Prevalence of hypertension in a low income settlement of Karachi, Pakistan.

OBJECTIVE: To assess the prevalence of hypertension in a low-income community in Karachi. METHODS: Cross sectional survey, through multi-stage sampling of 857 adult subjects in 405 households was conducted between April and September 2002. RESULTS: The overall prevalence of hypertension was 26% (95% C.I. 23, 29), the prevalence among males (34%) was higher than females (24%). The mean age of participants was 35 +/- 14 years, the prevalence of hypertension increased with age. Proportionately there were more cases of hypertension among male participants over 35 years of age as compared to female participants of the same age (p < or = 0.001). Fifty-eight percent of hypertensives were unaware of their hypertension. None of the hypertensive subjects who were aware of their condition had blood pressure under 140/90 mmHg. Stratified analysis revealed significant risk factors for hypertension. Hypertension was 1.7 (OR 95% C.I. 1.14, 2.42) times more common among males then females. Males were 1.7 (OR 95% C.I. 1.06, 2.6) times less likely to have been aware of their hypertension status. Age analysis revealed that the prevalence of hypertension increased with age and hypertensive subjects were 5.6 (OR 95% C.I. 3.9, 8.1) times more likely to be over 35 years of age. CONCLUSION: There is a need to control hypertension and prevent its complications through effective community interventions. The survey results indicate high prevalence and poor control of hypertension in the community, but more research is required to understand the reasons behind this phenomena.

The incidence of vaccine preventable influenza-like illness and medication use among Pakistani pilgrims to the Haj in Saudi Arabia.

Over the 33-day duration of the 1999 Haj in Saudi Arabia, we collected daily health status reports for 2070 Pakistani pilgrims over 13 years of age, 54% of whom had elected to receive influenza vaccine immediately before departing for the Haj. We calculated vaccine preventable outcome incidence as the difference in attack rates between vaccinated and unvaccinated persons. The incidences of vaccine preventable influenza-like illness (sore throat in combination with cough or fever of at least 38 degrees C), fever, and any symptom of upper respiratory infection were 22, 17, and 24 per 100 pilgrims per Haj. For every 100 persons who attended the Haj, 17 had a course of antibiotics and 23 had a course of nonprescription cold medication that was preventable with influenza vaccine use. Influenza leads to significant morbidity and medication use among Haj pilgrims. Vaccine against influenza should be considered for pilgrims before entry into Saudi Arabia.

Seroepidemiology of five major zoonotic parasite infections in inhabitants of Sidoarjo, East Java, Indonesia.

We conducted a seroepidemiological survey of zoonotic parasite infection in inhabitants of East Java, Indonesia. The subjects of the survey were 244 persons selected from visitors to Sidoarjo City Hospital in East Java between May 1992 and October 1993. Ninety-seven had diarrhea and the rest came to the hospital for routine check-ups. All serum samples were tested for antibodies against five zoonotic parasites: Toxoplasma gondii, Entamoeba histolytica, Toxocara canis. Angiostrongylus cantonensis, and Anisakis species. Tests used were enzyme-linked immunosorbent assays (ELISA), latex agglutination (LA) test, indirect fluorescence antibody (IFA) test, hemagglutination (HA) test, and gel diffusion precipitation (GDP) test. Some 64% of the subjects had antibodies to T. gondii. The prevalence of antibodies to E. histolytica varied from 2 to 15% depending on the test, but the true rate was probably the 7% or 8% obtained by the HA and IFA tests. The proportions of subjects with positive results were 63% for T. canis, 17% for A. cantonensis, and 11% for the Anisakis spp. The prevalence of antibodies to T. gondii and T. canis was lower in subjects aged 1 to 9 years than in older subjects, probably because the persons in this group had less time to be infected. Antibody titers to A. cantonensis and the Anisakis spp. were high in the juvenile group, perhaps because recent changes in eating habits have increased opportunities for infection.

Osteoma of the lateral pterygoid plate of the sphenoid bone.

A rare case is reported of an osteoma protruding into the pterygomandibular space from the lateral pterygoid plate of the sphenoid bone. The procedure of the diagnosis and treatment is described. In this case, computed tomography (CT) was the most useful in making a differential diagnosis. That the pulling force of the medial pterygoid muscle might be a causative agent in the growth of this osteoma is considered.

