[Malignant lymphoma of the penis: report of two cases].

We report two cases of malignant lymphoma of the penis. A 64-year-old man presented with painful indurations of the penis. A computerized tomography (CT) scan showed a swollen retroperitoneal lymph node 5 cm in diameter. Penectomy confirmed the diagnosis of B cell malignant lymphoma, diffuse large type. Despite systemic chemotherapy with a CHOP regimen, he died of disease 7 months postoperatively. The second case was in a 63-year-old man presenting with multiple nodules in the penis and scrotum. Biopsy of a scrotal nodule revealed B cell malignant lymphoma, diffuse medium size type. A CT scan demonstrated widespread lesions (stage III E). He has been in complete remission for 10 months following, multidisciplinary treatments. We reviewed 21 cases of malignant lymphoma of the penis which have been reported in the literature.

Surgical outcome of microscopic vasectomy reversal: an analysis of 30 cases.

The results of 30 consecutive microscopic vasovasostomy procedures performed at a single institution over a five-year period between 1991 and 1995 were reviewed. When the surgical outcomes of patients who were operated on less than five years after their vasectomy were compared with the outcomes of those patients who received a vasovasostomy more than five years after their vasectomy, decreases in technical success rates were observed as measured by appearance of sperm in ejaculate (56% vs. 36%), biologic recovery as measured by mean sperm counts (56 million vs. 35 million) and mean progressive sperm motility (44% vs. 21%), along with a decrease in clinical success, as measured by overall pregnancy rates (50% vs. 7%, p < 0.05). Therefore, a microscopic vasovasostomy within 5 years of a vasectomy is a favorable procedure for vasectomy reversal.

Three-dimensional model simulation in orthognathic surgery.

Surgery simulation with a three-dimensional model constructed with stereolithography from digital image data has proven to be useful in the maxillofacial area. The simulation and surgery of Le Fort I and bilateral sagittal split osteotomies were carried out in two patients. The models replicating the facial skeleton with precise internal anatomy facilitated design of the osteotomy and preparation of osteosynthesis. Each sectioned segment of the maxilla and mandible was accurately repositioned by transferring the positional relationships of multiple reference points on the model to the bone surface. Cephalograms taken at simulation and after surgery showed little difference in the positions of the landmarks.

Mandibular lengthening by distraction osteogenesis using osseointegrated implants and an intraoral device: a preliminary report.

PURPOSE: This study investigated an approach to distraction osteogenesis of the mandible using osseointegrated implants and an intraoral device. MATERIALS AND METHODS: Five adult dogs were used for this experiment. After the extraction of the left mandibular premolar and molar teeth, two osseointegrated implants were placed. Abutment connection, attachment of the intraoral distraction device, and an osteotomy in the region between the implants were performed 3 months after implantation. The distraction was done at rate of 1 mm/day for 10 consecutive days to elongate the mandible 10 mm. The animals were killed 2, 3, and 4 weeks after the distraction was completed, and radiographic and histologic examinations were done. RESULTS: The longer the time after completion of distraction, the more uniform the new bone that was observed radiographically and histologically in the gap created by the distraction. The titanium implants remained stable during the course of mandibular lengthening. CONCLUSION: An intraoral device using osseointegrated dental implants can serve as a mechanism for distraction osteogenesis in the maxillofacial skeleton.

A case of intraperitoneal testicular cancer with ascites.

We report an initial case of intraperitoneal testicular cancer with ascites. Although the cytology of aspirated ascites revealed no tumor cells, the beta-HCG and AFP values were high. Preoperative cisplatin-based combination chemotherapy was used without any pathological diagnosis. Following 2 cycles of chemotherapy, the beta-HCG and AFP values normalized. Furthermore, no ascites was observed on CT scan. Although an accurate histological diagnosis was not established before treatment, it was thought that the ascites originated from the disseminated testicular cancer cells in the peritoneum.

Chronic bacterial prostatitis treated with intraprostatic injection of antibiotics.

