[Fracture load of ceramic ball heads of hip joint prostheses].

To ensure the stability of ceramic ball heads of hip joint prostheses over the long term, both standards for the material and FDA-regulations for the components have been established. In this paper the philosophy underlying design and reliability is discussed. On the basis of fracture loads determined for Biolox ball heads the high level of stability and reliability that has been achieved and can now be guaranteed, is demonstrated.

[Type A behavior--a uniform construct?].

Single dimensions of the Type-A-behaviour pattern and their contributions to the etiology and pathogenesis of cardiovascular and psychosomatic diseases are very often discussed. Our studies are concerned with the analysis of 40 blue collar workers and patients (40 patients with hyperuricemia, 36 psychotherapy patients) in definite groups of age. By item analysis and multivariate statistical procedures (variance, factor and discriminant analysis) we found an overlapping effect of psychosocial factors of work and on the other hand the influence of neurotic symptoms and behaviour patterns.

Pathophysiology of subglottic tracheal stenosis in childhood.

Two particularly problematic clinical entities are chosen from the many possible causes of the pathophysiology of subglottic tracheal stenoses in childhood: malformations of the "hard" and "soft" types of stenosis occurring typically as primary lesions; and stenoses caused by trauma (following intubation or faulty tracheostomy) as the most common secondary lesions. It is shown with reference to experimental investigations that subepithelial layers play the leading role in the origin of stenoses, in contrast to hitherto existing ideas based on clinical observations. Traumatic stenoses caused by unphysiological mechanical stressing show striking parallels to those in other age groups, suggesting the conclusion that a reaction of the mesenchymal tissue layers of the airway regions in question is a typical response to mechanical forces.

[Lasermicrosurgery of benign lesions of the vocal cord and functional results (author's transl)].

There exist two different lasers for the endolaryngeal microsurgery: Carbon-dioxid-Laser (lambda = 10,6 micrometer) and Neodymium: YAG-Laser (lambda = 1,06 micrometer). We have performed with the Nd: YAG-Laser mediLas experimental studies with animals as well as clinical investigations. Now we do endolaryngeal lasermicrosurgery in selected cases as a method of treatment. The surgical method is reported. Our number of cases as well as the follow-up-study allow us to give a first statement upon the functional results obtained by lasermicrosurgery of benign vocal-cord-lesions.

Closing and opening pressures in the intrapulmonary airways of rats.

Pressures at which the peripheral airways (PA) and terminal bronchioles (TB), open and closed, were studied in rats' lungs in vitro. Airway luminal diameter was directly measured at diminishing transbronchial pressure (closing) and increasing transbronchial pressure (opening) by means of a stereomicroscope with a calibrated eyepiece. The PA and TB had internal diameters at 100 mm H2O ranging from 173 to 306 micrometer and 122 to 204 micrometer, respectively. Most of the airways closed suddenly in the pressure range between 75 and 5 mm H2O and only a few were still open at zero pressure. Closure began in the distal portion of the PA and mid-portion of the TB usually in a concentric manner. Terminal bronchioles and PA that initially were prepared at room temperature instead of at 0 degrees C often contained air at zero pressure and showed a decreased response to acetylcholine. Subthreshold concentrations of acetylcholine increased the critical closing pressure by more than 100%. The critical opening pressure was considerably higher than the corresponding closing pressure. It is proposed that bronchiolar closure depends on the tone of the airway smooth muscle, the elasticity of the airway wall and probably on the surface tension of the liquid lining the mucosal membrane.

[Laser surgery of the vocal cord. Clinical investigations with a view to treating small circumscribed carcinomas of the larynx (author's transl)].

It is reported on endolaryngel laser irradiation prior to operation, carried out, with varied dosage, on tumour-free areas of vocal cords of patients with larynx carcinoma. In these instances, the Nd:YAG-Laser MediLas (Messrs. MBB) with flexible light delivery, which we already used in earlier tests, was, for the first time, furnished with a laser endoscope--a special construction by Messrs. Karl Storz, Tuttlingen. The alterations caused on the vocal cords were investigated endoscopically and morphologically. It was proved that a laser induced lesion at a high dosage (55 W over 6 sec or 9 sec resp.) is capable to lead to a destruction of a carcinoma infiltrating into the vocal cord body.

Experimental studies of Neodym-YAG-laser microsurgery of the vocal cords in dogs.

In animals the endolaryngeal application of Laser rays of the Nd: YAG-Laser MediLas was tested under microsurgical conditions, the early and late alterations of the vocal cords were endoscopically observed and histologically studied. Laser surgery has the special merit of permitting a touchless and bloodless operation. The experiences with the Nd: YAG-Laser MediLas encourage us to use this procedure in clinical tests for the therapy of benignant squamous epithelium hyperplasia of vocal cords.

Morphological and physiological findings on hair cells with impaired metabolism (nitrogen, monoiodoacetate) in Caiman crocodilus with reference to sudden deafness.

Morphological and physiological studies were carried out on Caiman crocodilus under experimentally produced metabolic changes. During N2-respiration the cochlear potentials responded differently. The negative component (CM-) of the cochlear microphonics decreased continuously, whereas the CM+ component showed only a little change. The summation action potential (AP) exhibited a similar behaviour to that of CM-. These alterations were reversible up to periods of 30 min N2-respiration. The morphological findings, after N2-respiration, showed an intracellular oedema of the hair cells. The afferent synaptic contacts are always recognizable. Different degrees of disintegration within the presynaptic structures were seen. Efferent axosomal and axodendritic synapses were unchanged. MIA-perfusion led to an irreversible decrease of the cochlear potentials (CM+; CM-; AP) and morphologically to a total destruction of the papilla basilaris. The changes in the presynaptic structures during anaerobic metabolism offer the possibility of a new explanation for the reversibility of sudden deafness.

