[Local recurrences of carcinoma of the breast (author's transl)].

Distant metastases were sought for in 90 patients with local recurrence of breast carcinoma. Only 11 patients had demonstrable spread of the disease at the time of referral. Two-thirds of all local recurrences presented within two years of the primary operation. In 66 out of 90 patients the recurrence was in the chest wall, in 11 cases it was in the chest wall and in the lymphatic system and in only 13 cases was it primarily in the regional lymph nodes. The recurrence rate depended mainly on the previous treatment of the primary tumour: 42 recurrences were found in those cases treated only by surgery, 46 in cases treated surgically and with low-dose post-operative irradiation and only two in patients treated with over 4500 rad to the area of subsequent recurrence.

[Radiation therapy of humeroscapular periarthritis (author's transl)].

The effectiveness of radiation therapy in 233 cases with periarthritis humeroscapularis is reviewed in a prospective study from the Basel canton hospital. The patients are studied with regard to different parameters, such as duration of the anamnesis, importance of the dose, moment of evaluation of the results and influence of a 2-series technique. The total result, comprising 43.3% cases without complaints, 30.9% being better, and 25.8% whose troubles had not changed, is compared with the compiled statistical data of 4957 cases found in literature. The gonadal load is discussed on the basis of our own investigations.

[Radiotherapy of esophageal neoplasms. Review of the 1st post-therapeutic year].

The first post-therapeutic year in 33 patients with esophageal cancer treated by radiotherapy alone is analyzed. Effectiveness of treatment was measured by clinical symptoms, radiological findings, and autopsy results. Out of a group of 33 patients, 36.4% showed no subjective or objective symptoms 1 year after commencement of therapy. Based on our own experience and the literature, the concept of therapy in esophageal cancer is discussed.

[The question of the possibility to reproduce field positions in high-voltage therapy (author's transl)].

In order to obtain informations about the possibility to reproduce field positions in high-voltage therapy, controls were made by therapy films on several patients during the irradiation with a linear accelerator and a telecobalt unit. An incorrect position of the fields and/or lead satellites was found in 51% of the controls. The authors discuss possible reasons and their consequences for the radiotherapeutic practice. It must be taken into consideration especially when therapy plans are elaborated, that their realization may cause difficulties in the daily practice.

[Radiological features of the rectal manifestations of myelofibrosis(author's transl)].

In addition to the well known changes in bone structure, myelofibrosis can result in extramedullary tumour formation; in the gastr-intestinal tract, proliferation of reticulum can lead to fibrotic changes. This results in appearances similar to linitis plastica or Crohn's disease. In the discussion, the authors refer to tumours in other organs which have been described in the literature and which can be demonstrated radiologically.

[Radiotherapy of mammary cancer related to tumor size, tumor localization and surgical management (author's transl)].

Postoperative irradiation of the regional lymphatics is indicated when suspected positive lymph nodes remain after mastectomy. Postoperative chest wall irradiation is carried out in cases where staging of the tumor is T-3 and T-4. A local tumor control can be achieved with irradiation as the only means of therapy. Preoperative radiotherapy seems to lead to favorable results and should be encouraged in cases of advanced breast cancer.

[Pretherapeutic evaluation of bone matastases in patients with potentially curable breast cancer (author's transl)].

A pretherapeutic evaluation of skeletal metastases should be carried out in all patients with proven mammary cancer including early stages. The scintigraphy is more reliable for detecting bone metastases than the conventional roentgenological survey. We recommend to employ complementally skeletall scintigraphy and conventional X-ray survey in the search for bone tumors. Negative scintigraphical results do not require any supplementary research. In case of a pathological scan or skeletal pain, a target roentgneological examination should be carried out to differentiate between benign diseases and neoplatic lesions.
