Morphological findings in 70 kidneys of living donors for renal transplant.

Seventy donor kidneys for transplant were studied with light microscopy (LM), electron microscopy (EM) and immunofluorescence (IM) for C3, C4, Clq, IgG, IgA, IgE, IgM, and antifibrin; the samples were taken just before transplanting the allograft kidney. Glomerular changes were found in 35.7% of apparently normal living donors: 9 cases showed relative glomerular ischemia with an irregular basal membrane (12.9%); 5 cases showed a diffusely widened basal membrane without antecedents of hyperglycemia (7.1%); in one case (1.4%) there was a lesion similar to type 1 mesangio-capillary glomerulonephritis with C3++, IgG++, IgA+, and IgM+; in another case (1.4%) there were scant isolated C3 glomerular, subepithelial deposits with indentation of the basement membrane of the immunocomplex type with a microhematuria which was demonstrated only after donation, and in 9 cases (among them two pairs of siblings) there were mesangial IgA and mesangial electron-dense deposits compatible with Berger's disease (12.9%). None of these glomerulopathies were evident under LM.

Structural composition of central pulmonary arteries. Growth potential after surgical shunts.

Failure to achieve adequate pulmonary artery growth in patients with cyanotic congenital heart disease is a major obstacle to surgical correction. To assess whether differences in structural composition of central pulmonary arteries influence their growth potential after surgically created shunts, we obtained full-thickness biopsy specimens from the hilar pulmonary arteries of eight patients with pulmonary atresia or tetralogy of Fallot undergoing modified Blalock-Taussig shunts under 1 year of age. Tissue was processed for electron microscopic studies and a morphometric assessment was made of the volume proportions of smooth muscle, collagen, ground substance, and elastin. Initial pulmonary artery size was determined angiographically during the diagnostic cardiac catheterization. Pulmonary artery size was determined by cross-sectional echocardiograms 7 to 27 months later (mean 19 months). Pulmonary artery growth did not correlate with the interval between examinations but did correlate with the volume proportion of elastin (r = 0.73, standard error of the estimate = 1.45, p less than 0.05). Thus the structural composition of pulmonary arteries may influence their potential for growth after surgical shunts. In particular, an inadequate proportion of elastin may be a hindrance to growth.

Acute glomerulonephritis in children. An evolutive morphologic and immunologic study of the glomerular inflammation.

This study deals with 80 children showing a clinically typical acute nephritic syndrome with proteinuria and/or hematuria, edema, and hypertension. Their ages ranged between 2 years 5 months and 16 years; 36 cases were female. In 73 cases a streptococcal infection was demonstrated; the remaining 7 cases did not show morphologic differences with the former. No previous renal disease nor familial history of nephropathies were elicited. In every case a renal biopsy was obtained in the first 60 days of clinical disease; in 46 cases a second renal biopsy was obtained sometime between 6 and 45 months since the beginning of the disease. In 3 cases the renal tissue was considered to be normal by light microscopy, immunofluorescence and electron microscopy in both the first and second biopsies. These 3 patients were not considered as cases of morphologic acute glomerulonephritis in spite of showing a typical clinical picture and they are not included in the evaluation of the glomerular inflammatory evolution. In all the remaining 77 cases C3 granular and segmentary intramembranous and mesangial dense deposits were found in the first biopsy. The second biopsy showed morphologic and immunologic normalization in 27% of the cases, slight lesions in 56% and an increase in severity in 17% of the cases.

Clinical and morphological recovery between two episodes of acute glomerulonephritis: a light and electron microscopic study with immunofluorescence.

A 4 year-old girl presented with 2 episodes of acute glomerulonephritis with clinical and morphological recovery between them. Both episodes were preceded by impetigo. The first and third renal biopsies showed glomerulitis with diffuse dense deposits in the mesangium, segmentary intramembranous dense deposits, atypical "humps" and granular C3. The second biopsy, taken between episodes, was considered to be normal. Two years and 5 months after the second episode the clinical evaluation and laboratory tests were normal.