[Investigation of the concentration of cefmenoxime transferred to human serum and tissues in oral region].

The concentration transferred to human serum and tissues in oral region after the administration of cefmenoxime (CMX) were investigated. The results were obtained as follows: The mean values of CMX-concentrations in serum at 15, 30, 60, 120 and 180 minutes after the intravenous administration of 20 ml of 5% glucose solution containing 1 g of CMX were 84.1, 52.7, 28.2, 12.7 and 8.81 micrograms/ml, respectively. The half-life times of serum concentration at T1/2 alpha and T1/2 beta were 0.55 and 1.05 hours, respectively. The mean concentrations transferred to gingiva at 15 (1 case), 30, 60 and 120 minutes after the intravenous administration of the same dose of CMX were 49.2, 14.1, 9.93 and 3.48 micrograms/g, respectively. Moreover, the ratios (tissue concentration to serum concentration) were also 67.50, 27.95, 38.46 and 32.28% at each time, respectively. CMX was administered intravenously for postoperative treatments of 13 patients, resulting in good effect from the laboratory data and clinical findings. No side effects were obtained.

Hydration effects on muscle response.

The effect of bathing toad sartorius muscle in hypotonic solutions is studied. The twitch, tetanus force and active stiffness increase as the tonicity decreases from 1.0 R to 0.6 R and then decrease below 0.6 R. Explanation is given in terms of the effect on ATP-ase activity.

Norethisterone acetate and ethinylestradiol in early human pregnancy.

In a double-blind randomized trial, 25 women (8-9 weeks pregnant) received 20 mg norethisterone acetate (NET-AC) and 0.04 mg ethinylestradiol (EE2), 25 women receiving placebo. The patients were followed by ultrasound and the products at curettage studied macroscopically and microscopically. In an open trial, ten patients (5-9 weeks pregnant) received NET-AC + EE2, 11 serving as controls. The concentrations of plasma progesterone, estradiol-17 beta, 17 alpha-hydroxyprogesterone, norethisterone, and FSH were followed. The frequency of intrauterine hemorrhage in early pregnancy was not affected by NET-AC + EE2. Ultrasound was not reliable when studying the occurrence of decidual hemorrhages during early pregnancy. The pharmacokinetics and bioavailability of NET remain unchanged during early pregnancy. The treatment with NET-AC + EE2 had no effect on the heights and the time courses of the mean plasma values of progesterone, estradiol-17 beta, 17 alpha-hydroxyprogesterone, and FSH. No differences were found between treated and nontreated patients in the pathology of the placenta or decidua as determined by both macroscopic and microscopic examinations. The hypothesis of Papp and Gardo, that decidual hemorrhages may be induced in early pregnancy as a consequence of a "withdrawal effect" of hormonal pregnancy tests, is not supported by the results of this investigation.

Muscle crossbridge action in excitation and relaxation.

Pulse-propagation measurements on a muscle stimulated into tetanus show that the stiffness develops earlier and starts later than the tension. It is concluded that the myosin heads which moves towards the actin filaments during excitation become mechanically attached to them.

Pulse propagation in muscle.

A technique is described for measuring the propagation velocity of a mechanical pulse along a muscle. The pulse is generated and detected by piezoelectric crystals (bimorph benders). Measurement of the time delay for a known crystal separation gives the propagation velocity v (of the order of 100 m s-1) from which the Young's modulus is calculated. Primary results for elastic moduli of frog and toad muscles at 5 degrees C are: 2.6 X 10(6) N m-2 at rest, 3.1 X 10(7) N m-2 in tetanus and 2.1 X 10(7) N m-2 in rigor. Exploratory experiments are also described showing the application of the technique to (i) the development of tetanus, (ii) variation with sarcomere length and (iii) the mechanics of rigor. The pulse technique gives an almost instantaneous measurement of the purely elastic response of a muscle and is thereby of value in basic studies of the mechanism of contraction. It is also a useful non-destructive probe in following the effects of such variables as temperature, fatigue or chemical treatments.
