[The problems of lipid metabolism. Demands for diagnostically improved insight into the function of lipoproteins (author's transl)].

In the course of a follow-up of 87 subjects with known hyperlipoproteinemia a new, simplified method was tested for quantification of the individual lipoprotein fractions capable of being performed by every larger laboratory, in contrast to the expensive ultracentrifugation. The LDL/HDL ratio proved to be the conclusive parameter for daily diagnosis in hospital and general practise. This enabled the most important aspects of the lipid metabolic situation of a patient to be recognized immediately.

Of 3079 medically justified calls of the special ambulances at Ludwigshafen 70.6% were medical emergencies, of which 1059 were cardiac emergencies, 66.9% of these being myocardial infarctions. One quarter of patients with the typical features of myocardial infarction died in the first few minutes, one third within the first half-hour of onset of symptoms. The use of the special ambulances markedly raised the severity of those cases of infarction admitted to hospital so that, despite appropriate therapeutic measures in the early phase, total mortaltiy of cases of myocardial infarction treated by ambulance and at hospital was 51.1%. On the other hand, very early measures (mechanical and electrical resuscitation, intravenous infusion) can be of decisive importance in patients who would otherwise die as a result of arrhythmias. Long-term survival had been achieved in 29% before use of the special ambulance, in 46.8% after its introduction. No correspondingly good results through very early treatment were achieved after attempted suicide: the death rate was 2.6% both before and after introduction of the special ambulances (470 and 497 cases, respectively).

The problems referring to bioavailability may be considered as follows: up to now extact informations on bioavailability as to the determined substances to meet the requirements of therapy are not obtainable. Obvious is the lack of precise investigations about what kind of bioavailability is desirable in respect to rate of rise of the drug, its course of time and its quantity under consideration for the different groups of disease. For this reason and excluding special cases, only common suggestions can be given at present. Difficulties are also presented by the entitled demand for the proof of efficiency. Test of new substances in man are confined to compromises either on the significance of the tests or the security of the patient.
