[Particular aspects of creation and assessment of an obstetrical data bank].

The use of computers improves data storage capacity and leads to a homogeneous documentation of data obtained. After extensive preparations documentation was started using in 1975 the 2nd Department of Obstetrics and Gynecology computer. The obstetrical-gynecological data base is used for organisational, epidemiological, scientific and mixed purposes. The main purpose is periodical evaluation of routine procedures. The evaluation of obstetrical data is often combined with specific problems. To compare the data obtained, clinical findings have to be entered in standardized form and to be related to the week of pregnancy. The expected date of birth has to be corrected often as a result of several investigations. Furthermore, it has to be differentiated between single and multiple pregnancy. Good cooperation between physician and medical computer expert is necessary for good quality of evaluation. The evaluated data enable one to find new aspects and connections, which may offer the impetus to perform new prospective research.

[Computer-assisted documentation and recording of findings of obstetrical ultrasound findings].

Since 1976 on our department obstetrical reports are generated and stored by computer. This kind of documentation has been also introduced for obstetrical ultrasound investigations. One year ago the part of our database for storing and recording the results of obstetrical ultrasound examinations was changed according to new purposes. This system was designed to support diagnosis of pathological findings and to arrange the results more clearly. Immediately after the investigation the results are entered in a terminal (3278 IBM). Our database is a CICS (Customer Information Computer System) application of the WAMIS (Wiener Allgemeines Medizinisches Informationssystem) from the IMC (Institut für Medizinische Computerdokumentation). The results of the investigation are printed after storage in the data base. The screen and the obtained print-out are divided into several sections according to medical purposes. Since the results of fetal measurement are displayed graphically, fetal growth and the change of normal or abnormal findings can be judged immediately. The new layout of the hardcopy of the results of our ultrasound investigations improves diagnosis of pathological findings. Each examinator is forced to visualize fetal organs, signs of fetal vitality and to perform standardized measurement. Therefore, the possibility to find pathological alteration is improved especially if the investigator is less experienced.

[Smoking in pregnancy].

The course of pregnancy and delivery and the foetal outcome of women who smoked during pregnancy (n = 1751) were compared with nonsmokers (= 4937). The following variables were studied in retrospect: Duration of pregnancy, maternal weight gain, mode of delivery, foetal birthweight and the maturity of the newborn, Apgar scores, pH of the umbilical artery, perinatal mortality, frequency of foetal malformations, amount of amniotic fluid and the incidence of meconium staining, placental weight and the occurrence of placental infarctions. Birth weight, foetal length and head circumference were significantly smaller in the group of women who smoked. The incidence of babies with low birth weight and clinical signs of prematurity as well as placental infarctions was significantly increased in the same group. Gestational age was not different in smoking and nonsmoking gravidae. No statistical difference was found in the rate of C-sections, in Apgar scores, umbilical arterial pH, the incidence of malformations, perinatal mortality, and placental weight. Social factors were not considered in this study.

Composition of diesel exhaust with particular reference to particle bound organics including formation of artifacts.

For particulate emissions, standards were established by the US EPA in February 1980. Regulations limiting particulates from new light duty diesel vehicles are valid by model year 1982. The corresponding standards on a pure mass basis do not take into account any chemical character of the diesel particulate matter. Our investigation of the material composition shows that diesel particulates consist mainly of soot (up to 80% by weight) and adsorptively bound organics including polycyclic aromatic hydrocarbons (PAH). The qualitative and quantitative nature of hydrocarbon compounds associated with the particulates is dependent not only on the combustion parameters of the engine but also to an important degree on the sampling conditions when the particulates are collected (dilution ratio, temperature, filter material, sampling time etc.). Various methods for the analyses of PAH and their oxy- and nitro-derivatives are described including sampling, extraction, fractionation and chemical analysis. Quantitative comparison of PAH, nitro-PAH and oxy-PAH from different engines are given. For assessing mutagenicity of particulate matter, short-term biological tests are widely used. These biological tests often need a great amount of particulate matter requiring prolonged filter sampling times. Since it is well known that facile PAH oxidation can take place under the conditions used for sampling and analysis, the question rises if these PAH-derivates found in particle extracts partly or totally are produced during sampling (artifacts). Various results concerning nitro- and oxy-PAH are presented characterizing artifact formation as a minor problem under the conditions of the Federal Test Procedure. But results show that under other sampling conditions, e.g. electrostatic precipitation, higher NO2-concentrations and longer sampling times, artifact formation can become a bigger problem. The more stringent particulate standard of 0.2 g/mi for model years 1986 and 1987 respectively requires particulate trap technology. Preliminary investigations of the efficiency of ceramic filters used reveal that the reduction of the adsorptively bound organics is lower than the decrease of the solid carbonaceous fractions.

[Prognostic significance of morphological parameters in endometrial carcinomas].

A retrospective analysis of 163 cases of operations for carcinoma of the endometrium performed at the 2nd Department of Gynecology and Obstetrics, University of Vienna, demonstrates that the clinical staging as recommended by FIGO is not conclusive, and that the subdivision according to the size of the uterus is of no prognostic significance. On the other hand, the morphological parameters listed below have a strong prognostic relevance as is shown by survival curves and survival rates: Grading parameters (histological and cytological degree of differentiation as well as the number of mitotic figures); parameters of the extent of the tumor (histologically proven involvement of the cervix; depth of invasion in relation to the thickness of the myometrium; invasion of lymphatics). In addition, patients with suspicious or positive cytological vaginal smears have less propitious survival chances, whereas the age and the menopausal status of the women as well as the type of carcinoma were only of subordinate significance within this sample. The grading parameters are closely correlated, the determination of several parameters, however, increases the weight of the prognosis. Furthermore, they show a close correlation to the parameters of the extent of the tumor. Since the grading parameters can be ascertained pre-operatively from the curetting they should be taken into consideration when planning further therapy. The post-operative staging of the carcinoma has to take into account above all the depth of the invasion. A classification to that point is proposed.

