[Ointment treatment of 1st degree hemorrhoids. Comparison of the effectiveness of a phytogenic preparation with two new ointments containing synthetic drugs].

OBJECTIVES: Investigation of the usefulness of a vegetable-based hemorrhoid ointment in comparison with two other ointments containing synthetic substances, one of which additionally contained corticoid. STUDY DESIGN: Treatment of 90 patients with first-degree hemorrhoids in the surgical department of a University Polyclinic, within the framework of a double-blind three-limb study of 21 days duration. Follow-up examinations on the third, seventh, fourteenth and twenty-first day of treatment. TARGET CRITERIA: Four typical symptoms--pruritus, bleeding, burning sensation, sore sensation--graded by both physician and patient. RESULTS: All three ointments proved highly effective. Both during the course of treatment and at the final examination carried out on the 21st day, no major differences were to be found between the three treatment groups. In the case of some of the test parameters, a positive tendency in favor of the herbal ointment was observed.

[Cryosurgical treatment of polypi of the rectum and colon (author's transl].

UNLABELLED: Single cases of genuine colon rectum polypi, histologically adenomatous, adeno-villous and villous tumours have been treated with cryosurgery. INDICATION: Relapses after polyectomy showing malignant degeneration; malignantly degenerated polypi, treatment of the basis of sessile lesions; sessile polypi up to 1 cm in diameter; extensive polyposis--polyposis of the rectum. After an initial regression of the polypous changes growth can be stated and quickly developing degenration leading to the formation of a carcinoma of the rectum and to stenoses of the lumen. It was only partially possible to clarify the cause of the growth in animal experiments. Primary application of low temperatures in polyposis recti and in primarily malignant degenerated polypi have to be avoided.

[Various cytological foundations and consequences of cryosurgery].

The methodically and strictly localized destruction of tissue presumes a precise knowledge of the physically conditioned and cytologically provable alterations after the influence of deep temperatures. According to generally effective principles of cold-biology it is referred to organspecific and functional-anatomic particularities which have to be considered in the cryosurgery.

[The functional behavior of peripheral circulation in cryogenic surgery].

The reactive behaviour of little vessels of the circulatory periphery against the effect of extremely deep temperatures was examined in a sense of a microflow measurement by means of the intravital microcinematography. The results obtained by mathematical analysis of the single figure demonstrate that the vessels of microcirculation are remarkably resistant against rapid withdrawal of heat. In this way the quickly recovered blood-flow in the periphery of circulation can reduce detrimentally the time of tissue thawing treated by cryosurgery. Both the shortening of the thawing phase after cryosurgery conditioned by hemodynamics and the development of a cold edema can be met by experimentally confirmed adrenergic influence of the vasoconstriction under local anesthesia.

[Diagnostics and therapy of benign bone tumours (author's transl)].

The diagnosis of benign bone tumours must be confirmed histologically; only in rare cases this can be omitted. After having identified the kind of tumour, a specific therapy must be established, covering a wide range, like excochleation as well as amputation. Malignant degeneration and high rates of relapse are possible.

[Cryosurgery in carcinomas of the rectum and anus (author's transl)].

3 years of clinical experience in cryosurgical treatment of carcinomas of the rectum and anus and of malignant polypi is reported. Nearly all patients were in so a poor condition not to underwent conventional operations. In these cases cryosurgery offers an alternative to the usual local procedure of tumour resection.

Anus Neoplasms

[Rectocolonic polyps (author's transl)].

After presenting a classification of colon polyps the author comments on a follow-up group of 200 patients. Only genuine tumours have been taken into consideration. The rate of malignant degeneration is very high. Each determined polyp has to be removed totally to achieve prophylaxis. Postoperative control of the patients is an imperative demand for their lifetime.
