[Study on the direct interaction between alkaline phophatase and Cu(II) ions by spectral analysis].

In this paper, the results of EPR, VIS and enzyme activity show that alkaline phophatase from calf intrest directly interacted with additional Cu(II). The added Cu(II) enter into A site of active sites of AKP which decrease enzyme activity. With increase of pH value, the decrease in the intensity of the Cu(II) EPR signal and the change of place signal in VIS were observed. These results suggest that Cu(II) migrate in the active center of the enzyme.

[Study on the interaction of copper-zinc superoxide dismutase with cobalt(II)-tryptophane by spectral analysis].

The effect of changing the amount of external added Co(trp)n and pH values on the interaction of Cu2Zn2 SOD with organic metal compound (Cobalt(II)-Tryptophane) have been studied by means of ICP, VIS and measurement of enzyme activity. It has found that in aqueous solution, there exists a direct interaction of the metal ions of the active center in the metalloenzyme (Cu2Zn2 SOD) with external added Co(Trp)n. As a result, part of the metal ions in metalloenzyme were replaced and the corresponding metalloenzyme derivatives (Co(II)-substituted derivatives of SOD) were produced and the catalytic activity of enzyme were affected. It has also studied the intensity of interactions in different molar ratios (Cobalt(II)-Tryptophane) and pH values and got some results under the effect of corresponding factors.

[Study on the interaction between nuclease P1 and CuCl2 (II) by visible absorption spectra].

In this paper, we studied the directly interaction between nuclease Pl and CuCl2 in aqueous solution by visible absorption spectra. As a result, the zinc ion in the active site of nuclease Pl can be replaced by additional Cu(II). At the the same time, Cu(II) entered into the active site of enzyme forming the correspondent derivatives of "Cu(II)-Pl". The variety of pH value influenced the types of derivatives and the enzyme activity.

[Preliminary study on the pharmacological action spicatus].

In this report the pharmacological action of Spicatus was studied. The results insicated that it had diureric, antibiotic and anti-inflammatory effects, yet had Iittle toxic side-effect. It had significant inhibitory effect on crofon oil-caused mice ear swell. It also had marked diuretic effect in orcinary rats, but had Iittie effect on uric pH the rats. It exhibited certain inhibition of Staphycoloccus aureus, Eschrichia coli and Pseudomonas aeruginosa in vitro. The maximum tolerable dose test in mice showed no marked toxic effect, LD50 > 80 g/kg.

[Initial experience with endoscopic subfascial dissection of perforating veins in 100 interventions].

Progress in minimal invasive surgery made us look for a new concept to treat our patients with varicosis, which is one of the most frequent operations in our department. We changed our method from an epifascial ligation to an endoscopic subfascial dissection of the perforating veins (ESDP). This method was developed and first described by Hauer in 1985 [2]. Between March 1992 and May 1993 we treated 85 patients by this method, performing ESDP on 100 legs. In most of the cases the operation is combined with a classic Babcock's procedure, although it can be done isolated. Sectioning of the perforating veins needs special instruments and must be performed in partial deprivation of blood supply, which is produced by a sterile Löfqvist roller cuff [5]. The sectioning of the perforating veins through a small incision in the medial part of the lower leg allows a dissection of all perforating veins in the medial and dorsal sections of the lower leg. The subfascial procedure reduces delayed wound healing, especially in patients with trophic skin disorders. The operation in deprivation of blood supply allows smaller and therefore cosmetically better incisions also for the other subcutaneous varices.

[Ultrasound selection criteria for laparoscopic cholecystectomy].

Laparoscopic cholecystectomy was carried out in 340 patients in the period November 9, 1990, to November 8, 1991. Preoperatively sonographical selection was in use. Only in 3 cases it was necessary to convert to laparotomy and cholecystectomy (including one elective conversion). 80% of the patients admitted to the hospital with symptomatic gallstones could be treated by laparoscopic technique. There was no injury of the bile ducts. Most relevant criterias for sonographical selection are the following: Thickening of the wall of the gallbladder, diameter and number of the gallstones, position of the fundus of the gallbladder in relation to the caudal margin of the liver, diameter of the common bile duct and exclusion of intraabdominal adhesions by using a high-frequent ultrasound transducer. Sonographical criterias for exclusion are a completely stone-filled gallbladder, a scleroatrophic gallbladder, acute cholecystitis with wall-thickening without edema and extended intraabdominal adhesions in the right upper quadrant. Sensitivity of sonographical selection was 98.5%, specifity 97.6%.

Acute abdominal pain. Actual surgical aspects of sonography.

The results of a prospective study show the most common causes of acute abdominal pain, and the diagnostic role of sonography in such cases is evaluated. Sonography performs three significant functions: visualization of pathologically changed organs, identification of healthy organs, and ultrasound-guided puncture of intra-abdominal collections of fluid. Affections in the right upper quadrant can be best diagnosed sonographically. Edematous pancreatitis can be easily identified; interpretation is difficult, however, in the presence of necrosis and abscesses. Therefore, computed tomography is also necessary. Sonography can differentiate between a mechanical and a paralytic ileus, but radiological examination is still necessary. In inflammatory diseases of the gastrointestinal tract, sonography may be helpful for diagnosis. An aortic aneurysm can be easily identified, whereas mesenteric infarction cannot. In the future, however, the use of a duplex scan may help in recognizing this condition. Affections of the abdominal wall can be visualized well by using a high-frequency transducer.

Abdomen, Acute↗

[Which acute abdominal diseases must have urgent surgery?].

To answer this question, a prospective study of 342 patients was carried out. 18% could be discharged after exclusion of any serious disease; 63% were admitted for observation and in some cases delayed operation; in 19% of cases an emergency operation was performed (mostly for appendicitis and obstruction). The decision for early operation depended largely on the simple methods of history and repeated examination. More complicated diagnostic methods were helpful in a few cases only.

[Imaging procedures and follow-up in pediatric surgical diseases].

The most important modern imaging diagnostics are the ultrasound, scintigraphy, computed tomography and recently the nuclear magnetic resonance (NMR). The diagnostic tools should be used one after another, always starting with the easiest to perform, with low risk and no harm to the patient, at low costs. In routine diagnostic procedures (e.g. blunt abdominal trauma) this will be the sonography. The result of the ultrasound may give rise to other, but more invasive methods. Computed tomography, e.g. may give more detailed information about position, size and operability of a tumor whereas scintigraphy has its strong point in disclosing functional organ disorders (e.g. kidney, liver and especially in splenic transplants).

Abdominal Neoplasms↗

[Intestinal invagination in adulthood].

Intussusception in adults is rare. Symptoms are often uncharacteristic. Therefore diagnosis is difficult. Diagnosis can be confirmed by radiologic and endoscopic measures. Surgical treatment consists in the resection of the struck part of the intestine according to the principles of tumor surgery. The danger of perforation rises if desinvagination is tried. It is reported about 11 own cases in which 7 neoplasms were the cause of intussusception. Two patients died of aspiration pneumonia and intracerebral hemorrhage caused by rupture of an aneurysm of the cerebri media artery.
