Polymorphous light eruption: action spectrum and photoprotection.

Polymorphous light eruption is a common seasonal photodermatosis with a typical history and clinical picture. In the interval, when no lesions are present, the diagnosis relies on artificial reproduction of polymorphous light eruption by phototesting. Photochemotherapy (psoralens with ultraviolet A [PUVA]) is currently an effective preventive treatment. One hundred sixty-seven patients with either a history of polymorphous light eruption or manifest disease entered our study. Of 142 patients tested, 49% developed typical lesions of polymorphous light eruption at the test sites. In 56% the action spectrum was found to be in the ultraviolet A range, in 17% in the ultraviolet B range, and in 26% in both ranges. A total of 122 patients received preventive treatment with PUVA. Of these, fifty-one returned for follow-up. Of the patients who were followed up, 64% reported total protection during outdoor activities in the summer, 26% reported partial protection, and 10% were not protected. Failure to improve was unrelated to the action spectrum. The action spectrum and the incidence of positive results on phototests in our patient population differed from those reported by others. It is possible that differences in the test protocols and in the light sources used may account for this discrepancy. There is clearly a need for a standardized test procedure. However, the majority of patients benefit from PUVA pretreatment regardless of their action spectrum.

Nonmelanoma skin tumors in long-term photochemotherapy treatment of psoriasis. An 8-year follow-up study.

Two hundred ninety-seven long-term photochemotherapy (PUVA)-treated patients from an original cohort of 418 subjects reported in 1980 were reevaluated in a second follow-up in order to determine the risk of tumor development under long-term PUVA. Within an observation period of up to 8 years (mean, 63.1 months) six patients with squamous cell carcinomas and three with basal cell carcinomas were observed. Eight of the nine tumor patients had been exposed to potential carcinogens such as arsenic and/or ionizing radiation prior to PUVA treatment. Five with squamous cell carcinomas were skin type I or II; in four of the six patients with squamous cell carcinomas the tumors were located on unexposed skin areas. The mean cumulative ultraviolet A (UVA) dose in three of the six squamous cell carcinoma patients was three times as high as that in the group of nontumor patients. The other three squamous cell carcinoma patients had lower mean doses than nontumor patients, as did the three patients with basal cell carcinomas. Although the cumulative UVA dose may eventually turn out to be relevant for PUVA carcinogenesis, our present data do not sufficiently substantiate a correlation between cumulative UVA dose and squamous cell carcinoma formation in PUVA-treated patients. This report confirms that previous exposure to carcinogens appears to be the most important factor for nonmelanoma skin tumor formation in long-term PUVA patients.

Immediate pigment darkening phenomenon. A reevaluation of its mechanisms.

Proposed mechanisms of immediate pigment darkening (IPD) are controversial. They include photooxidation of "premelanin," changes in the distribution pattern of microfilaments and microtubules, movement of melanosomes to melanocyte dendrites, increased transfer of melanosomes to keratinocytes, and changes in the melanosome distribution pattern in keratinocytes. We investigated the following aspects of IPD: production of IPD by UVA under physiologic and nonphysiologic conditions in fullthickness skin and epidermal sheets; reversibility of IPD in vitro after in vivo and in vitro production; blocking of IPD by disruption of the microfibrillar or microtubular system in vitro; alterations of the cytoskeleton of melanocytes; the melanosome distribution pattern in melanocytes and keratinocytes. The results were as follows: IPD could be elicited in vitro in full-thickness skin and in epidermal sheets. Its production was temperature independent (0 degrees-37 degrees C) and was not inhibited by repeated freezing and thawing, or by formalin fixation. IPD was reversible in vitro under tissue culture conditions but only in viable skin. IPD could not be blocked by substances that disrupt the microfibrillar or microtubular system (cytochalasin B, colcemid, vincristine). As shown with a monoclonal antivimentin antibody, IPD-producing UVA doses did not induce changes in the cytoskeleton of melanocytes. No changes in number and distribution pattern of melanosomes were observed electron-microscopically and by morphometric analysis of EM micrographs. Production of IPD does not depend on the structural and functional integrity of the melanocyte cytoskeletal apparatus and is not confined to viable skin, whereas its reversibility is. The fact that no increased melanosome transfer occurs may explain the lack of a UV protective action.

