Oxidative stress in the fetal lamb brain following intermittent umbilical cord occlusion: a path analysis.

OBJECTIVE: To evaluate the relative contribution of cord occlusion length intervals between occlusions and experimental duration on oxidative stress in the fetal lamb brain. DESIGN: Acute, partially exteriorised fetal lambs with intermittent total cord occlusion. SETTING: The Vivarium of Westmead Hospital, University of Sydney, Australia and The Chinese University of Hong Kong. MAIN OUTCOME MEASURES: Arterio-venous differences in the concentration of organic hydroperoxides, measured in paired samples of carotid arterial and jugular venous blood, as an index of oxidative stress in the brain. METHODS: Thirty-two fetal lambs were exposed to graded hypoxia, induced by intermittent total umbilical cord compression of 30 seconds, 60 seconds and 90 seconds duration, occurring every minute for a total of 27 occlusions over 81 minutes. Three sham experiments were also performed. In addition to organic hydroperoxides, carotid arterial blood samples were also assayed in 15 animals (two sham) for oxygen saturation, pH, hypoxanthine, xanthine and urate concentrations. A causal model for oxidative stress was defined: occlusions leading to hypoxia with a rise in hypoxanthine; reperfusion during intervals between occlusions leading to the accelerated production of xanthine and uric acid and the generation of oxygen free radicals, which in turn, are responsible for the rise in lipid peroxidation. Path analysis was performed to assess the strength of the relationships between these variables and the cord occlusion length, the interval between occlusions and the duration of the experiment. RESULTS: Sham experiments showed no change in organic hydroperoxide production. Following 30-second umbilical cord occlusions a sixfold drop in mean organic hydroperoxides was observed between carotid arterial and jugular venous levels. In contrast, following occlusions of 60 seconds duration (or longer) a median 20-fold increase in organic hydroperoxide production was observed. Path analysis revealed a strong indirect pathway from occlusion length --> hypoxanthine --> urate and weak positive pathways from oxygen saturation --> urate and from interval between occlusions --> urate. After accounting for these pathways reflecting oxidative stress, a strong direct path remained from time from first occlusion --> organic hydroperoxide production. CONCLUSIONS: Peroxidation of lipids in the brain occurs under conditions of severe hypoxia and reperfusion associated with intermittent umbilical cord occlusions of 60 seconds or longer. The path analysis supported the causal model as originally defined, with the exception that the indirect pathway via pH was found to be trivial.

A randomized trial of two preparations of vaginal prostaglandin for pre-induction cervical ripening.

OBJECTIVE: To compare two prostaglandin (PG) E2 preparations for pre-induction cervical ripening in a randomized clinical trial. METHODS: Two milligrams of vaginal PGE2 gel was compared with a vaginal PGE2 3-mg tablet in 200 nulliparous women. Outcomes assessed were induction failure, need for labor augmentation, pain relief requirements, fetal heart rate (FHR) abnormalities, operative delivery rate, induction-to-delivery interval, neonatal condition, and occurrence of uterine hyperstimulation. RESULTS: There was no statistical difference in pre- and post-dose cervical scores. Compared with the tablet group, women in the gel group were more likely to have significant FHR abnormalities in early labor (odds ratio [OR] 4.77, 95% confidence interval [CI] 1.15-19.5) requiring cesarean delivery. Fetal heart rate tracings in the active phase of labor were also more likely to be abnormal in the gel group (chi 2 = 4.31, P < .05). Compared with the gel group, women in the tablet group were significantly more likely to require operative delivery for poor progress in labor (OR 2.83, 95% CI 1.20-7.24). Other clinical outcomes were identical, with no significant differences in the overall rate of failed induction, cesarean delivery, rate of assisted delivery, requirement for oxytocin infusion, induction-to-delivery interval, pain relief requirements, or neonatal condition. CONCLUSIONS: When compared with the PGE2 tablet, the use of PGE2 gel for cervical ripening and labor induction in nulliparous women did not result in significant improvements in labor outcome. Whereas the gel was associated with an increase in significant FHR abnormalities, the tablet was associated with an increase in the rate of operative delivery for poor progress in labor.

Epidemiology and aetiology of acute bronchiolitis in Hong Kong infants.

The epidemiological, clinical and virological features of 1220 children with acute bronchiolitis admitted to the Prince of Wales Hospital, Hong Kong, from 1985 to 1988 are reported. They accounted for 6.6% of total paediatric admissions and provided a case incidence of bronchiolitis requiring admission to hospital of approximately 21 per 1000 children 0-24 months of age. The clinical course and outcome was in general benign. The average hospital stay was 5 days and there were no deaths. Ten per cent of patients were repeatedly admitted to hospital with recurrent wheezing after discharge. Two infants developed bronchiolitis obliterans. Respiratory syncytial virus (RSV) was shown by direct immunofluorescence, virus culture and serology to be the commonest cause of acute bronchiolitis in Hong Kong. Other aetiological agents included parainfluenza and influenza viruses, adenoviruses, and Mycoplasma pneumoniae. In contrast to western countries, a seasonal variation of bronchiolitis was found with a peak incidence in the summer months. The significance of these observations is discussed.

