Pulmonary meningioma. Immunohistochemical and ultrastructural features.

Two cases of solitary primary pulmonary tumors showing the immunohistochemical and ultrastructural features of meningothelial meningiomas are presented. The benign clinical and radiologic course, the negative computed tomography scan of the brain (case 1), and negative neuropathologic investigation (case 2) support the diagnosis of a primary pulmonary meningioma rather than a metastazing malignant intracranial meningioma. Negative neuroendocrine markers (neuron-specific enolase, chromogranin, bombesin) and the lack of neurosecretory granules by electron microscopy confirm the diagnosis of this rare pulmonary tumor.

[The high autopsy rate in Vienna].

In 1983 34% of all persons who died in the Republic of Austria were autopsied. We examined the frequency of postmortem examinations in the capital city, Vienna, between 1983 and 1987 in view of this high overall autopsy rate. The annual autopsy rate in all 9 pathology institutes and the department of legal medicine of the University of Vienna was on average 51 to 53%. There was a slight statistically verifiable decline in 1987. The reason for this high necropsy rate is that Austrian law permits the autopsies without the consent of next of kin if it appears indicated for medical, scientific or educational reasons; further more the fact, that the chairman of the department is at the same time the coroner for the hospital. A high autopsy rate is necessary to maintain a high standard of diagnostic accuracy even in modern medicine. The diagnoses were inaccurate or incomplete in 15% of all cases. The high autopsy rate is the basis for extremely accurate mortality and morbidity statistics and provides the basis for long range public health planning. Medical students develop an insight in disease patterns, which cannot be gained in any other way. The pathology departments of Vienna introduce a new city-wide computer-supported diagnostic file system, which will provide a baseline for comparative scientific studies.

Austria

[The differential diagnosis of lung ossifications].

In addition to the well-known secondary ossification of pulmonary depositions of calcium, and interstitial ossification in the case of mitral valve disease, there is also a rare idiopathic ramifying ossification in the lungs, the pathomorphology of which is reported in this case history.

Shaft fissures due to implantation of cementless total endoprostheses of the hip joint. An experimental study.

Since 1982, at the Orthopedic Department of the Pulmologic Center of the City of Vienna we have used the Zweymüller shaft system and the pyramidal threaded socket by Schwägerl when implanting cementless hip endoprostheses. Some patients repeatedly complained about pains in the shaft region within the first postoperative months, though these complaints could not be explained clinically or radiographically. On 15 human cadavers we implanted a total of 20 Zweymüller shafts and examined the femurs for ruptures of the shaft. Our special interest was the course of the fracture lines, their localization, and their display on roentgenograms. It appeared that fissures of the shaft are easily caused by the forced driving-in of the prosthesis. In about half of all cases the fissure cannot be seen on roentgenograms because of the site is covered by the implant itself. The fracture lines do not always begin intertrochanterally or from the calcar femoris; therefore, they cannot be recognized during surgery. This problem should be considered during implantation as well as during postoperative remobilization.

[Hamartoma of the lung. Dysontogenetic lesion or true tumor? Study of 100 personal cases].

100 hamartomas of the lung (89 operative and 11 autopsy specimens were examined in detail and all parameters statistically correlated. Clinical identification and diagnosis have improved continuously in the last two decades. Men are affected more frequently. The hamartomas of the middle lobes were exceptional from several points of view (size, growth form, age). The typical hamartoma does not become malignant. Those patients who have a co-existent bronchial carcinoma are generally younger than those who have malignant disease only. Precise investigation of various individual criteria demonstrated that the replacement of the old dysontogenesis theory through the concept of pure tumorigenesis (mesenchymoma) is not justified.

[Diagnostic problems and incidence distribution in primary lung tumors].

Reasons for discrepancies in histopathologic findings in an international study are explored in relation to the author's own analysis of 4,485 operative or post mortem specimens. A frequency distribution of these tumours is presented according to the WHO-classification system.

Adenocarcinoma

[Growth rate of hamartomas of the lung].

Investigations of 89 hamartomas of the lung showed that those located in the middle lobe were earlier treated surgically than those located in other regions of the lung. Based on the differences between means of the size of the hamartomas and of the age at the time of surgical intervention, it could be calculated that typical hamartomas have a doubling time of their diameter of about 14 years.

[Non-virilizing hormonal therapy in women with secondary disorders of sexual responsiveness].

26 women with secondary disturbance of sexual responsiveness were treated mainly with dehydroepiandrosterone preparations. The sexual tonus was tested before and after treatment by a sexual score system previously described. Urinary 17-ketosteroid and plasma testosterone fractions were controlled in 17 patients; these values were, in general, below the normal range before treatment. Treatment was considered to be successful in 17 patients, while in the remaining 9, who mainly belonged to the group of younger patients, no success was achieved. However, the results point out that therapeutic success with hormones of low biological activity can be expected only after numerous months of treatment. The pattern obtained on determination of hormonal parameters usually corresponded to the results of treatment.

[127 operated small lung cancers (1973-1980)].

We report upon 127 small-size carcinomas which have been operated from 1973 to 1980 at the "Pulmologisches Zentrum der Stadt Wien". The different parameters of tumours are brought into relation one to each other. 61 patients on which surgery was performed during the years 1973 to 1976 have been observed for 5 years or more. It has been investigated whether the time a patient survives is influenced by the size, by the location, by the type or by the stage of the tumour.
