. Seven patients had limb length inequality, five patients had dysplasia or aplasia, one patient had Turner's syndrome and one patient had suffered traumatic amputation of all the fingers of one hand. The mean lengthenings achieved were: femur 4.9 cm; tibia 5.6 cm; fibula 1.5 cm; humerus 7.5 cm; ulna 3.5 cm; radius 1.9 cm; and metacarpals 1.6 cm. There were 14 cases of pin-problems which needed surgery. Nine cases of infection around the pin holes were treated with antibiotics only. We had five cases with severe pain due to distraction. We had two cases of fracture after removal of the external fixation. Both fractures healed without further problems, but in one case there was three cm loss of the achieved lengthening. Our conclusion is that the Ilizarov method for bone lengthening is a demanding procedure. Because of the many--often minor--complications the patients must be seen frequently, as one must be prepared for "aggressive" treatment of the problems.

The age-dependent incidence of injuries due to road traffic accidents in Odense, Denmark from 1980 to 1992.

The study was based on data concerning persons treated at Odense University Hospital as a result of road traffic accidents in the period 1980-92. Incidence rates of road traffic accident injuries were calculated on the basis of the population in Odense municipality. The study group included persons older or even 65 years of age, while persons younger than 65 years of age were used as a reference group.

Diagnosis of scaphoid fractures. A prospective multicenter study of 1,052 patients with 160 fractures.

In a prospective multicenter study of 1,052 patients with clinical signs of a scaphoid fracture, mammographic films and fine intensifying screens were used at the radiographic examination. 5 standardized projections including 3 special projections focused on the scaphoid were taken. 150 fractures were diagnosed at the first examination but in 10 cases the fracture was first diagnosed at a second radiographic examination after 10-14 days. The second examination still seems mandatory despite the use of high quality radiographs with optimal spatial resolution and contrast, and the value of supplementary special projections.

[Poisoning accidents with household chemicals among children].

A review is presented of the registration of all poisoning accidents among children aged 0-6 years treated at the University Hospital, Odense, Denmark during the period 1.1.1980-31.12.1992. There were 1751 poisoning accidents of which 482 were accidents with household chemicals. There were 69 accidents with lamp oil (Petroleum) of which 67 were in the age group 0-3 years. A peak incidence in the age group 0-3 years old is seen in 1986. In 1992 the incidence is at about the same level as in 1980. The incidence in the age group 4-6 years is at almost the same level throughout the entire period. We conclude that it is necessary to continue with campaigns to prevent accidents with household chemicals among children.

[Aquatic injuries].

The pattern of injury sustained in an aquascape is described. The most serious and the highest number of injuries were sustained in the water chute (head, face). 15 percent of the injuries were bone or dental fractures. Prophylactic advice is given.