Direct transrectal infiltration of the prostate with antibiotics was performed in 25 selected patients with refractory chronic prostatitis, and sequential samples of urine and prostatic secretion were studied at 3-monthly intervals thereafter. The criterion for cure of chronic bacterial prostatitis was persistently sterile cultures of urine and prostatic secretion for at least 12 months after completion of treatment. Complete cure was obtained in 14 patients (56%) after one or two infiltrations. In seven other patients clinical and bacteriologic remission was obtained after several injections but the follow-up period was too short to judge the efficacy of the intraprostatic antibiotic administration. The remaining four patients (16%) had sterile specimen cultures 3 months after one injection, but were lost to further follow-up. The results suggest that local antibiotic treatment of chronic bacterial prostatitis is useful, although careful randomized studies with long follow-up are required to evaluate the merits of the method.

Effect of varicocelectomy on sperm parameters and pregnancy rate in patients with subclinical varicocele: a randomized prospective controlled study.

PURPOSE: We assessed whether subclinical varicocele ligation improves fertility and/or semen parameters. MATERIALS AND METHODS: A total of 85 patients with a subclinical varicocele diagnosed by scrotal thermography presented with infertility. Patients were randomly assigned to groups 1 (high ligation of the internal spermatic vein) and 2 (followed without any treatment). At least 3 semen samples were obtained at study entry and 1 year later in both groups. RESULTS: The pregnancy rate in group 1 was 6.7% compared to 10% in group 2, and the difference was not statistically significant. Group 1 had significantly higher levels of sperm density and total motile sperm count at 1 year. There were no significant differences between groups 1 and 2 regarding change in seminal volume, sperm motility and abnormal sperm morphology. CONCLUSIONS: Subclinical varicocelectomy has some effect on spermatogenesis but no beneficial effect on pregnancy rate.

A comparison of transurethral resection of the prostate and medical treatment for the patient with moderate symptoms of benign prostatic hyperplasia.

Transurethral resection of the prostate is the most common surgical treatment for benign prostatic hyperplasia. We conducted a prospective randomized clinical trial to compare this surgery with medical treatment in men with moderate symptoms of benign prostatic hyperplasia. Of 98 men over the age of 54 years who were screened between June 1993 and June 1995, 53 were studied (25 in the surgery group and 28 in the medication group). Patients' symptoms and the degree to which they were bothered by urinary difficulties were measured with standardized questionnaires and medical evaluations. The men randomly assigned to the surgery group underwent surgery within 2 weeks after the assignment. Surgery was not associated with an impotence or urinary incontinence. The follow-up period was 1 year. Surgery was significantly associated with improvement in residual urinary volume and peak flow rate; and also in the scores for urinary difficulties, sexual performance and interference with activities of daily living (P < 0.001 for all comparisons). We concluded that for patients with moderate symptoms of benign prostatic hyperplasia, surgery is more effective than medication in improving genitourinary symptoms and enhancing the quality of life. Thus, medication as treatment should be reserved for patients who are less bothered by urinary difficulty or who wish to delay surgery.

The incidence of antisperm antibodies in patients with seminal tract obstructions.

Antisperm antibodies are one of the important factors in male infertility caused by vasal obstructions. To investigate the incidence of serum antisperm antibodies in patients with various kinds of seminal tract obstructions, we retrospectively analyzed the sera of 60 patients using the indirect immunobead test (IBT) to find serum antisperm antibodies. Immunoglobulin (Ig)G and IgA class antisperm antibodies were positive in 55% and 18% of those patients with a vasal obstruction caused by inguinal herniorrhaphy and in 60% and 20% of vasectomized patients respectively; whereas these antibodies were positive in 13% and 0% of those patients with an epididymal obstruction of unknown etiology and in 8% and 3% of those patients with congenital bilateral absence of the vas deferens (CBAVD). The incidence of antisperm antibodies was significantly higher in patients with a vasal obstruction than in those with an epididymal obstruction (p < 0.001).

Does epididymal length in men with congenital bilateral absence of the vas deferens have a correlation with the fertilization rate of epididymal sperm retrieved by micropuncture technique?

