Crossbow homicides.

A total of eight cases of homicide by crossbow are reported, including six intentional, assault-like killings and one hired killer. The bolts showed a high penetration capacity despite the rather low kinetic energy (<100 J): a field-tip traversed one upper arm and the thorax (36 cm) and two broadheads caused perforating injuries of the thorax (25-26 cm). This was due to the high sectional density and the split-like penetration mechanism. Wound morphology was especially important if the perpetrator had extracted the bolt, which occurred in half of the cases. The shape of the entrance wound depended on the type of arrowhead: broadheads produced star-shaped to triangular wounds, field-tips caused circular, oval or slit-like injuries. Foreign material from the arrowhead was found inside two injuries. In assaults, the crossbow was used to hunt the victim down from a short distance which does not require practice but still has the advantage of a distance weapon. However, immediate incapacitation occurred rarely so that additional violence was frequently applied. The noiseless character of the weapon explains why many victims were taken by surprise and why the corpses initially remained unnoticed. Crossbows can therefore be considered ideal weapons for man hunting and some were bought for the very purpose of the killing.

Absinthe: attention performance and mood under the influence of thujone.

OBJECTIVE: The aim of this study was to determine whether the impacts of absinthe on attention performance and mood were different from those experienced with beverages that contain only alcohol. The ingredient causing absinthe's toxicity is believed to be thujone. METHOD: A total of 25 healthy subjects participated in the study. An attention performance test and two questionnaires testing different mood dimensions were used. Three drinks with an identical amount of alcohol but with different amounts of thujone were offered. RESULTS: The results of the present study showed that the simultaneous administration of alcohol containing a high concentration of thujone had a negative effect on attention performance. Under this condition, the subjects tended to direct their attention to signals in the central field of attention and to neglect peripheral signals; the number of correct reactions decreased significantly in the peripheral field of attention, and reaction time and the number of "false alarm" reactions increased significantly. The effects were most prominent at the time of the first measurement. When the subjects were under the influence of alcohol or were administered both alcohol and a low thujone concentration, these effects were not observed. The assessment of mood state dimensions showed that the anxiolytic effect of alcohol was temporarily counteracted by a high thujone concentration. CONCLUSIONS: As they are apparently opposed to the effect of alcohol, the reactions observed here can be explained by the antagonistic effect of thujone on the gamma-aminobutyric acid receptor. Similar alterations were observed for the other mood state dimensions examined.

Methadone and its role in drug-related fatalities in Cologne 1989-2000.

All drug-associated deaths from 1989 to 2000 were analysed at the Institute of Forensic Medicine in Cologne. Information concerning sex, gender, drug consumption, time, place and circumstances of death were analysed. A number of 605 cases were recorded; in 518 cases a toxicological analysis was possible and in 171 an autopsy was performed. When it was possible to determine the cause of death form the information available, acute drug intoxication was recorded in 65%. Heroin head the list of identified substances. Sixty-three percent of the specimens showed a combination of several substances, especially a combination of morphine, benzodiazepines, other medications and alcohol. In comparison with other studies the percentage of methadone-positive specimens is low, even though the proportion of specimens positive for methadone increased from 1989 to 2000. This analysis is discussed using background information concerning the management of substitution therapy and the available literature.

Sildenafil (Viagra): is there an influence on psychological performance?

BACKGROUND: Sildenafil (Viagra) is a well-introduced medicine for erectile dysfunction; many studies about effects and side effects are published. Beside these aspects of treatment the influence of sildenafil on psychophysical performance is of interest. cGMP is one of the most important second messengers in the central nervous system (CNS), so even very small changes of the intracellular cGMP-level caused by phosphodiesterases inhibition may be relevant for CNS-function. We wanted to verify the hypothesis whether sildenfail influences human psychomotor performance, especially under the aspect of traffic safety, or not. METHODS: Designed as a pilot study we tested 6 male healthy volunteers using a test battery of 7 different psychophysical performances tests. Each individual did the test battery twice, once without drug and once after a single oral dose of 100-mg sildenafil. 3 persons did the first and 3 others did the second experiment under the influence of drug (UID). All results (37 parameters) were analysed by t-test for paired samples using a confidence interval of 95%. RESULTS: Only two parameters of 2 different tests showed significant differences. In the simple choice reaction test (DR2) the mean reaction time got better in the group with sildenafil; in the multiple choice reaction test with stress induction (RST3) the amount of wrong answers indicated a weak influence of performance without statistical significance, six parameters (dominantly in the speed anticipation test (DEST)) represented an increase and one other (RST3 second part) showed a decrease UID. The uppermost parameters (76% of all items) stayed on equal levels for both groups. CONCLUSION: Sildenafil showed no important impairment of psychophysical performance, no strong improvement was found as well. With a look at the therapeutically indication of sildenafil the improvement in sexual activity may indicate no incapacity in traffic and other psychomotoric/psychophysical functions.

["Handy-cap" in street traffic. Mobile telephone considerably affects driving-relevant performance].

