Strain difference of susceptibility to 4-nitroquinoline 1-oxide-induced tongue carcinoma in rats.

Strain difference of susceptibility to 4-nitroquinoline 1-oxide (4NQO)-induced squamous cell carcinomas of the tongue among Dark-Agouti, Long-Evans, Sprague-Dawley, ACI/Ms, Fischer 344, Donryu and Wistar/Furth rats was surveyed by evaluating the survival times, incidences and sizes of developed tumors as markers of susceptibility. Administration of 4NQO dissolved in drinking water induced squamous cell carcinomas in various sites of the upper digestive tract mucosa of all the experimental male and female rats of the seven strains. Regarding the mean survival times, Wistar/Furth rats survived much longer than any other strain of rats, and Dark-Agouti showed the shortest survival. The incidence of large, mass-type carcinomas of the tongue of Dark-Agouti rats was higher than in any other strain of rats, while that of Wistar/Furth rats was the lowest. Subsequently the mitotic activity and bromodeoxyuridine incorporation in the tongue epithelium of Dark-Agouti and Wistar/Furth rats were estimated after a short-term administration of 4NQO. There was a pronounced difference between the two strains of rats, because the proliferative responses of the tongue epithelium of Dark-Agouti rats to the 4NQO stimulation were much higher than those of Wistar/Furth rats. These results indicated that there are marked differences in the susceptibility to 4NQO-induced tongue carcinoma among the seven strains of rats, and that Dark-Agouti and Wistar/Furth rats could be useful as models of highly and poorly susceptible strains, respectively, for further genetic analysis.

[Left ventricular filling disturbances in cardiac amyloidosis: a study of atrial sound and diastolic inflow velocities].

Cardiac amyloidosis is characterized by left ventricular filling disturbances in a relatively early stage. To investigate such disturbances more precisely, we studied atrial sound and left ventricular inflow velocity patterns. Twelve cases diagnosed as cardiac amyloidosis according to the clinical criteria including rectal biopsies and serum amyloid proteins or at autopsy were reviewed and analyzed. Their mean age was 60.9 +/- 12.5 years. Twelve age-matched cases with hypertrophic cardiomyopathy (HCM) served as the controls. We measured the amplitude of atrial sound by low-frequency phonocardiograms and the ratio of the heights of the A wave of apexcardiograms (ACG) to the total amplitude of the ACG. The mitral inflow velocity patterns were recorded using pulsed Doppler echocardiography. The rapid filling wave (R), atrial filling wave (A) and the ratio of A to R (A/R) were measured. In the amyloidosis group, atrial sound moderately increased in 2 cases, it was faint in 9 and not manifest in the remaining one. The A wave in the amyloidosis group was significantly smaller than that in the HCM group (p < 0.001) (12.4 +/- 3.9 vs 22.4 +/- 5.6%). In the left ventricular inflow velocity patterns, the R in amyloidosis was smaller than that in HCM (41.7 +/- 16.0 vs 56.4 +/- 12.1 cm/sec) (p < 0.02). The A in amyloidosis was also smaller than that in HCM (40.5 +/- 13.4 vs 58.1 +/- 13.0 cm/sec) (p < 0.006). The A/R was 1.0 +/- 0.34 in amyloidosis and 1.1 +/- 0.32 in HCM (N.S.). A case of endogenous Enterococcus faecalis endophthalmitis.

A case of successful treatment by vitrectomy and lensectomy of early-stage endogenous endophthalmitis caused by Enterococcus faecalis was reported. The case was a 50-year-old man with poorly controlled diabetes. Following T-tube drainage for a necrotic cholecystitis operation, he developed iridocyclitis in both eyes as well as fever. At the time of his first visit to our clinic, his right eye had already lost light perception. His left eye had visual acuity recognizing hand movement, marked uveitis, complicated cataract, and dense vitreous opacity. As gram-positive cocci were isolated from the aspirated vitreous, lensectomy and vitrectomy under irrigation with antibiotics were conducted. After the administration of systemic postoperative antibiotics and human immunoglobulin, the patient showed remarkable improvement in the state of his ocular fundus. By 60 days after the surgery, the visual acuity of his left eye recovered to 0.2. Endogenous Enterococcus faecalis endophthalmitis has rarely been reported in Japan. The diagnosis and treatment of this disease with reference to the above findings were discussed.

Cataract Extraction

Study of suppressive effect of intravenous fluconazole on endogenous Candida endophthalmitis in rabbits.

The effect of intravenous fluconazole on endogenous Candida endophthalmitis in rabbits was investigated. Preventive and therapeutic experiments were carried out. In the preventive series, rabbits were injected intravenously with 5 mg/kg of fluconazole at 30 minutes, 1 day and 2 days after intravenous inoculation with Candida albicans spores. The control group received no medication. No treated rabbits developed ocular lesions and no Candida spores were isolated from the treated eyes. On the other hand, all control rabbits developed bilateral chorioretinitis and C. albicans was isolated invariably from the control eyes. In the therapeutic series, intravenous fluconazole (5 mg/kg body weight) was administered from 3 to 6 days after inoculation. All rabbits developed chorioretinitis and Candida spores were isolated from all eyes. Therefore, the results of this study prove that intravenous fluconazole is more effective in preventive use than in therapeutic use against endogenous Candida endophthalmitis in rabbits.

