[Recurrent canaliculitis and dacryocystitis as a sequela of persistent infection with Chlamydia trachomatis].

We report on 4 patients aged 39-62 years with follicular conjunctivitis, canaliculitis, canalicular obstruction, dacryocystitis and nasolacrimal duct obstruction caused by chronic oculogenital infection with Chlamydia trachomatis. Microbiological diagnosis was accomplished by means of the McCoy cell culture technique as a highly specific tool for detection of the chlamydial infection. In addition, serological tests (IgA-IPA and IgG-IPA techniques) were performed. Bacteriological examination of conjunctival smears showed coinfection in 3 patients (Proteus mirabilis in 1, Staphylococcus aureus coagulase-positive in 3). Despite successful operation techniques, the obstructive inflammation caused by oculogenital chlamydial infection could only be cured by systemic treatment with specific antibiotics, such as doxycycline or erythromycin. All cases of chronic follicular conjunctivitis with lacrimal inflammation that are resistant to topical antibiotics should suggest the possibility of infection with Chlamydia trachomatis.

[The effect of triamterene on the ERG of Long-Evans rats].

Dopamine is an important mediator of the neuronal adaptation mechanism of the retina. Tetrahydrobiopterin (BH4) is cofactor of the rate-limiting enzymatic tyrosine-hydroxylase step in the biosynthetic pathway of dopamine. This enzymatic step causes an oxidation of BH4 to dihydrobiopterin (BH2), which is reduced again to BH4 by action of dihydropterin-reductase (DHPR) and dihydrofolate-reductase (DHFR). Triamterene, a pterin derivate, is known to induce DHFR after initial inhibition. Concomitant elevation of retinal BH4 will result in a higher dopamine concentration. ERG changes of Long-Evans rats mediated by this triamterene effect on dopamine concentration were examined after the rats received triamterene with tap water in two experimental series for 5 and 11 days. After 5 days of triamterene administration a significant decrease of scotopic a- and b-wave ERG amplitudes were registered. After 11 days of triamterene application the decrease could be shown to be even more substantial. This effect was completely reversible: 10 weeks after cessation of triamterene supply ERG amplitudes increased to previous levels. The observed transient ERG changes support the concept of biopterin as an mediator of neuronal adaptation mechanisms of the retina.

[Pseudotumor orbitae--a chameleon in diagnosis and therapy?].

Between 1972 and 1988, 25 patients were diagnosed and treated for "pseudotumor orbitae" in the Departments of Radiotherapy and Ophthalmology of Münster University Clinics. All cases had been initially confirmed by histology. Later in seven cases (28%) the histology showed a definitive alteration, and eventually turned to malignancy in five patients. Criteria indicating such a shift are seen in higher age, subacute onset of the disease, low pain, minor signs of inflammation, low echoes in ultrasound diagnostics, and the absence of "Dutcher bodies" in histiocytes, if these are evident in histology. These patients at increased risk of malignant transformation should be considered for radiotherapy at an earlier stage than prescribed by the current routine.

[Cold dosage in retinal cryotherapy using echography].

In cases of obscured view of the fundus a precise timing of cryotherapy of the retina is not possible. In order to avoid complications from overtreatment we use the real time ultrasonography for monitoring the spreading ice in relation to the retina.

Peritoneal lavage in abdominal sepsis. A controlled clinical study.

The value of intraoperative peritoneal lavage (IOPL) with saline solution, with or without antibiotics, in the treatment of peritoneal contamination, continues to be controversial. A prospective trial was carried out in 87 patients who underwent emergency laparotomies for peritonitis. They were randomized to be treated in one of three ways: group 1 (mean acute physiologic and chronic health evaluation [APACHE] II score, 8) received no IOPL; group 2 (mean APACHE II score, 10) received IOPL with saline solution; and group 3 (mean APACHE II score, 8) received IOPL with saline solution and 2 g of chloramphenicol succinate. In groups 1, 2, and 3, the mortality was 21%, 21%, and 10%, respectively (not significant), and correlated well with the preoperative APACHE II scores. In groups 1, 2, and 3, the mean hospital stay was 13, 13, and 10 days, respectively (not significant), and the incidence of wound infections was 20%, 17%, and 17%, respectively; the incidence of surgical complications was 10%, 24% and 7%, respectively (not significant), and of medical complications, 24%, 31%, and 17%, respectively. We concluded that IOPL with saline solution or antibiotics did not influence the outcome following laparotomies for peritonitis.

APACHE II in emergency operations for perforated ulcers.