[The spectrum of transient acantholytic dermatoses].

In reviewing the literature, we made an attempt to expand the term "transient acantholytic dermatosis" (TAD) beyond the criteria of classical Grover's disease to a broader spectrum of diseases and to systematically categorize the variety of reported clinical pictures. Moreover, we endeavoured to separate these conditions from Darier's and Hailey-Hailey's disease and from immunofluorescence-positive acantholytic dermatoses. Three patients of our own are presented with a heretofore undescribed bullous exanthematic variant.

Photochemotherapy for cutaneous T cell lymphoma. A follow-up study.

In 1975 we started a prospective study on oral methoxsalen photochemotherapy (PUVA) in cutaneous T cell lymphoma (CTCL). The first short-term follow-up of nineteen patients (1978) showed that PUVA may induce long-lasting remission in early stages, and that eventual relapses respond comparably well when PUVA is resumed. We now present the follow-up data of the original nineteen patients, covering a period of up to 7 years, and of an additional twenty-five patients who have entered the trial since April, 1977. Similar to earlier reports, all patients with eczematoid and plaque lesions (stages IA and IB) cleared. Likewise, eczematoid and plaque lesions in patients with early tumors (stage IIB) were cleared. During a mean follow-up of 44 months, 55% of stage IA patients and 39% of stage IB patients remained free of disease. In patients who experienced relapses, the mean disease-free interval was 20 months for stage IA and 17 months for stage IB. All patients with stage IIB experienced multiple relapses and only three of seven were alive after 6 years, despite additional x-ray or cytotoxic therapy. The observation in this study that five of nine stage IA patients and ten of twenty-six stage IB patients have remained in continuous remission after a single PUVA course for up to 79 months indicates that PUVA may induce long-lasting disease-free intervals if used in the early stage of disease. However, the observation period still does not prove whether permanent cure can be achieved in some cases or not.

Dysplastic nevus syndrome with multiple primary amelanotic melanomas in oculocutaneous albinism.

Melanomas are rare in albinos, although the incidence of solar radiation-induced skin tumors is extremely high because of the absence of photoprotective melanin. This report describes a 40-year-old white woman with tyrosinase-negative oculocutaneous albinism who developed four primary amelanotic melanomas--three of the superficial spreading and one of the nodular type-and, in addition, displayed nevi that exhibited histologically the characteristic features of dysplastic nevi. The concomitant occurrence of multiple primary melanomas and several dysplastic nevi classifies the patient's condition as "dysplastic nevus syndrome," which to our knowledge has not been described in albinism so far.

Safety and therapeutic effectiveness of selected psoralens in psoriasis.

This review summarizes the most important facts regarding the clinical effectiveness of oral photochemotherapy for psoriasis with UV radiation at 320-400 nm (UVA) and selected furocoumarins. The most widely used compound is 8-methoxypsoralen (8-MOP); its effectiveness has been documented by many clinical trials. Oral 5-methoxypsoralen (5-MOP) has been evaluated as an alternative drug because it is less erythemogenic and thus reduces the danger of accidental over-exposure. It has been as effective as 8-MOP in clearing psoriasis, but the UVA doses required were considerably higher. Although oral 4,5',8-trimethylpsoralen does not clear psoriasis satisfactorily, excellent treatment results have been recorded when it was applied topically. The 3 drugs produce bifunctional adducts with DNA (cross-links), which may be of particular importance for mutagenesis and tumor formation. In an attempt to reduce possible oncogenic hazards, investigators are currently testing non-crosslinking furocoumarins for their therapeutic effectiveness. At present, these compounds are available for topical use only. 3-Carbethoxypsoralen has been reported to produce excellent treatment results by others but was ineffective in our clinical trials. Similarly disappointing was the application of 4,5'-dimethylangelicin and 5-methylangelicin. Although monofunctional compounds also inhibit cell proliferation in vitro, it appears that cross-linking is a prerequisite for the therapeutic success in psoriasis. The actual important of cross-links, however, remains to be clarified.(ABSTRACT TRUNCATED AT 250 WORDS)

[UV-B phototherapy in psoriasis].

