In the treatment of bone infections the reconstruction and rehabilitation of bone defects is a problem that often requires treatment secondary to curative treatment of the infection. For the reconstruction of smaller and more extensive defects we used predominantly (92.7%) autogenous (autologous) untreated spongiosa and in only 7% of the cases allogenic (homologous) spongiosa from an organ bank, this being added if necessary. Recently the additionally introduced vascularized bone chip has become a useful extension of the therapy concept. The problems and complications of defect reconstruction are demonstrated for 705 cases of bone infection with 472 defects of different sizes, based on a comprehensive classification with defect calculation. Surgical technical approach and special aspects of after-treatment are described, as well as the results for every group of cases. We achieved stability and freedom from infection in a total of 93.7% of the patients. As was to be expected, the problems grow with the size of the defect. Particularly problematic are joint infections with adjacent extensive circular defect.

In a retrospective histological study, resected specimens obtained from 23 patients with adenocarcinoma in Barrett's esophagus (Group I) and endoscopic multiple (step) biopsies from 38 patients without carcinoma in Barrett's esophagus (Group II) were investigated for dysplastic changes. Dysplasia was most frequently found in the type of mucosa comprising intestinal metaplasia. There seem to be two pathways to dysplasia in Barrett's esophagus. In Group I dysplasia was found in 18 out of 23, and in Group II in 2 out of 38 patients. In Barrett's esophagus, dysplasia may be considered not only a precursor of carcinoma, but also a marker for coexisting carcinoma.
Bacterial infection of major joints is an accident surgery emergency requiring immediate treatment by wide opening, meticulous debridement, insertion of Septopal and short-term systemic treatment with antibiotics. In case of pronounced exsudative joint infection two surgical procedures are necessary: initial surgery leaves arthrotomy open to prevent any possible retention, whereas in a second procedure about 10 days later the open wounds are subjected to secondary closure after repeating the debridement. Early functional treatment, which is decisive for the ultimate joint function, can be initiated in each case directly after surgery. Prognosis of joint function depends on the time at which consequent therapy is initiated. In 13 joint infections with early or delayed onset of treatment at our hospital during 1980 to 1984, all infections were permanently cleared with good joint function without requiring any subsequent surgery or revision. Primary (in one case secondary) arthrodeses were required in six chronic joint infections with late onset of treatment. Since 17 of the 19 joint infections were referred to us after unsatisfactory pretreatment, we can emphasise the fact that in our opinion it will be unsatisfactory to treat joint infections either with antibiotics only or by systemic antibiotic treatment combined with local instillation of antibiotics, punctures or local rinsing, as well as by percutaneously applied closed through-drainage and suction drainage.
Atrophodermia idiopathica progressiva Pasini-Pierini (AIPP) in two young female patients is described. This disease, which begins in the second decade, is slowly progressive and has the characteristics of an originally atrophic dermatosis. No regression to sclerosised lesions could be observed. The distribution of lesions was a strictly one-sided, with the radicular pattern of some lesions suggesting the participation of neurogenic factors. The finding of antinuclear antibodies of the nucleolar type suggests an immunological pathogenesis. HLA typing did not reveal any extraordinary patterns or, interestingly, any of the types associated with immunological diseases.

Osteosynthesis with fixateur externe is indicated in unstable, infected fractures, infected pseudarthroses and defect pseudarthroses, second-degree and third-degree open fractures as well as closed or unstable old fractures and pseudarthroses with marked damage of the soft parts, as well as in all arthrodeses of the knee joint or ankle joint (foot arthrodeses). The main field of application is the shaft of the lower leg with its adjacent joints. To achieve satisfactory stability, mounting should always be three-dimensional and numerous additional details must be taken care of, all of which are described in this article. Whereas in the case of an infection no change of osteosynthesis is permissible after external fixation, it should be considered under certain conditions in the aseptic range. Results and complications after 123 aseptic and 88 septic osteosyntheses with fixateur externe during the last 5 years are described in detail and discussed.
