[Is premedication in gastroscopy hazardous?].

Premedication for gastroscopies is still controversial. Most gastroenterologists use premedication routinely, often without control of vital signs. However, even if rarely, serious (mainly cardiorespiratory) complications still occur. In 101 patients in whom a gastroscopy had been performed, oxygen saturation before, during and after the endoscopy was investigated. A fall in saturation was seen in most of the patients but was usually minor. However, severe hypoxemia occurred in some patients, especially if premedication was used. Older patients and patients with severe anemia were particularly at risk. These results emphasize that premedication should not be used without precaution and special surveillance, especially in high risk patients. Prophylactic oxygen administration significantly diminishes the risk.

Age Factors

[Medical-psychological problems in dental practice].

Medical-psychological problems are of great importance in dental practice. The paper presents an overview on the ways of establishing the dentist-patient-relationship, on the perception of pain and anxiety, the influence of the dentist in this respect, the information process and psychological preparation for surgery. A thorough intervention-oriented consideration of the psychological factors involved will lead to a further improvement of quality and effectiveness in dentistry.

[Structural differentiation of psychological performances in epilepsy patients].

Investigating the specific of psychic performance disorders in epileptics structural aspects in addition to quantitative performance differences are considerable. We compared psychometrically obtained performance structures of epileptic children and adults and of parallelised healthy control persons. It became evident, that the performance differentiation hypothesis is to corroborate also in epileptic patients. The psychic performance structure of epileptics is characterized by a lower differentiation level of the basis functions underlining the performances. Our factor analytic investigations emphasize the importance of deceleration of psychic/psychomotoric speed in epileptics as primary basic disorder.

Adolescent

[Specific aspects of psychological disorders of performance in children with epilepsy].

We examined 49 normal intelligent epileptic and parallelised in sex, age and intelligence just as much healthy and 14 minimal cerebral damaged children without seizures. Using a psychological test battery psychic/psychomotor speed, flexibility/preservation proneness, memory span and memory and concentration were registrated. Besides reduced memory performances we found a deceleration of psychic/psychomotor speed as primary basic disorder within the performance structure of epileptic children analogous to our previous examined adults. The tridimensional comparison suggests remarkable psychopathological differences among the two groups of patients.

Adolescent

[Measuring reaction times in healthy probands before and following a single dose of psychotropic drugs].

A single dose of Clomipramin, 50 mg; Amitriptylin, 50 mg; Desipramin, 50 mg; Imipramin, 50 mg; Diazepam, 10 mg; Carbamazepin, 200 mg; Haloperidol, 3 mg; were each administered orally, and Clomipramin, 25 mg was administered intravenously, and reaction times were measured and compared with those obtained after administering a placebo. The statistical examination was conducted using Wilcoxon's test. In the case of Amitriptylin, Diazepam, and Haloperidol, there was a statistically significant reduction in the reaction times.

Humans

[Psychometric studies on the specificity of psychological performance disorders of epileptic patients].

Using a psychological test battery for registration of psychic/psychomotoric speed and flexibility/perseveration we examined 78 epileptic patients with different forms of epilepsy and the same number of brain healthy control persons, parallelised in sex, age, education, profession and intelligence. The registrated data were varianz-, correlation- and factoranalised. We found a deceleration of psychic/psychomotoric speed as a primary basic disorder in epileptics. Decreases of flexibility independent of speed were not ascertainable.

Adolescent

[Development of psychological EEG research].

A historical reminiscence is given followed by a description of development tendencies, problems and aims of psychological EEG research. It is necessary to pay attention on methodical and methodological problems of psychological EEG research. Methodical questions concerning the registration of psychic variables and the EEG analysis are discussed. The results of examinations carried out by the authors explain the practical importance of psychological EEG research.

Adolescent

[Relationship between automatically analyzed quantitative EEG data and psychological performance parameters in patients with epilepsy].

A report is given on results of comprehensive studies in which significant positive correlations between automatically analysed parameters reflecting the frequency behaviour of the EEG background activity and psychometrically recorded relatively complex psychic and psychomotoric tempo performances as well as some partial aspects of the re-adjustment capability in epileptics were found. EEG indicator values could be ascertained which point to psychic performance disturbances in epileptics.

Adolescent

[Disturbances in family relations and suicide attempts in childhood and adolescence (author's transl)].

Suicide attempts in childhood and adolescence include the general necessity of specific neuropsychiatric and psychological diagnostics and therapy. To classify in "serious" or "demonstrative-hysterical"suicide attempts is to refuse, because there exist personality disorders in every case. Essential sources are disturbances in the emotional parents-child-relations and chronic conflicts between the marriage partners, so that always an analysis of the family relations structure is indicated.
