[Formal uniform organization of quality parameters in public health--a methodological suggestion].

UNLABELLED: The relevance of many suggestions for quality management of medical practice and health care is still unsatisfactory. The mass of all the existing suggestions for quality assurance and quality control made a homogeneous method even more difficult all efforts should be directed at finding quality parameters for the most frequent performances in basic care (family doctors). A uniform method and setting up an institute will be important for coordination between specialists, medical societies and relevant institution for exchanging experiences, practice and findings. OBJECTIVE: Suggestions for a homogeneous method to develop quality parameters of medical care.

[Worries about the job, unemployment and health status--initial results of a study in Saxony].

1628 patients of general practitioners in Saxonia were asked and clinically examined in 1993 to find out the context between unemployment and health conditions. Surprisingly, the working people who "only" worry about losing their jobs were more concerned than the unemployed ones. However, more than 50% of the women and 41% of the men were troubled by this problem. The worried people reported about 23% a bad health condition but only 20.5% of the unemployed and 14% of the working people who had no fear of losing their place of work. In addition, those who were worried suffered from high social strain and more chronic diseases. In East Germany, this population is more disposed to health risks than any other one today, especially the unemployed. The family doctors will often be the first and only persons who will be able to recognise these connections.

[Concept of medical students on partnership and marriage and subsequent changes].

An analysis carried out in Dresden in 1991 gives an insight into the ideas that medical students have in respect of marriage/partnership and family. In many respects there are no differences compared with the ideas governing the minds and emotions of young adults. A certain period of time will elapse (the length of this period has already been envisaged) before we can find out eventual differences and to what extent. More than previously students will have to weight the pros and cons of profession and family. (Own) children now occupy the eighth rank of importance in the sequence of values: this may be a first sign of such re-assessment. No special features were seen in respect of the desire to marry and to have children. Female students are more likely to think in traditional terms as far as marriage and partnership are concerned whereas they think along update lines in respect of the demands made on their partner and on the manner in which partnership should be structured, with particular emphasis on equal rights. Male students are more often in favour of partnership or single life but rather traditional in their ideas of partnership.

[More effective surgical care and its regional organisation (author's transl)].

The decrease of the number of surgical cases within the past few years was not essentially due to the decrease of the number of hospital surgeons or to the decrease of the number of beds in surgical units. Decrease in the length of stay and increase in bed occupancy rate might effectively respond to increasing demands. A community comprising about 60 000 inhabitants will occupy a fulltime outpatient surgeon. Smaller communities should be served by an integrate inpatient-outpatient service. There is no need for each surgical unit to run a separate emergency service of its own.