Evaluation and correlation of hormone levels in normal and pathologic pregnancies.

Estriol determination for monitoring high-risk pregnancies, either in serum or urine samples, has been in use for many years. The disadvantages are the inaccuracy of a 24-hour urine sample and the delay of two days. The advantages are a possible high variation of serum hormone evaluations from a very short sampling period. In order to prove the reliability of serum hormone patterns in pathological pregnancies, we compared them to 24-hours urinary E3 patterns. Free E3 and HCS levels were determined in serum samples by Radioimmunoassays. Total 24-hour urinary E3 was determined by a procedure according to Frandsen. For the graphic presentation, hormone levels from each pregnancy were grouped weekly and the mean values +/- SD in the different weeks of gestation were computed. To estimate the tendency of hormone patterns we performed a Z-transformation of grouped single hormone evaluations. A comparison between the serum and urine E3 measurements showed similar hormone patterns. E3 hormone patterns measured either in urine or serum proved to be well comparable. The mean of the transformed values showed the tendency of the hormone patterns even more distinctly than the graphic presentation. The hormone parameters E3 and HCS for monitoring high-risk pregnancies should be rated according to the diseases. In cases of placental insufficiency, E3 seemed to be the more sensitive parameter, whereas in severe cases of EPH-gestosis HCS might be more reliable.

[Calculation of the probability of survival after combination surgical-radiotherapeutic treatment of endometrial carcinoma].

The probability of survive of 106 patients suffering from endometrial cancer after combined treatment by operation and irradiation was calculated and compared with the percentage of healed patients after 5 years. It could be demonstrated that by set up of graphs of survive the different tendencies of different kinds of treatment were visible very soon, while the usual calculation of the percentage of healed patients after 5 years did not indicate any significant difference. To find out the optimal kind of treatment of endometrial cancer the evaluation of a maximum of parameters of the patient as well as of the therapy is demanded.

[Premature rupture of membranes, fetal outcome, management of delivery (author's transl)].

In 764 gravides from totally 1738 women, which delivered 1977 in our hospital, happened a premature rupture of membranes. 319 patients had a spontaneous (SPR), 454 an artificial premature rupture of membranes (APR). Comparing the number of complications, the perinatal situation and the maternal behaviour, so you will find a significant difference between SPR and APR. The fetal outcome of the children, born after APR was better then born after SPR. The same is valuable as for the operative management of delivery. The perinatal morbidity after APR was nearly equal to the situation after normal rupture of membranes. Therefore you can see, that the bad experiences are more valuable for the SPR.

[Electronic data processing in irradiation of gyneocological cancer (author's transl)].

For statistical processing of data resulting from irradiation of gynecological cancer a system of EDP working off-line is presented. Not only healing and survival rates will be evaluated but also data about the morbidity of irradiated patients. Thereby the employed therapy should be improved. Moreover, the patient follow-up can be supervised by a special program with automatical summoning of negligent patients.

[Experiences with a computer documentation of obstetrical data (author's transl)].

Experiences with a computer information system at the 2nd Department of Obstetrics and Gynaecology, University of Vienna Medical School are reported. The data input is done via video-terminals located at different places in the department. Included are anamnestic data, results of physical examinations, pelvic examinations and laboratory findings and data concerning the course of delivery. The print-out of the stored data replaces the conventional charts. The advantages of this information system are: 1. The support of the clinical routine work by a more accurate and systematic documentation; 2. a comprehensive compiling of the patients data; 3. all stored data are readily available for scientific evaluation.

[Documentation of obstetric case reports by means of video-terminals as a part of the Vienna General medical information system "WAMIS" (author's transl)].

A report is given on the introduction of a complete documentation system of obstetric case reports by means of video-terminals and printer-terminals in both departments of Obstetrics and Gynaecology of the 1niversity of Vienna. The special routine for the admission of patients, which produces the heading of the case report is demonstrated, as well as the sheets of collecting the data in respect to case history, antenatal examinations, labour room reports and details of obstetric operations. The computer prints the case history using the collected data. These computer-printed case reports replace the customary handwritten reports. Laboratory data, therapeutic measures and final diagnosis are computed in the same way. The advantages of this form of organization of data collection for clinical purposes and research work are discussed.

[Significance and incidence of risk factors in pre- and dysmaturity].

Anamnestic and clinical data of 10 654 deliveries were related to premature and dysmature pregnancies. The aim of the study was to get a correlation between special data on the one hand and pre- and dysmaturity on the other hand. The risk of dysmaturity is the highest in gravidae with a gestosisscore of more than 4. The risk of prematurity is the highest in gravidae with bleedings during pregnancy. The most frequent symptom in prematurity was a high cervix-score. It occurred in the prematurity collective in 40.7%. In the dysmaturity collective occurred the abuse of nicotine in 32.9%. Some symptoms have a high prognostic value for a pathological pregnancy, but it was impossible to develop a high risk score following these data. An high specificity was connected with a low sensitivity and vice versa.
