Asialylated IgG in the serum and synovial fluid of patients with rheumatoid arthritis.

A novel enzyme linked immunosorbent assay has been developed that detects an asialylation change in the IgG molecules from patients with rheumatoid arthritis (RA) by the binding of biotinylated RCA 1 (ricin) to capture IgG. Elevated levels of asialylated IgG were detected in paired serum and synovial fluids (SF) of patients with RA. When compared with healthy controls, particularly in the SF (p less than 0.05) correlations of the asialylation change were found in paired samples (p less than 0.01) and between asialylated IgG and rheumatoid factors (p less than 0.01), C-reactive protein (p less than 0.02) and the Lee and Ritchie indices p less than 0.02 and 0.05, respectively. We found the proportion of asialylated IgG was higher in DR4 positive patients, indicating a possible association of these individuals to a sialyl transferase abnormality.

[Erroneous admission of HIV patients into a psychiatric hospital].

The case reports of four HIV positive psychiatric inpatients are presented, in the light of which widespread contact fears and avoidance reactions in lower offices of justice, social and health administration become evident. Attention is drawn to possible misuse of psychiatric hospitals for quarantine purpose as long as there are no clear provisions to deal with the increasing group of HIV subjects.

Markers for human placental trophoblasts in two-dimensional gel electrophoresis.

Primary cultures of trophoblasts established from human term placentae showed high viability and reproducibility. Two-dimensional gel patterns obtained by metabolically labeling the trophoblasts with [35S]-methionine demonstrated that their pattern of gene expression was stable during the 6-d period investigated. Gel analysis demonstrated the keratins 7, 8, 14, 17, 18, and 19. Analysis of the gel pattern confirmed the presence of a small proportion of contaminating fibroblasts and lymphocytes. The gel patterns were compared with that of skin fibroblasts, peripheral lymphocytes, and epithelial cells to identify a group of proteins that are enriched in the trophoblasts and thus may be used as marker for these cells.

Semliki forest virus: cause of a fatal case of human encephalitis.

A fatal case of human encephalitis has been observed for which our results indicate that Semliki Forest virus (SFV) was the etiologic agent. This is surprising in view of the fact that this virus, which has been widely studied, was believed to be one of the arboviruses nonpathogenic for man. Described are the clinical course, the virological examinations performed, and the histopathological findings in the central nervous system.

[Peripheral nerve changes in Tangier disease. Comparative light-, electron-microscopic and cytometric studies (author's transl)].

A sural nerve biopsy was performed in a 55-year-old male patient with Tangier disease (familial-lipoprotein deficiency). Light-microscopy showed an increase in the endoneural connective tissue and a loss of nerve fibers indicating a chronic peripheral neuropathy. Electron-microscopy revealed an accumulation of lipid droplets within Schwann cells of myelinated and unmyelinated nerve fibers. When compared with age-matched controls the myelinated fiber density was reduced with a relative preponderance of small myelinated fibers. In addition, distributional cytometric studies of nerve fibers in relation to the perineurium and endoneurial capillaries showed: Contrary to 4.6-7.5 micron thick nerve fibers, which accumulated in the center of the nerve fascicle, small (0.5-4.5 micron) and large (7.6-10.0 micron) fibers lay nearby the perineurium. The measured increase in small myelinated nerve fibers around endoneurial capillaries may be explained as a sign of regeneration.

[Clinical and experimental evaluation of hydrophilic contact lenses for continuous-wear (author's transl)].

In a clinical and experimental study we tested a new contact lens for continuous-wear. The modified HEMA-lens ("Permalens", Global Vision Ltd, Southampton/GB; Cooper Laboratories, Düsseldorf) has a water content of 74% at pH 7.4. The oxygen transmissibility is, depending on the thickness of the lens, 38--67% of the oxygen in the air. In a first trial we had to eliminate 6 patients due to wrong fitting (much too steep) or dry eyes. After development of a new guide for selection of lens base curves 10 patients with monocular aphakia (10 eyes) and 12 patients with mild myopia (24 eyes) were fitted with the hydrophilic soft contact lens for permanent wear. From a total of 34 eyes in 11 cases (32%) the lenses were removed due to complications (1 conjunctivitis, 1 keratitis, 1 vascularization) or due to lack of cooperation. In 23 cases (68%) the continuous-war lenses were well tolerated without complications for 5 months. In a comparative study in vitro we tested the effect of the contact lens on the viability (swelling rate, intracellular membrane potential of the epithelium) of the isolated human cornea. A damaging effect of the lens on corneal function was not observed. The hydrophilic soft contact lens can be fitted for continuous-wear.

[Vaginal exstirpation of uterus following gynaecological or obstetrical laparotomies (author's transl)].

Of a total of 3,213 vaginal exstirpations of the uterus, this operation was performed in 132 women following gynaecological laparotomies. In spite of this, the incidence of complications was not increased. It is concluded that the advantage of the vaginal route of operation in hysterectomies need not be refused to patients in whom laparotomies have been performed if certain conditions are observed.

[Lasermicrosurgery of the flattened anterior chamber (author's transl)].