To investigate whether the variable length of the epididymis in men with congenital bilateral absence of the vas deferens (CBAVD) might have a correlation with fertilization and pregnancy rates in in vitro fertilization (IVF) treatment, we conducted a retrospective study involving a total of 60 CBAVD patients. All patients in this study had epididymal micropuncture in conjunction with perivascular nerve stimulation as part of the IVF program at the Nagoya University Hospital Reproduction Center. The patients were classified into 3 groups: group I consisted of patients having only a proximal portion of the caput epididymidis with a length of between 0.5 and 1.9 cm, group II consisted of patients with the caput and a portion of the corpus epididymidis with a length of between 2.0 and 4.0 cm, and group III consisted of patients with the caput, corpus and cauda of the epididymis measuring more than 4.0 cm. There were no differences in the sperm count, progression and normal morphology among the 3 groups; however, the motility was progressively higher in patients with a longer epididymis (12% in group 1, 18% in group II and 31% in group III). It was evident that group III showed the highest fertilization and pregnancy rate per patient (23% and 28%, respectively) among the 3 groups. This study shows that epididymal sperm from CBAVD patients with a longer epididymis have a higher IVF rate than CBAVD patients with a shorter epididymis.

Effect of alpha-blockers on epididymal sperm concentration, motility and testicular productivity in the rat.

The present study was undertaken to determine whether alpha-blockers affect epididymal sperm parameters and testicular spermatogenesis. Sprague-Dawley rats were given orally a 1 mg/kg body weight dose of Bunazosin, a selective alpha-adrenoreceptor antagonist, daily for one month as a short-term administration group and three months as a long-term administration group. Using a micropuncture technique, epididymal sperm concentration and sperm motility were measured. Epididymal sperm concentrations were significantly increased in the Bunazosin group, but progressive motility of spermatozoa were not altered. Daily testicular sperm production was also increased after administration of Bunazosin. There were no differences in epididymal sperm concentration and daily sperm production between the one-month group and three-month group. An epididymal wide storage capacity and short transit time after administration of the alpha-blocker could increase the sperm concentration in the cauda epididymidis. Because there were no differences in the epididymal sperm parameters and daily testicular sperm production between the two groups, these changes may occur at 1 month after administration of the alpha-blocker. Moreover, the alpha-blocker might affect testicular function.

[A case report of primary renal carcinoid tumor].

A 54-year-old woman was referred to our institute because of abdominal pain and constipation. The computed tomography of the abdomen revealed a 3 x 3 cm of low density tumor at the upper pole of the left kidney and para-aortic lymphadenopathy. Gastrointestinal investigation revealed no particular findings suggesting a carcinoid syndrome associated with the left kidney tumor. The specimen of the resected tumor showed staining pattern specific to carcinoid tumor; positive staining for chromogranin A and neuron-specific-enolase. Electron microscopy confirmed the presence of dense-core granules in the tumor cells. No evidence of metastasis or recurrence was noted for 13 months after the operation. This was a very rare case of primary renal carcinoid, representing the 26th case in the literature.

Two cases of intrascrotal liposarcoma.

Liposarcomas in the scrotum are relatively rare and only 40 cases have previously been reported in Japan. The inside wall of the scrotum is an unusual origin and we report here two cases arising at this site that have been followed for 21 and 40 months, respectively, after surgical resection. Case 1: the patient was a 24-year-old male who noticed a left intrascrotal mass which was histologically demonstrated to be a myxoid type liposarcoma. Case 2: the patient was a 66-year-old male who had the complaint of a left intrascrotal swelling. He underwent radical orchiectomy and histologic examination of the resected tumor revealed a well-differentiated type of liposarcoma. These two cases are the 41st and 42nd intrascrotal liposarcomas reported in Japan.

Comparison of the effectiveness of placebo and alpha-blocker therapy for the treatment of idiopathic oligozoospermia.

OBJECTIVE: To determine whether alpha-blocker (bunazosin) improves fertility and/or semen parameters DESIGN: Placebo-controlled, double-blind clinical study. SETTING: Nagoya University Hospital Andrology Clinic, Nagoya, Japan. PATIENTS: Thirty-four men with sperm density between 5 and 20 x 10(6) sperm/mL, normal serum gonadotropins and T, and a fertile partner were enrolled in this study. INTERVENTIONS: After a 3-month control period, patients randomly were prescribed bunazosin 2 mg/d or a placebo, two tablets per day, for 6 months. Semen and blood samples were collected before and after therapy. Semen parameters, serum gonadotropins, T, PRL, and E2 were evaluated before and after therapy. RESULTS: The pregnancy rate (PR) in the alpha-blocker group was 25%, compared with 6.7% in the placebo group. There was no statistical difference in the PR between groups. The alpha-blocker group had significantly higher levels of sperm density and total motile sperm count. There were no differences between the placebo and alpha-blocker groups in seminal volume, the percentage of motile sperm, and normal morphology or hormone levels. CONCLUSIONS: The authors conclude that alpha-blocker is a useful drug in the treatment of idiopathic moderate oligozoospermic men.