Assessment of cardiotocographs.

In recent years advances in medical electronic equipment for monitoring, diagnosis and treatment of patients have led to a large increase in the number and variety of instrumentation available to the medical profession. There is a considerable amount of duplication of equipment and in the absence of readily available information buyers are unlikely to make informed decisions about the ideal instrument for their particular circumstances. One method of increasing the users' awareness is a comparative, independent assessment of equipment, with the results disseminated to the interested parties. This paper describes the essential qualities of cardiotocographs: how they are assessed as part of the UK Department of Health's evaluation programme and the measures to inform users of the latest evaluation information.

Nasopharyngeal carcinoma and Epstein-Barr virus--associated serologic markers.

The association of NPC and EBV is well established. The EBV-specific serologic profile of patients with possible NPC is valuable for the clinical assessment of the disease. We review the immunologic responses of patients with NPC to the different EBV-specific serologic markers. The application of these markers in the diagnosis, early detection, and prognosis of the disease is also discussed.

Relation between aging and rated teaching effectiveness of academic psychologists.

Within a combined cross-sectional and longitudinal design, several analyses showed that student-rated teaching effectiveness in university professors declined with age. For 106 full-time psychology teachers, aged 26 to 55, who were studied for time periods ranging from 2 to 15 years, an overall negative correlation of r = -.33 (p less than .05) was found between age and general teaching effectiveness. A similar decrement with age occurred for items measuring specific teaching behaviors. Age accounted for 8% of the variance in general teaching effectiveness.

Persistent high fever and gall-bladder wall thickening in a child with primary Epstein-Barr viral infection.

A 3.5 year old girl presented with a history of high fever, rigors, and mild cough for 1 week. Physical examination revealed normal chest findings but gross hepatomegaly was detected. Liver function tests were abnormal and indicated biliary obstruction. Ultrasonography revealed a distended gall-bladder with increased wall thickness up to 0.6 cm. The diagnosis of primary Epstein-Barr viral infection was eventually made by specific serological study. The patient's fever subsided 2 weeks later and her liver function tests returned to normal 1 month later. Abdominal ultrasonography at this time was normal.

Prevalence of cytomegalovirus cervical excretion in pregnant women in Hong Kong.

In 1986 a study was made on the prevalence of cervical colonization by cytomegalovirus (CMV) in normal third trimester pregnant Chinese women in Hong Kong. Seven out of 95 unselected subjects had positive CMV culture, making a prevalence of 7.4%. This is a low prevalence when compared with the prevalence found in other people of Asian origin and similar to the prevalence found in the Caucasians.

The fetal electrocardiogram: current clinical developments in Nottingham.

The fetal electrocardiogram (FECG) has been recorded from 155 women in labor using a fetal scalp electrode and a minicomputerized enhanced averaging technique. Fetal hypoxia, acidosis, and stress have been diagnosed by analysis of blood from the fetal scalp, and post delivery from full gas analysis from blood taken from the umbilical artery and vein. Umbilical venous levels of serum lactate, adrenaline, noradrenaline, potassium and hemoglobin were also determined. A P wave was seen to be present in all cases recorded. The P-R interval correlated both with fetal autonomic nervous activity, and with the R-R interval of the healthy fetus. The P-R--R-R interval correlation altered with increasing fetal acidosis. QRS complex changes were varied and not usually related to fetal condition. ST segment changes occurred with increasing fetal acidosis. T wave analysis although incomplete, has shown no obvious correlation with fetal condition.

Study of discrepancies in rubella haemagglutinin titrations and a reappraisal of diluents used in the rubella haemagglutination inhibition technique.

To elucidate inconsistencies in rubella haemagglutinin assays the components of the assay technique were examined. The results of carefully controlled assays of rubella haemagglutinin antigens from different sources in various plates and diluents with four species of indicator cells are reported. The quality and quantity of gelatin in the dextrose-gelatin-veronal buffered diluent commonly used in rubella haemagglutinin assays had a profound effect on the haemagglutination pattern and antigen titre. The veronal buffered saline used in the complement fixation test offered a valid alternative to the more complex diluents incorporating gelatin currently used in rubella haemagglutinin assays and haemagglutination inhibition tests.

A micro immune haemolysis test for rubella antibody screening.

A micro test for the detection of rubella IgG antibody by passive haemolysis in free solution (MIH) is presented. The technique is reproducible, sensitive, simple to perform and economical in the use of reagents. Identical results were obtained in all but one of 408 tests on serum specimens screened for rubella antibody at 15 i.u./ml by the MIH and haemagglutination inhibition (HI) techniques. The quantitative potential of the MIH test and the relative merits of the MIH, HI and radial haemolysis (RH) procedures for rubella screening are discussed.