The increasing popularity of mobile telephones prompts the question: are they a danger when used while driving? To date, Germany has no law regulating such use. An international enquiry showed that four European and three non-European countries punish the use of mobile phones by drivers. Analysis of scientific studies on the hazard potential of such devices when used in vehicles unequivocally shows a marked impairment of driving performance. Although this cannot be applied to any particular individual, there is indeed a general potential road traffic hazard. There is thus an urgent need to make users aware of the dangers and for legislature to demand the use of no-hand phones in vehicles. A specific punishment by law (inhibition of mobile phones) must be critically discussed.

Child homicide in Cologne (1985-94).

Twenty five child homicides investigated in the Cologne University Institute of Forensic Medicine from 1985-94 were retrospectively analysed with special reference to the evidential value of the autopsy findings and possible peculiarities related to the infants' physiological condition. About 65% of the victims were boys younger than 3 years. About 65% of the child homicides were committed by the mother in the parental flat, predominantly in the early hours of the evening during the weekend. The mode of death were 10x blunt injury, 6x sharp violence, 6x strangulation, 3x smothering, 2x drowning, 2x gunshot and 5x neglect (starvation). The defenseless, helpless and immobile condition of the infant particularly favours a homicide by manual assault, smothering or neglect (starvation).

[How much are external fixator nuts tightened in general practice].

External fixation is an approved system of treatment in orthopedic surgery. 21 different modalities of assembly--using the AO (ASIF) tubular system--were selected for a clinical trial in 3 major trauma hospitals. It is considered that all nuts of the applied external fixators were tightened properly. In experimental work it was found, that a torque of 8-11 Nm is sufficient and can be recommended. A suitable torque-wrench is available.

[The possibility of pharmacologic therapy of extrapyramidal hyperkineses with tiapride].

A novel substance, tiaprid (Tiapridex), from the family of the substituted benzamides, can be used in an attempt to block the dopaminergic receptors of the corpus striatum without inducing the psychiatric side effects on the mesolimbic system associated with, for instance, neuroleptics. Since hyperkinetic illness of the brain-stem ganglions accompanied by myoclonia, athetosis and dystonia are believed to stem from a state of imbalance between dopaminergic and cholinergic processes in which dopaminergic activity predominates, the application of tiaprid was tested on patients with etiologically different illnesses in an open clinical study. The course of treatment was documented by measurements and a record of clinical findings. No tolerance problems were encountered, and the substance had no grave side effects. Its effects proved to be good to very good on myoclonic hyperkinesis and good to moderate on athetosis, but dystonia seemed to be resistant to this form of treatment.

[The usefulness of dexamethasonephosphate in the conservative treatment of lumbar pain--a double-blind study (author's transl)].

Analgesic drugs, physiotherapy and corticosteroids are concurrently used in the conservative treatment of lumbar pain. In 100 patients a double-blind study with dexamethasonephosphate versus placebo was performed. Lumbar pain and no findings with myelography were the basic requirements for inclusion into the study. There was no significant difference between the treatment group and the placebo group concerning the clinical investigation before and after treatment. The positive effect of dexamethasonephosphate, which was seen in a pilot study, was not confirmed in the double-blind study. Because corticosteroids are also psychotropic drugs there is a mild antidepressive effect. Considering the fact that cortisone has no effect on the symptoms of lumbar pain, it should not be used furthermore in the conservative treatment of lumbar pain.

[Intramuscular application of diclofenac--case report and critical consideration of a therapeutic measure].

Diclofenac is a non-steroidal anti-inflammatory drug of the phenylacetic class. It is frequently given as an intramuscular injection. However several, sometimes severe, side effects have been described after an i.m. administration. Based on a short case report about a fatal complication in the context of the i.m. administration of diclofenac, the arguments for and against the intramuscular injection of the drug are critically discussed. As a result, the administration of diclofenac as an intramuscular injection should be critically reviewed and alternatives -- as suppositories are available -- should be taken into account.

[Techno-festival mit fatal outcome].

The authors report on a 21-year old female who died suddenly during a techno-festival. The autopsy findings and the results of chemical-toxicological analyses were not able to clear up the cause of death. Pathophysiological and toxicological considerations demonstrate that only a combination of facts are sufficient to explain the sudden death of this young woman.

[DNA contamination by examination instruments].

Using DNA-analysis systems in forensic examination, conditions without any (iatrogenic) contamination are necessary for analysis of evidence. Failing these conditions a contamination is possible and interpretation of the results may be difficult. Our experiments demonstrated a relevant transfer of DNA-material if contaminated instruments were used.

[Phenomenology of kicking and kicking to death].

28 autopsy cases with injuries due to blunt trauma comprising kicking or stamping on the victim are analysed for characteristics of the victims, the perpetrator and the assault as well as for the injury pattern. Emphasis is put on patterned contusions which in cases of medium impact reflect the characteristics of the footwear that made them as a mirror image.

[Routine case of "auto-erotic accident"--surprising turn in evaluation of cause of death after chemical-toxicologic analysis].

A man with indications of autoerotic manipulations was found dead in his flat and a homicide had to be excluded. Diverse material found in the flat suggested that the man was a consumer of cocaine. But the analyses of blood, urine and organs showed high concentrations of amphetamine and it was thought about an unintentionally mix-up with cocaine. A hair analysis was made and only amphetamine was detected. Therefore the utensils (mirror, razor-blade and sniff-pipe) belong to a rare nasal consumption of amphetamine.
