Impact of cefotaxime and ceftriaxone on the bowel and vaginal flora after single-dose prophylaxis in vaginal hysterectomy.

The aim of this randomised controlled study was to investigate the effect of a preoperative, single dose, intravenous injection of ceftriaxone 2g and cefotaxime 2g in 2 groups of 30 patients who underwent vaginal hysterectomy. The parameters were: efficacy and tolerance; impact on the aerobic bowel and vaginal flora; and serum concentrations. There were no significant differences in efficacy, haematology or clinical chemistry parameters. In the ceftriaxone group 2 patients had exanthemas, 2 had diarrhoea. In the cefotaxime group no such side effects occurred. Serum concentrations were 9.75 and 0.07 mg/L for ceftriaxone and cefotaxime, respectively, after 24 and 12 hours, respectively. Ceftriaxone had a pronounced and continuous effect on the bowel flora: Streptococcus faecium and Candida spp. increased, reaching counts of 10(8) and 10(7) organisms/g faeces, respectively, at day 10. Under cefotaxime no dramatic alteration was determined and growth of Candida spp. was not influenced. Growth of Escherichia coli and other bacteria of the vaginal flora was not influenced by either drug. Overgrowth of Candida spp. under ceftriaxone was significant (p less than 0.05) compared with that under cefotaxime. The results of this study indicate that the high biliary excretion of ceftriaxone (45%) and its long acting properties are responsible for the high selection pressure and overgrowth of Candida. Cefotaxime, a drug with low biliary excretion (5%) and shorter action, had only a negligible effect on the microflora with quick normalisation. The observed overgrowth of Candida spp., even after a single injection of ceftriaxone, could cause problems when this drug is being used for surgical prophylaxis or treatment. This applies especially to immunocompromised patients.

[Short-term perioperative prophylaxis with cefotaxim in obstetric and gynecological surgery].

The effectiveness of short-term peri-operative prophylaxis with cefotaxim (Claforan) in gynaecological and obstetric operations was prospectively tested in 764 women (randomized by day of week). The incidence of postoperative infectious complications after vaginal or abdominal hysterectomy was significantly lower in the cefotaxim group than the control group. The rate was also clearly lower after laparotomy without hysterectomy and after abdominal section, but the differences were not statistically significant. In the patients with risk factors, i.e. obesity, cardiopulmonary disease or after laparotomy, which in the control group were correlated with a significantly higher rate of infectious complications, cefotaxim prophylaxis significantly reduced the rate of complications too. There were no side-effects from cefotaxim.

[Incidence rate of late complications in legally induced abortion in the Federal Republic of Germany (author's transl)].

The article goes into the question as to whether and to what extent legally induced abortion can exercise a detrimental influence on later pregnancies in the form of late complication; this problem was investigated by means of the stratified sample technique. A few statistically significant observations were made leading to the conclusion that legally induced abortion does involve late complications in subsequent pregnancies. The study was performed in 650 women demonstrating certain disturbances during pregnancy or birth, and an equal number of women in whom parturition occurred without disturbances after uncomplicated pregnancy; the study period extended from March 1980 to July 1980 and was conducted in various large obstetrico-gynaecological departments in Federal German hospitals. It was found that legally induced abortion was a statistically more frequent precedent to abdominal pregnancy, retarded abortion, cervical insufficiency or imminent premature births, than among the women of the control group (p less than 0.001). The method employed to induce abortion (suction abrasio, etc.) less important for the occurrence of complications than the length of gestation at the time abortion was induced. In induced abortions beyond the 8th week, late complications are significantly more frequent than in women in whom abortion had been induced before the end of the 8th week (p less than 0.01).