[Laser surgery of the vocal cord. An experimental study (author's transl)].

In animals the endolaryngeal application of laser rays of the Nd: YAG Laser MediLas (lambda = 1.06 mum) was tested under microsurgical conditions, the early and late alterations of the vocal cords caused by a dosage of 17 watt/mm2 fractioned over 7 sec were endoscopically observed and studied by light- and electronmicroscopy. A circumscribed vocal cord edema induced by laser entails no functional impairment whatsoever. Histologically the laser lesions can be interpreted as epithelial defect with a reactive inflammation of the adjacent connective tissue. A fundamental loss in substance cannot be realized. The superficial defect healed within 18 days; after 7 weeks the zone of regeneration and the undamaged part of the vocal cord could no longer be differentiated neither by light- nor by electronmicroscopy. Laser surgery has the special merit of permitting a touchless and bloodless operation. These experiences with the Nd: YAG Laser MediLas encourage us to use this procedure in clinical tests for the therapy of benignant squamous epithelium hyperplasia of vocal cords.

[Biometrical fresh volume analysis of nucleus caudatus, putamen and globus pallidus in an ontogenic series of Tupaia belangeri].

The fresh volumes of the nucl. caudatus (including the nucl. accumbens septi), putamen, globus pallidus and of the whole brain of 37 male Tupaia belangeri aged between 36-536 days of ontogenesis have been determined. The growth of these nuclear regions is described by the logistic function. The parameters were approximated by means of an iterative procedure by the least square method. The "ideal" volume of the globus pallidus is 9,6 mm3, of the striatum 113 mm3, and of the whole brain 3 260 mm3. The degrees of maturity of globus pallidus, striatum and the whole brain do not differ significantly from each other concerning the date of birth (43th day of ontogenesis), the growth factors are about 5,5. The nucl. caudatus, including the nucl. accumbens septi, and the putamen demonstrate almost the same course in their degrees of maturity and growth dynamics within the tested age interval. In the course of postnatal development the globus pallidus grows more quickly than the whole brain, and the whole brain, on the other hand, more quickly than the striatum. The half value time of the globus pallidus is about 53 days of ontogenesis, of the whole brain about 56 days of ontogenesis and of the striatum about 58 days of ontogenesis. On the 70th day of ontogenesis, the difference between the degrees of maturity of globus pallidus (93 per cent) and striatum (= nucl. caudatus + nucl. accumbens septi + putamen) (72 per cent) is 21 percent. This complicated growth dynamics can be seen exactly by considering the non-linear logistic functions, the degrees of maturity, and the growth-rate of the degrees of maturity.

Age Factors

Effects of highly dosed ultrasound on the growing rabbit's larynx. Scanning electron microscopic and reflecting ligth microscopic findings.

The morphologically recognizable effects of highly dosed ultrasound application on the growing rabbit's larynx (5 Watt/cm2 for 10 min) were studied by means of scanning electron microscopic (SEM) and reflecting light microscopic (RLM) methods. It could be shown that the area of cartilaginous necrosis corresponded exactly to the location of ultrasound application and was sharply limited by the normal cartilage. After a survival period of six weeks, areas of chondroneogenesis became visible on the endolaryngeal perichondrium. After a survival period of twelve weeks, an outer zone of cartilaginous necrosis was still to be seen. Connective tissue directly affected by ultrasound and the endolaryngeal muscles did not show any retarded damage. The results of our experiments permit to state that highly dose ultrasound application on the cartilaginous framework leads to a growth retardation with a subsequent alteration in form and shape.

Influence of temperature and decreased water content of inspired air on the ciliated bronchial epithelium. A physiological and electron microscopical study.

Experiments have been carried out on the bronchi and intrapulmonary airways of rats in vitro, to determine the physiological effects of air with water content (WC) of: 2.19 mg/1 (5% RH (relative humidity) 37 degrees C), 8.77 mg/1 (20% RH, 37 degrees C), 24.13 mg/1 (50% RH, 37 degrees C) and 30.71 mg/1 (70% RH, 37 degrees C) on ciliary beat frequency; and also, the morphological effect of air at temperatures of 30, 40 and 48 degrees at a constant WC of 28.0 mg/l on the ciliated epithelium. The results showed that (i) ciliary beat frequency decreased with decreasing WC and irreversible ciliostasis was reached sooner, the lower the WC value; (ii) ciliary beat frequency was not significantly affected above 24 mg/l; (iii) 45 min and 90 min exposure at 30 degrees C caused an enormous increase in the depth of the mucus film. The cilia and the epithelial cells did not differ from the controls at 40 degrees C; (iv) in airways exposed for 45 min at 48 degrees C, the cilia appeared to be in total disarray. Some were clustered together in whirling arrangements, while others were detached from their bases. The substructure and the cytoplasm of the cells on the surface and in the deeper layers, appeared to have been destroyed; (v) after 90 min at 48 degrees C, the damage to all structures had progressed. Only isolated tissue or cytoplasmic elements were perceptible.