67 patients, 41 with chronic plaque-psoriasis and 26 with acute eruptive or seborrhoic psoriasis, were treated in a stand-up unit equipped with UV-B fluorescent bulbs. 92 per cent of the latter group of patients showed satisfactory results (13 exposures, total UV-B dose: 464 mJ/cm2), whereas only 46 per cent of the patients with plaque psoriasis responded comparably (19 exposures, total UV-B dose: 994 mJ/cm2). A split comparison study of PUVA and UV-B on plaque-psoriasis revealed the superiority of PUVA in both initial treatment phase and duration of the remission. For certain types of psoriasis, UV-B phototherapy represents an alternative option. With regard to long-term hazards, however, as carcinogenicity in particular, UV-B has to be judged with the same scrutiny as PUVA.

Histiocytosis X cells in eosinophilic granuloma express Ia and T6 antigens.

Morphologic similarities between histiocytosis X (HX) cells and epidermal Langerhans cells (LC) have led to the hypothesis that HX represents a proliferative disorder of LC. In order to prove the validity of this assumption, we tested single cell suspensions isolated from an eosinophilic granuloma type HX lesion for the presence of various antigenic determinants defined by monoclonal antibodies using an immunoelectron microscopic technique. An anti-Ia reagent reacted with essentially all histiocytic cells and a small portion of lymphocytes whereas plasma cells and eosinophils were negative. T6 antigen, in contrast, was disclosed exclusively on HX cells either with or without Birbeck granules. Pan-T cell-reagent OKT3 reacted only with small lymphocytes. The finding that HX cells from eosinophilic granuloma lesions are the only cells that have the identical surface marker equipment as epidermal LC (Ia antigens, T6 antigen, Fc-IgG, and C3 receptors) strongly supports the concept that these cells are derived from the LC lineage.

Effects of whole body UV-irradiation on oxygen delivery from the erythrocyte.

In 16 healthy caucasian volunteers (mean age: 22.2 years) the influence of whole body UV-irradiation on the oxygen transport properties of erythrocytes was investigated. Four hours after irradiation with UV (using the minimal erythema dose, MED) no variation of haemoglobin concentration, hematocrit, mean corpuscular haemoglobin concentration, pH or standard bicarbonate could be found, whereas inorganic plasma phosphate (Pi), calcium, the intraerythrocytic 2,3-diphosphoglycerate (2,3-DPG), the activity of erythrocytic phosphofructokinase (PFK) and pyruvatekinase (PK) increased significantly. The half saturation tension of oxygen (P50-value) tended to increase. The increase of Pi causes--via a stimulation of the glycolytic pathway--an increase in 2,3-DPG concentration and thus results in a shift of the oxygen dissociation curve. It is therefore possible to enhance tissue oxygenation by whole body UV-irradiation.

The syndrome of milker's nodules in burn injury: evidence for indirect viral transmission.

Four patients with first- to second-degree burns developed multiple unusual nodular lesions confined to the burned areas 2 to 3 weeks after the accident. Electron microscopy disclosed viral particles within epidermal cells. These were identified as subgroup II poxvirus. Viral culture established the diagnosis of paravaccinia (milker's nodule) infection. Since none of the patients had had direct contact with infected animals, but had been in contact with contaminated objects, an indirect viral transmission, previously not reported for milker's nodules, appears the most likely mode of infection.