Most reports concerning the management of patients with perforated peptic ulcers suffer from inadequate documentation of the severity of illness and the operative risk of these patients. This study attempts to evaluate the prognostic value of the APACHE II scoring system in a homogeneous group of patients with perforated peptide ulcers, to assess it as a potential clinical and research tool for future studies, and to examine its usefulness in stratifying these patients according to their surgical risk. Our results confirm the prognostic value of the APACHE II scoring system, measured either retrospectively or prospectively, in patients who undergo emergency operations for perforated ulcers. When measured preoperatively, it accurately stratifies these patients to various risk groups and should therefore be used in future trials examining various treatment aspects of this condition. Our data, evaluating the results of various procedures in subgroups of patients stratified by the APACHE II, failed to support the superiority of one approach over another.

[Monapterin and biopterin in degenerated retinas].

We measured the concentrations of biopterin, neopterin and monapterin in the neuroretina of humans and different rat strains and found a relatively high amount of oxidized monapterin in these tissues. The dystrophic retinae of RCS rats revealed an inverse ratio of biopterin-to-monapterin compared to LE rats. This ratio was altered in LE rats after exposure to strong light. These results suggest that changes in pterin metabolism may correlate with retinal dystrophy.

[The influence of anamorphotic image formation on the visual evoked cortical potential].

The correction of astigmatism by spectacle lenses involves anamorphotic image formations and produces distortion of retinal images, putting strain on the sensory part of fusion. These distortions were artificially produced by anamorphotic telescopes in a group of emmetropic volunteers. In binocular visually evoked cortical potential (VECP), a transient increase in amplitudes was recordable after application of the anamorphotic distortions. The removal of anamorphotic telescopes resulted in a distinct decrease in amplitude, followed by an overshoot increase before recompensation to pretest values occurred. The amplitude reactions observed are suggested as being an objective indicator of the cortical fusion mechanism.

[Extent and prognosis of eyeball ruptures].

On the basis of 51 ruptures of the globe we present the localization, clinical symptomatology and surgical procedure, as well as the long-term results. The unfavorable prognostic factors are described and compared with these in the literature.

[Occurrence of retinal complications by reproliferation following vitreoretinal silicone surgery].

Clinical follow-ups of 235 patients following vitrectomy and silicone oil injection were evaluated with special regard to the occurrence and time course of reproliferations. The total incidence of reproliferations was 33%. The highest rate was found after ocular injuries (47%), followed by rhegmatogenous retinal detachment (30%) and proliferative diabetic retinopathy (23%). In all three groups the incidence of reproliferation was highest during the first three postoperative months. In cases with ocular injuries and rhegmatogenous retinal detachment, the rate of reproliferation was found to be highest during the first postoperative month, while in cases of proliferative diabetic retinopathy the incidence peaked during the second and third months. Therefore, in order to prevent complications due to the silicone oil, the oil should be evacuated between the third and sixth postoperative month.

Operative management of bleeding post-bulbar duodenal ulcers and possible injury to the common bile duct.

Emergency surgery for a bleeding post-bulbar duodenal ulcer may entail a risk of injury to the common bile duct. In an experimental study on 20 fresh cadavers, the distance between the common bile duct and the duodenum was measured, and sutures and metal clips were placed in the duodenal wall. Sutures presented a greater risk of injury to the common bile duct than metal clips. Recent clinical experience with the application of metal clips in a bleeding post-bulbar ulcer is presented and relevant reports summarised.

[Concordance of Kearns-Sayre syndrome and Klinefelter syndrome].

In the last years the Kearns-Sayre-Syndrome has been defined with the typical trias of chronic external ophthalmoplegia, pigmentary retinal dystrophy and cardiac conduction defects. Today it is no longer believed to present an entity but a variant of the multiple plussymptoms of the ophthalmoplegia-plus group. In pediatrics the existence of this clinical disorder is not yet well acknowledged. The case of a 16-year-old patient is used as an example for the impressive clinical symptoms and the involvement of several organs in this clinical disorder. The combination with a Klinefelter-Syndrome has not previously been reported.

[Gyrate atrophy with macular dystrophy].

In a case of gyrate atrophy with hyperornithinemia a macular dystrophy was found. The ERG amplitude was markedly decreased whereas the b-wave implicit time was found to be normal. There was an obvious parallel between the extent of the peripheral and central alterations of the retina in both eyes. It may be assumed that in this case of gyrate atrophy the remaining retinal areas suffered little damage as far as qualitative parameters are concerned. Macular dystrophy may be one form of manifestation of the underlying disease.
