Comparison of single-dose antibiotic prophylaxis in uncomplicated transurethral resection of the prostate.

We evaluated the necessity for antibiotic prophylaxis in uncomplicated transurethral resection of the prostate. A total of 107 patients was entered into a double-blind, prospective, placebo-controlled, randomized trial. Only 7 patients were excluded because they had positive preoperative urine cultures. All patients received a single dose of either 1 gm. cefonicid or saline placebo intramuscularly before surgery. No further antibiotics were administered. Urine cultures were obtained intraoperatively, daily while hospitalized, and at 2 and 4 weeks postoperatively. A growth of 10(4) organisms constituted a positive urine culture. Postoperative infection rates were statistically significant with 12% (6 of 51) in the cefonicid group and 37% (18 of 49) in the placebo group (p = 0.003). During the initial 2 days postoperatively there were no infections in the cefonicid treated patients as opposed to 8 in the placebo group (p = 0.003). Our study demonstrated the need for antibiotic prophylaxis to prevent infection after uncomplicated transurethral resection of the prostate. This can be accomplished by using a single dose, broad-spectrum cephalosporin (cefonicid). This procedure simplifies the implementation and decreases the cost of prophylaxis for transurethral resection of the prostate.

Preliminary observations on the results of combined temporary 192iridium implantation and external beam irradiation for carcinoma of the prostate.

We treated 43 patients with adenocarcinoma of the prostate confined to the pelvis with a combination of staging pelvic lymphadenectomy, temporary implantation of the prostate with 192iridium and external beam irradiation. The procedure was relatively safe with low early morbidity. However, there were significant late complications. Of these patients 29 had needle biopsy of the prostate 1 year after completion of therapy and 15 demonstrated residual tumor. Clinical stage C and poorly differentiated tumors were not well controlled.

Adenocarcinoma↗

Secondary carcinoma of the penis.

Secondary tumors of the penis are rare. The genitourinary and gastrointestinal tracts are the most common sites of origin. Immunoperoxidase tissue stains are useful in determining the primary site of secondary tumors of the penis. We report 2 such cases with primary tumors in the colon and rectum.

Features of residual renal function in end-stage renal failure associated with spinal cord injury.

The characteristics of end-stage renal disease (ESRD) complicating spinal cord injury (SCI) were studied retrospectively in 43 male hemodialysis patients. A control group of male patients dialyzed in the same institution were studied for comparison. The SCI patients had significantly lower serum creatinine concentrations and daily urinary creatinine excretion than the control group, despite comparable creatinine clearances. Therefore, serum creatinine, when compared with the familiar values in non-SCI patients, may greatly underestimate the severity of the renal impairment. Urine output was higher, urine specific gravity lower, and renal glucosuria more common in the SCI patients. 24-hour urinary protein excretion was higher and serum albumin was lower in the SCI patients, with 48% of the patients exhibiting nephrotic range proteinuria. Urine pH was markedly elevated, and pyuria and bacteriuria were present in all SCI patients. Fractional excretion of potassium (159 +/- 16%) exceeded its filtered load in most SCI patients.

Hematologic features of chronic renal failure associated with spinal cord injury.

We examined the available hematologic data on 43 spinal cord injury (SCI) patients with end-stage renal disease (ESRD) treated with hemodialysis since 1972. A control group consisting of 45 ESRD patients without SCI was included for comparison. Moderate to severe anemia was found in all patients. The observed anemia was generally normocytic, normochromic, and associated with a low reticulocyte response. Transfusion requirement in the SCI group was considerably more than in the control group. There was a high incidence of splenomegaly, splenic amyloidosis, and lymph node enlargement in the SCI group. Lymph node and bone marrow amyloid deposits were found in some SCI patients but not in the control subjects.

Complications of external condom drainage.

As a result of recent advancements in urodynamic evaluation of male patients with spinal cord injury and improvement in medico-surgical treatment of bladder and sphincter, a high percentage of SCI patients are catheter free and on external condom drainage. However, the improper application of a condom in more than 15 per cent of these patients has caused a variety of complications, some of them very serious. Therefore, the common complications of condom application are described and illustrated. Also, the treatment of those complications are discussed and in particular the preventive measures are presented, and the importance of the stressed.

Urethral sphincterotomy for chronic spinal cord injury.

This study involves 53 patients with chronic spinal cord injury for 3 to 30 years in duration who had remained on catheter retention, had experienced different complications and were selected for urethral sphincterotomy. The pertinent and related literature, the results of patient evaluation and the indications, methods and complications of sphincterotomies will be discussed. Also presented are interesting points of the urodynamic studies and the justification and encouraging results of sphincterotomy in these patients.

Experience with penile prosthesis in spinal cord injury patients.

During the last 2 years 30 patients in the spinal cord injury service at our hospital received a penile prosthesis implant. The records of 25 patients were reviewed and 20 patients were interviewed 3 to 27 months postoperatively. A detailed evaluation of each patient shows that in properly selected patients the results of this operation can be satisfactory and gratifying.
