Cardiovascular reflexes and pudendal evoked responses in chronic haemodialysis patients.

The bulbocavernous reflex (BCR), cortical pudendal evoked responses (CPERs), non-invasive cardiovascular tests and nerve conduction studies were performed in 16 patients on chronic haemodialysis and in a group of normal subjects. BCR and CPERs were more severely affected in patients with impotence. There was a significant correlation between the Valsalva ratio and P1 latency of the CPERs. The BCR and CPERs are alternative techniques for the assessment of impotence in uremic patients.

[Transient myeloproliferative disorder associated with Down's syndrome in newborn infants].

Four cases of neonatal transient leukemoid reaction associated to Down's syndrome (TLRAD) studied in a Metropolitan Hospital at Santiago, Chile, since 1980 to 1985 are described. Diagnosis was done at the first week of live by blood cell counts in all the patients, and in every case these were characterized by high leukocyte counts (15,000 to 48,000 x mm3 blood), 13 to 25% circulating white blood cell blasts, 25 to 61% blast forms in bone marrow aspirates and normal erythrocyte and blood platelet series. In three cases the lower inferior hepatic border was palpated 3 to 5 cm under the rib's margin at the clavicular mid line. White blood cell blasts disappeared from circulation after 1 to 4 months in coincidence with reduction of liver's size to normal. Two of these patients died after remission of their leukemoid reaction as a result of serious systemic infections, without clinical nor laboratory evidence of leukemoid reaction neither leukemia, even though one of them was submitted to a complete postmortem examination. The other two cases had favourable clinical evolution and their hematologic controls have being normal along the following three years.

[Acute poisoning with thyroxine in children].

Two girls aged 5 and 14 years were admitted to hospital after ingestion of L-thyroxine in estimated doses of 72 micrograms.kg and 118 micrograms.kg in the preceding 4 and 8 hours, respectively. Both had high serum T4 (19 and 20 micrograms/dl) and were asymptomatic at the time of admission and both were treated with Iopanoic acid 3 g orally every 72 hours, in order to block the conversion of T4 into T3. Serum levels of T4 were still elevated for up to ten days (8 and 16 micrograms/dl, respectively at day 10), but serum levels of T3 came down to 60 micrograms/dl within the first 48 hours of treatment in both cases. It should be taken into account that these patients may be initially asymptomatic and that symptoms may appear even later than 24 hours after the ingestion, so they should be admitted and treated at hospital. Iopanoic acid has been proved to be a treatment of choice in order to block the conversion of T4 into T3.

[Hemorrhagic fever and the dengue shock syndrome].

Dengue Hemorrhagic Fever (DHF) and Dengue Shock Syndrome (DSS) are the severe and/or fatal clinical manifestations of dengue fever. Epidemics of DHF/DSS occur mainly in the southeast Asian countries where children are seriously affected and high case-fatality ratio is annually reported. Recently significant epidemics of DHF/DSS have been reported in Cuba, Venezuela and Brazil, which means that reinforcements of the epidemiological surveillance in the countries of the American region that show high virus transmission, are urgently needed. The main purpose of the present article is to review relevant information regarding the clinical manifestations, pathology, diagnostic procedures, treatment of cases, pathophysiologic mechanisms and some data related with specific DHF/DSS epidemics.