The method of laser-microsurgery for restoration of lost anterior chamber in aphakic pupillary block and malignant glaucoma is once more reported. The problems of the flattened anterior chamber are discussed. Principle, technique, and indication of laser-coagulation of the iris are demonstrated. A short discussion of the method, description of late observations and instructions for postoperative treatment are given. In each of the 23 eyes this dangerous postoperative complication could be abolished in a simple manner without any risk. This operation must be considered as the method of choice.

Anterior Chamber

Alanine formation and alanine aminotransferase activity in the nerve tissue with proliferating macroglia.

In the nerve tissue with proliferating macroglia cells were observed a lowered oxygen consumption, an increased aerobic glycolysis and alanine formation and a higher alanine aminotransferase and glutamate dehydrogenase activity than in the control tissue in the homogenates and in the cell sap fraction. The substrate saturation curves, apparent Km and pH optimum values in the tissue with proliferating macroglia and in the control did not differ from one another. The authors assume that a higher alanine aminotransferase activity in the tissue with macroglia proliferation can reflect either a higher synthesis of the enzyme in the altered tissue, or a predominance of glial elements in the altered tissue possessing a higher alanine aminotransferase activity than the nerve cells.

[Problems in treatment of ocular and cerebral vascular disturbances (author's transl)].

With regard to a paper of Leydhecker the basic principles of a combined treatment of ocular and cerebral vascular disturbances with Strophanthin, resp. Digitalis, Vasodilatators, and Hydrocortisone medicaments are again discussed. The influence of hypoxia in vasodilatation, the confirmation of alterations in visual field, as well as the effect of decreased blood viscosity on the circulation are discussed. The most important item, cardiotherapy, is emphasized once again. The peripheral effects of Strophanthin, especially on the brain, are confirmed by communications of other authors. The prognosis of this polypragmatic therapy in long-term observations is also limited.

The corpus luteum periodicum in the bitch: histochemical and biochemical investigations during different phases of the cycle.

The corpus luteum periodicum (c.l.p.) of the bitch was studied using enzyme histochemistry, clinical and biochemical methods. Assessment of variations of the serum progesterone level during different stages of the ovarian cycle and the study of enzymatic activity of the luteal tissue furnished informations on vitality and biosynthetic capacity of luteinizing cells: The period of functional activity of the c.l.p. nearly ceased about day 60 of the cycle (proestrus included) in the animals examined. In the light microscope the neo-synthesis of progesterone in the c.l.-tissue can be made visible by histochemical demonstration of the activity of Delta(5)-3 beta-hydroxysteroiddehydrogenase (3 beta-HSD) only up to a progesterone level exceeding 6.7 ng/ml plasma. The data obtained contribute to our baseline knowledge of the mechanism involved in the etiology of pseudopregnancylike conditions in the bitch.

EM findings on the source of reactive microglia on the mammalian brain.

In traumatic brain lesions microglia cells are often plastered on the outer surface of capillaries. Basement membranes delimit these juxtacepillary cells from pericytes and endothelial cells. In altered nervous tissue many stages in the activation of pericytes may be seen at the same time. It is clearly demonstrable that these pericytes attack the basement membrane vigorously from inside. After penetration of the basement membrane these cells propel themselves between the perivascular feet processes of astrocytes and squeeze between the intracellular gaps, becoming surrounded by the cell processes of the neruopil. The observations suggest that in reactive stages in microglia cells may, at least in part, originate from the perivascular mesenchymal tissue.

Central neuroblastic tumour associated with smooth muscle fibers. A study in the optic and electron microscopes.

The present paper is a study of a microcystic intraspinal tumour that developed in a 4-year-old child. The histologic examination showed the presence of neuroblastic elements developing next to smooth muscle fibers. The tumour had diffusely invaded the meninges, presenting a marked collagen reaction. Ultrastructurally, the neuroblastic elements exhibited an arrangement proper to the neural tube and the presence of a series of osmiophil neurosecretory dense-core vesicles. The fine structural morphology of the smooth muscle fibers was clearly revealed, the cells containing microfibrils and being delimited by a basement membrane. The muscular fibers were visibly in contact with the blood vessels. These aspects demonstrate that the medullomyoblastoma is a malignant bidermal teratoid tumour of the central nervous system. Dysgenesis of the ectomesenchyma may be considered to stand at the basis of the development of this type of tumour.

Scanning electron microscopy in ophthalmology.

In the context the technique of the scanning electron microscopy is described and the results are shown gained by the scanning electron microscope in ophthalmology. It is pointed out that questions specific to surface can be answered quicker and easier, if superificial microscopy together with the scanning electron microscope is used an additional source of information. Scanning electron optical pictures are very useful for ophthalmologic science.

Ciliary Body

[Pigment glaucoma. Scanning-electronmicroscopic findings. (author's transl)].

In case of a patient suffering from pigment glaucoma trabeculectomy was performed on both eyes. In one trabecular section the outer wall of Schlemm's canal, and in the other the trabecular framework were investigated by scanning-electronmicroscopy. The intertrabecular spaces were largely blocked by pigment granula, which, in some places, were stored in the clump cells of the iris. Even the outer wall of Schlemm's canal was dotted with pigment granula. The scanning-electronmicroscopic findings revealed the pigmental drainage blocking of the aqueous humour and explained the acute pressure rise in the pigment glaucoma with wide iridocorneal angle by an acute obstruction of the already largely blocked intertrabecular spaces.
