Malignant fibrous histiocytoma: a retrospective evaluation of 24 patients.

Twenty-four patients with malignant fibrous histiocytoma are being analyzed: 54.2% occurred in the sixth, seventh, and eighth decades of life; 45.8% were located in the extremities. This is a soft tissue sarcoma noted for its pleomorphic histologic appearance. It is postulated that the origin cell is a histiocyte. The local recurrence of metastatic potential of this tumor is difficult to predict from its histological features. Local recurrence after complete excision is frequent. Also, there is a high incidence of distant metastases. Wide local excision with a wide margin around the tumor is the most important treatment modality. The use of postoperative radiotherapy and chemotherapy needs to be evaluated.

Postirradiation malignant fibrous histiocytoma.

Three patients who were treated successfully with postoperative external radiation therapy for ovarian carcinoma, endometrial adenocarcinoma and bilateral retinoblastoma respectively developed years later malignant fibrous histiocytoma (MFH) within the irradiated field. MFH is a recently described soft tissue sarcoma known for its dual fibroblastic and histiocytic differentiation resulting in a pleomorphic histologic appearance.

Epiglottic enlargement following radiation treatment of head and neck tumors.

This article describes delayed noninfectious epiglottic enlargement in two children, one with rhabdomyosarcoma of nasopharynx and the other with undifferentiated anaplastic carcinoma of the neck. The pathogenesis is assumed to be delayed laryngeal injury following radiotherapy alone or combined with chemotherapy. The radiographic findings in these cases were identical to those of acute epiglottitis.

A clinicopathologic study of orbital and adnexal non-Hodgkin's lymphoma.

An analysis of non-Hodgkin's lymphoma involving the orbital structures was performed at the University of Iowa between 1937 and 1975. Sixteen cases of primary orbital lymphoma were diagnosed. Histopathologic reclassification according to the Rappaport scheme and the clinical course of each histologic sub-category was described. There were 5 patients with reactive hyperplasia, 2 patients with well-differentiated lymphoid proliferation with Dutcher bodies which were also felt to be reactive, 3 patients with diffuse poorly differentiated lymphocytic lymphoma, 4 patients with nodular poorly differentiated lymphocytic lymphoma, and 2 patients with diffuse histiocytic lymphoma. It was concluded that the Rappaport classification is applicable to orbital lymphoid tumors and that those lymphomas which do present as primary tumors should be staged as one would stage the same histologic category of lymphoma presenting in other sites. Radiation therapy appears to be an effective treatment for local control; however, patients with primary orbital lymphoma should undergo observation for systemic disease similar to patients with lymphoma presenting in other sites. Excisional biopsy is recommended to facilitate precise classification.

Accuracy of lymphangiography in the diagnosis of paraaortic lymph node metastases from carcinoma of the cervix.

A study was undertaken to evaluate the use of lymphograms as a method of diagnosing cancer of the cervix metastatic to the paraaortic lymph nodes. Twenty-one patients underwent lymphograms, surgical biopsy, and histologic examination of the paraaortic nodes. The specificity of lymphangiographic examinations is not accurate enough to be of clinical significance in the detection of paraaortic lymph node metastasis from carcinoma of the cervix. Lymphangiography can be of assistance in locating suspicious lymph nodes. These nodes should then be biopsied. The final therapeutic decisions should be based on tissue diagnosis.

DiGeorge syndrome associated with multiple squamous cell carcinomas.

Experimental evidence and clinical experiences have have shown that immune mechanisms and cancer are closely related. A case of a 42-year-old man with DiGeorge syndrome is presented as an interesting example of an impaired immune mechanism and is described in association with multiple squamous cell carcinomas of the upper respiratory system. The congenital absence of the thymus gland in DiGeorge syndrome results in an absence of a cell-mediated immune response. There are plasma cells and germinal centers in lymph nodes, but lymphocytes in the paracortical areas are sparse. Patients wiith DiGeorge syndrome have no delayed hypersensitivity, cannot be actively sensitized with dinitrochlorobenzene, reject allografts poorly, and have no lymphocytic response to phytohemagglutinin antigens.

Bifurcation level of the aorta: landmark for pelvic irradiation.

The position of the aortic bifurcation in women in relation to the lumbar vertebral bodies was determined in 100 pelvic arteriograms to provide some help in teleradiation therapy planning for pelvic malignancies. It was found that in order to cover the area of the aortic bifurcation, the radiation field should extend to the top of the body of the fourth lumbar vertebra.
