Postpartum glycated hemoglobin A1c and glucose tolerance test in mothers of large babies.

An attempt to study macrosomia and carbohydrate metabolism was made by determination of glycated hemoglobin A1c and by the oral glucose tolerance test (OGTT) in early puerperium. We studied 76 women who gave birth to large babies greater than or equal to 4.5 kg, 74 women whose babies were 3-4 kg as controls, and 36 type II diabetics. The median of HbA1c concentration in the diabetics (8.06%) was significantly higher than in the controls (6.49%), and the large babies' mothers (6.48%), P less than 0.001. No significant difference was found between HbA1c or glucose intolerance in mothers of large babies and mothers with average size babies. HbA1c showed an association with glucose levels in the diabetics and controls, P less than 0.001, but not in the large baby group. Mothers of large babies were as old and obese as the diabetics. We speculate that the relationship of postpartum GTT, HbA1c and gestational diabetes is unwarranted.

Urinary C-peptide in the neonate correlates both to maternal glucose tolerance and to fetal size at birth.

In 18 women with gestational diabetes the variables of an oral glucose tolerance test (fasting and 2-hour blood glucose values and area under the blood glucose curve) performed in the last trimester of pregnancy correlated significantly with the urinary C-peptide excretion during the first 12 hours of the life (r = 0.47, 0.71, and 0.60, respectively). In a combined group with 28 type II pregnant diabetic women there was also a significant correlation between the urinary C-peptide excretion of the infants and their skinfold. Assay of the urinary C-peptide excretion of the neonate, reflecting its insulin production, seems to be a sensitive parameter to study the influence of the maternal carbohydrate metabolism in the offspring.

Emergency hysterectomy in obstetric practice: five year review.

From January 1980 to December 1984, there were 86,483 deliveries and 25 emergency obstetric hysterectomies at the Maternity Hospital, Kuwait. The indications for hysterectomy were placental disorders (64%), uterine rupture (28%), and extension of the lower uterine scar during cesarean section (8%). The incidence of emergency hysterectomy increased with age and parity. Postoperative morbidity was higher with subtotal than with total hysterectomy. The commonest post-operative complication was urinary tract infection. There was one maternal death from consumptive coagulopathy associated with intra-uterine fetal death.

Prospective serological and clinical studies on infants born in Kuwait with an elevated IgM in cord blood.

Little is known about congenital viral and toxoplasmal infections in the developing countries. This study attempts to provide preliminary data on such infections among Arab mothers and infants in Kuwait. Babies born with an elevated IgM in cord blood were selected at birth and then studied prospectively during the first year of life for serological and clinical evidence of viral and toxoplasmal infections. There appeared to be a significant association between the elevation of total cord blood IgM and the selection of cases with cytomegalovirus infection. Demonstration of specific IgM in cord sera was attempted, whenever possible, to provide further evidence of congenital infection and to exclude early postnatal infections. In 18.5% of cases there was an association between serological evidence of infection and certain clinical abnormalities. Apart from cytomegalovirus and rubella virus, no evidence was found for congenital infection by other viruses or toxoplasma in this study.

Maternal plasma proteins, magnesium, zinc and copper concentration at term associated with birth size in Kuwait.

The study was carried out on Arab women who were admitted for delivery to Al-Sabah Hospital in Kuwait. The concentration of maternal serum proteins and magnesium was not correlated to infant birth size. The zinc and copper concentration in maternal serum at term was positively correlated to infant birthweight, length and head circumference. The results suggested the possibility that the mothers in the present study had a marginal zinc and copper nutriture, which when combined the hemodilution due to pregnancy led to serum levels inadequate to supply the requirement of the fast growing fetus.

Seroepidemiology of viral and toxoplasmal infections during pregnancy among Arab women of child-bearing age in Kuwait.

Five hundred and seventy three of 606 (94.6%), 583 of 604 (96.5%) and 575 of 595 (96.6%) women were found to have antibody to rubella virus, cytomegalovirus and herpes simplex virus, respectively. The Geometric Mean Titre (GMT) of haemagglutination-inhibition antibody to rubella virus was 60.97, while the GMT of complement-fixation antibody to cytomegalovirus and herpes simplex virus were 28.49 and 25.01, respectively. Furthermore, 301 of 517 (58.2%) women were also found to possess antibody to toxoplasma (GMT = 10.12) as detected by the passive-haemagglutination test. No significant differences in antibody prevalence or GMT could be found among the various nationalities studied except among Bedouins who showed a significantly higher rate and GMT of antibody to rubella virus and for the Mediterranean Arabs who showed significantly higher rate of immunity to and GMT of toxoplasma antibody when compared to other nationalities. Finally, no significant differences in antibody prevalence or GMT could be found among the various age groups.

Surface ultrastructure of the human placental villi and sites of contact with maternal red blood cells.

With the aid of the scanning electron microscope the possible sites of materno-foetal oxygen transfer were investigated. Fresh small tissue pieces were obtained and processed from at least six regions of central and peripheral parts of the maternal surface of human full-term placentas. The surface ultrastructural features of the syncytiotrophoblast lining the stem and floating villi were surveyed. The microvilli projecting from the apical portion of the syncytiotrophoblast appeared to be highly pleomorphic and showed regional variation in their distribution. On the other hand, our results confirmed the occurrence of non-microvillous areas on the apices of some floating villi. When present, these areas were always free from contact with maternal red blood cells. Maternal red blood cells, however, were seen in close contact only with areas covered with microvilli. Occasionally, impressions ("footmarks") were apparent on some surfaces and detached microvilli were seen adherent to the surface of other maternal red cells which had separated from the villous surface. Our results indicate, therefore, that the microvillous areas of the chorionic villi are the most likely sites for oxygen transfer and that one of the functions of the microvilli is gas transfer across the placenta.

Influence of pregnancy outcome on subsequent pregnancy.

In this study to determine the harmful effect of abnormal pregnancy outcome on the immediately following pregnancy, 573 recently delivered women (with 2347 pregnancies) were interviewed. Data were recorded on their pregnancy outcomes (normal, abortion, stillbirth and congenital malformation), as well as on their interpregnancy intervals. A significant difference was found between the chances of having a spontaneous abortion following a normal outcome (5.5%) against that following a spontaneous abortion (31.1%). Also, the chances of a normal outcome following a normal outcome were 92.4%, compared with 63.9% following a spontaneous abortion. The delivery of a malformed baby is associated with a larger proportion of abortion and congenital malformation in the subsequent pregnancy, while stillbirth was followed by a larger proportion of abortion and stillbirth. A longer interpregnancy interval did not appear to have any protective effect on the subsequent pregnancy.

Gunshot fetal injury.

A stray bullet hit a pregnant woman. The uterus, being close to the abdominal wall, shielded the other viscera and received the bullet with fetal thoraco-abdominal fetal injury. The literature is reviewed.

Seroepidemiology of rubella in Kuwait: an alternative vaccination policy.

1054 females of child-bearing age in Kuwait were screened for rubella antibodies. 56 or 5.4% were found seronegative. In view of this relatively low rate of susceptibility, a large-scale rubella vaccination campaign in Kuwait may not be necessary. An alternative vaccination policy is suggested.