[Concordance of risk factors for cerebro-cardiovascular diseases among spouses].

The present study was carried out to clarify the concordance or similarity of risk factors among 298 spouse pairs in a random sample who lived in Mizobe town of Kagoshima Prefecture. A sample of 298 spouse pairs was surveyed by using the house-to-house interviewing method and was classified into three groups by the marriage duration. The following results were obtained. 1) Statistically significant partial correlation coefficients were demonstrated among the spouses for blood pressure in the present study. However, they were smaller than among spouses in remote islands, this fact suggested that there was no higher spouse concordance in this surveyed area than in remote island areas, and also that there might be a difference in the shared environment of spouse pairs and the genetic effect on blood pressure between the former area and the latter. 2) In regard to the intake of salt in miso soup and the ratio of sodium to potassium (Na/K), an increasing gradient of partial correlation coefficients for the risk factors among the spouses was observed with increasing marriage duration, suggesting that the intake might be caused by the shared environment on the basis of a similar dietary pattern for the spouse pairs.

Aortic dissection associated with aortitis syndrome.

A 50-year-old woman with aortitis syndrome complicated by aortic dissection, is reported. The dissection was observed at the level of the descending thoracic aorta by aortography and at the intimal side of the dilated aorta on CT. An aneurysm of the right subclavian artery and a diffusely thick wall of the abdominal aorta were also observed. This case suggests that the uneven wall of the aorta in aortitis syndrome might be dissected at the intimal side by dilatation.

Two cases of cicatricial pemphigoid showing atypical immunofluorescent findings.

We describe two patients with the clinical symptoms of cicatricial pemphigoid (CP). Biopsy specimens of the conjunctiva were taken. Histologic examination revealed subepidermal bullae and infiltration of inflammatory mononuclear cells. Direct immunofluorescent study showed immunoglobulins bound to the basement membrane zone (BMZ) in these patients. The patients also had intercellular immunoglobulin deposition in the conjunctival epithelium. No circulating anti-BMZ antibodies were detected, but one patient had a circulating antiintercellular antibody. Rare cases of CP with atypical immunofluorescent findings are reported.

[Endophthalmitis in Japan--a nationwide study with reference to type and etiology].

A nationwide clinical study on bacterial and fungal endophthalmitis was done with special reference to their type and etiology. Two hundred and eighty cases of endophthalmitis were reviewed. All cases were divided into two categories such as exogenous (trauma, surgery, and ulcer) and endogenous (metastasis). Penetrating trauma was responsible for 93 cases (33.2%), surgery for 88 cases (31.4%), ulcer for 11 cases (3.9%) and metastasis for 88 cases (31.4%). Out of the 280 cases, 79 cases (28.2%) were microbiologically diagnosed through aqueous or vitreous tap or biopsy. The total numbers of isolated organisms amounted to 87 consisting of 43 Gram-positive bacteria, 30 Gram-negative bacteria and 14 fungi. The positive isolation was 31 out of 86 (36%) from the aqueous and 52 out of 103 (50%) from the vitreous. Exogenous types associated with trauma, surgery or ulcer were mainly caused by Gram-positive bacteria of 37 strains, led by coagulase-negative staphylococci in 14 cases. On the other hand, endogenous cases were mainly due to Gram-negative bacteria of 14 strains including Klebsiella pneumoniae in 5 cases, E. coli in 4 cases and fungi of 11 strains including candida spp in 9 cases.

[A case of metastatic Enterococcus faecalis endophthalmitis].

Pattern of medical care services utilization in Fiji.

The present study was designed to analyze sociodemographic and health related factors that influence the utilization of medical care services in Fiji by using the method of discriminant function. Some sociodemographic and health related independent variables had significant effects on the two patterns of modern and traditional medical services utilization. Discriminant function coefficients for sex, age and race are large enough to suggest the importance of discriminating between two patterns of medical care utilization. This suggests that traditional medical service is coexistent with modern medicine in Fiji and might play an extremely important role in providing health care for rural communities in the same way as modern medicine.

[The suppressive effect of intravenous fluconazole against endogenous candida endophthalmitis in rabbits].

The therapeutic effect of intravenous fluconazole against endogenous candida endophthalmitis was investigated in rabbits. Twelve albino rabbits and a candida strain isolated from the human vitreous were used. Endogenous candida endophthalmitis was made by intravenous inoculation of 8 x 10(5) candida albicans spores in rabbits. In the first of two series of experiments, three rabbits out of six were injected intravenously with 5 mg/kg.b.w. of fluconazole at 30 minutes, 1 day and 2 days after inoculation. Another three control rabbits received no medication. The clinical course of the eyes was observed for a week and the viable cell numbers of candida in the eye were counted at 7 days after inoculation. Three rabbits treated with fluconazole developed no ocular lesions and no candida spores whatsoever were isolated from the eyes. In contrast, three rabbits in the control group developed bilateral chorioretinitis and candida albicans was isolated from each of 6 eyes. In the second series, three out of six rabbits were injected intravenously of the same dose of fluconazole as described above for four days starting at 3 days after inoculation, when chorioretinitis was first observed. No medication was given to another three rabbits as a control. All six rabbits in this series developed chorioretinitis similarly and candida spores were isolated from all eyes at 7 days after inoculation. These results show that intravenous fluconazole is effective in the prevention of the endogenous candida endophthalmitis in rabbits but ineffective for these short term therapeutic applications after onset.