New treatment of idiopathic severe oligozoospermia with mast cell blocker: results of a single-blind study.

OBJECTIVE: To determine whether mast cell blocker (tranilast) improves fertility and/or semen parameters in severe oligozoospermia. DESIGN: Placebo-controlled single-blind clinical study. SETTING: Nagoya University Hospital Andrology Clinic, Nagoya, Japan. PATIENTS: Fifty men with sperm density < 5 x 10(6) sperm/mL, normal serum gonadotropins, and T, and a fertile partner were enrolled in this study. INTERVENTIONS: Patients were prescribed randomly 300 mg/d tranilast or a placebo, three tablets per day, for 3 months. Semen and blood samples were collected before and after therapy. Semen parameters, serum gonadotropins, T, and PRL were evaluated before and after therapy. RESULTS: The pregnancy rate (PR) in the mast cell blocker group was 28.6% compared with 0% in the placebo group. There was a statistical difference in the PR between groups. The mast cell blocker group had significantly higher levels of sperm density, sperm motility, and total motile sperm count. There were no differences between the mast cell blocker and placebo groups in seminal volume and normal sperm morphology. CONCLUSIONS: The authors conclude that mast cell blocker is clinically useful for the treatment of idiopathic severe oligozoospermic men.

Effects of alpha-blocker on sperm concentration, motility, intraluminal pressure and fluid movement in the rat cauda epididymis.

PURPOSE: To investigate whether alpha blocker has an effect on epididymal sperm parameters, intraluminal pressure and fluid movement. MATERIALS AND METHODS: Sprague-Dawley rats were given daily oral doses 1 mg./kg., 2 mg./kg., or 10 mg./kg. of Bunazosin, a selective alpha blocker, for 1 month. A micropuncture technique was used to collect intraluminal fluids. RESULTS: Epididymal sperm concentrations were significantly increased. Progressive motility of spermatozoa was not altered. Intraluminal pressures were significantly reduced, and intraluminal fluid movement rates were significantly increased. CONCLUSION: Alpha blocker has an effect on sperm transport and storage in the rat testis and epididymis.

Bulk sperm collection by epididymal micropuncture and stimulation of the spermatic nerve; a novel method for sperm retrieval for IVF for surgically irreparable vasal obstruction.

We have developed a new sperm collection method by epididymal micropuncture in conjunction with perivascular nerve stimulation to obtain as many uncontaminated spermatozoa as possible for IVF for patients with surgically unreconstructable vasal obstruction. Fifteen azoospermic men with congenital absence of the vas deferens or vasal obstruction not amenable to surgical repair enrolled in this study. Under general anaesthesia the testis and epididymis were retrieved and stabilized in a testicular holder with the aid of 2% agar. The epididymal micropuncture was performed using a micropipette placed in a micromanipulator. A pair of Ag-AgCl ring electrodes were placed around the spermatic cord for perivascular nerve stimulation with DC (intensity 136 V, frequency 20 Hz) for 30 s. Electrical stimulation was repeated every 1 min until no further fluid was obtained. Epididymal fluid obtained ranged from 78 to 980 microliters (mean 169 microliters). Sperm concentration ranged from 0.7 to 200 million (mean 53.5 million). Sperm motility ranged from 0.5% to 70% (mean: 19.9%). The percentage of spermatozoa with normal morphology ranged from 3% to 32% (mean 16.4%). Of 16 cycles in which spermatozoa and oocytes were retrieved, 56% (9/16) resulted in fertilization and 18.8% (3/16) developed clinical pregnancies. The clinical pregnancy rate per couple was 20% (3/15). This is the first clinical experience of the bulk sperm collection system for IVF for the treatment of unreconstructable obstructive azoospermia. Our results indicate that our new technique, in conjunction with IVF, is clinically useful for patients with surgically irreparable vasal obstruction.
