It has been suggested that the addition of lidocaine to the saline used to fill tissue expanders will reduce the pain often associated with the expansion process. In vitro experiments have shown that lidocaine as it is normally supplied will diffuse through an expander only at a very slow rate, which would probably be inadequate for a clinical effect. We found that the addition of sodium bicarbonate resulted in a substantial increase in the rate of diffusion. Studies in rabbits demonstrated that at a pH of 8.0, 75 percent of the lidocaine dose crossed the silicone elastomer membrane at 24 hours and greater than 95 percent had left the expander at 1 week. We have concluded that intraluminal lidocaine can be effective only when the pH is close to the pKa of lidocaine.

An extended dorsalis pedis island flap was used to cover the heel in two patients. The arterial predicles were extended by cutting and bridging with vein grafts to allow the flaps to reach the defects. By this method, excellent coverage is obtained and the protective sensation may be retained or restored by preservation or extension of the sensory nerves.
Graft-versus-host disease is becoming a common clinical problem, one which can require the consultation of a plastic surgeon for restoration of skin coverage. An understanding of the immunological basis of the disease is essential to the clinical management of this condition. A case of GVHD is reported, and the principles of treatment are discussed.

Fifty-one consecutive gallium scans in fifty patients with suspected intraabdominal abscess are reviewed. There were twelve true-positive, seven false-positive, twenty-nine true-negative, and two false-negative scans. Accumulation of gallium in the colon and in areas of healing and repair caused soem interpretive difficulty. Despite a significant number of false-positive results, we believe that gallium scanning is a valuable diagnostic tool. Careful attention to technic and cautious interpretation should significantly reduce false-positive and false-negative scans.

Experience with 184 consecutive biopsies of the breast for 147 benign lesions and 37 carcinomas over an 18 month period has been reviewed. Local anesthesia was used in 78 per cent of the patients, and it was the method of choice whenever the preoperative diagnosis was benign. General anesthesia was used when the lesion was thought likely to be malignant. Anesthesia within these guidelines was used in 88 per cent of patients with correct preoperative diagnoses. Exceptions were made primarily, but not exclusively, because of patient preference. The preoperative diagnoses were correct in 168 patients, unstated in five, and incorrect in 11. Six women erroneously were thought to have carcinoma, and five others unexpectedly had malignant disease. Although biopsy remains mandatory, the preoperative diagnosis of carcinoma of the breast can currently be made with sufficient accuracy to justify restricting recommendation for general anesthesia to those patients thought likely to have carcinoma of the breast.