[A comparison study of SPECT and MRI in remote effects of chronic cerebrovascular disease].

A comparison study of SPECT and MRI was performed to investigate remote effects in 10 patients with chronic cerebrovascular disease. Hypoperfusion in the cerebellar hemisphere contralateral to supratentorial lesions, the overlying cerebral cortex ipsilateral to subcortical lesions, the thalamus ipsilateral to cortical lesions, and the visual cortex distant to optic radiation lesions was demonstrated using SPECT with 123I-IMP. Although hypoperfusion in these remote areas was considered to be related to irreversible degenerative processes in the chronic phase, MR images did not reveal any abnormalities.

Conformation of adenosine 3',5'-monophosphate in solution as studied by the NMR-desert method. II. Self-association and temperature-dependent glycosidic isomerization at pH 7.

A study has been made of the association and the temperature-dependent conformation of adenosine 3',5'-monophosphate (cyclic AMP) in a neutral aqueous (2H2O) solution by means of proton magnetic resonance chemical shift and relaxation. The concentration and temperature-dependent chemical shifts of H(1'), H(2), and H(8), have enabled us to estimate the self-association constant, Ka = 1.1 +/- 0.3 M-1 at 25 degrees C and thermodynamic parameters delta H = -5.8 +/- 1.5 kcal/mol and delta S (25 degrees C) = -19.0 +/- 3 cal/mol per degree. The NMR-DESERT (Deuterium Substitution Effect on Relaxation Times) method has been utilized for the determination of the syn-anti conformational equilibrium in the monomeric state and for the determination of the mutual orientation of the two adenine rings in the dimeric state of cyclic AMP. The molecules were found to coexist with nearly equimolarity or syn-anti conformers and thermal activation of the molecules perturbs the syn-anti conformational equilibrium to comprise the syn form in preference at higher temperature. The glycosidic isomerization (from anti to syn) was found to be characterized both by a positive enthalpy change and by a positive entropy change. The cyclic AMP molecules prefer to take a 'trans-stacking' conformation in the dimeric state where the two molecules are arranged in such a way that the H(2) of one molecule is close to the H(8) of the other.

Chemical Phenomena

Comparison of in vivo and in vitro capability of complement fixation by pemphigus antibodies.

Using complement immunofluorescence, the capability of complement fixation by bound antibodies in pemphigus lesional skin was compared with circulating intercellular antibodies of complement-fixing type in the same patient. Five lesional skin samples from 6 patients, containing bound IgG, gave positive binding of complement (C3) supplied from fresh guinea pig serum, while only one serum sample showed complement-fixing pemphigus antibodies in the circulation. These results support the view that complement-fixing pemphigus antibodies are rapidly localized to the skin and play a role on the production of acantholysis.

Solubilization of 6-oxybenzo(a)pyrene radical by caffeine and DNA as studied by magnetic resonance. Observation of intermolecular charge transfer.

Crystals of 6-oxybenzo(a)pyrene free radical, formed chemically from the hydroxy derivative of the carcinogen benzo(a)pyrene, can be solubilized in aqueous solutions of DNA and of caffeine. ESR spectral evidence indicate that the radicals exist as dispersed monomers associated with DNA and with caffeine. Comparison of NMR spin-lattice and spin-spin relaxation times in the protons of caffeine has given direct evidence that a part of the unpaired electron (at least 10(-4)) is transferred from the radical to the associated caffeine molecule. Simple consideration of Mulliken's charge transfer theory, however, leads to the conclusion that the intermolecular charge transfer is not likely to be a major source of stabilization energy of the complex.

Benzopyrenes

Multinucleate epidermal cells in non-neoplastic dermatoses.

Multinucleate epidermal cells (MEC) originated from keratinocytes were observed histologically in 92 of 197 cases of non-neoplastic dermatoses, such as lupus erythematosus (26 of 70), lichenoid eruptions (26 of 47), Hailey-Hailey disease (5 of 5), psoriasis vulgaris (20 of 49) and so on. Most of these cells had two nuclei, and a few had three nuclei. MEC usually showed perinuclear bands of eosinophilic material which also showed positive staining for epidermal fibres (tonofilaments). In all cases of Hailey-Hailey disease, close correlation between formation of MEC and dyskeratotic tendency was observed. From these findings, it is concluded that the mechanism of formation of MEC in these dermatoses may be similar to that of Bowen's disease, in which dyskeratotic tonofilaments become entangled with the spindles of the mitotic apparatus so that normal cell division cannot take place. It seems likely that MEC are a manifestation not only of malignant dyskeratosis but also of benign one.
