Role of prostate-specific antigen and digital rectal examination in the detection of prostate cancer.

Prostate-specific antigen (PSA) is a kallikrein-like serine protease that is secreted exclusively by the epithelial cells of all types of prostatic tissue, benign and malignant. Its serum concentration is raised in men with prostatic disease including cancer. We have evaluated its usefulness in the diagnosis of prostate cancer by measuring serum PSA concentrations in 260 men aged 50 years or over. All had abnormalities at digital rectal examination (DRE) involving suspected cancer, signs and symptoms of benign prostatic hyperplasia and equivocal findings on DRE, and miscellaneous other conditions, including hematospermia, chronic prostatitis and microscopic hematuria. Transrectal prostatic needle biopsies were performed in the men with abnormal findings on DRE or elevated serum PSA (above 4 ng/ml). Serum PSA ranged from 4.0 to 9.9 ng/ml in 14 (5%) of the 260 men. Four of the men in this group (31%) who underwent prostatic biopsy had prostate cancer. Serum PSA levels greater than or equal to 10.0 ng/ml were found in 8 (3%) of the 260 men. 5 of these 8 (63%) who underwent prostatic biopsy had cancer. If DRE alone had been used to screen the men having biopsies, 4 of the 10 cancers (40%) would have been missed. If PSA alone had been used to screen these men, only 1 of the 10 cancers would have been missed. Serum PSA measurement was more reliable than DRE for detecting prostate cancer. Since these two methods do not always detect the same malignant tumor, the combined use of DRE and PSA testing affords a more complete evaluation of the prostate gland for malignant involvement.

Appearance of spermatozoon after administration of mast cell blocker to a patient with azoospermia.

Since a close relationship has been suggested to exist between testicular disfunction and the increased mast cells in the testis, we used a mast cell blocker for the treatment of patients with idiopathic infestility. An infertile male with idiopathic azoospermia was treated with administration of a mast cell blocker, tranilast for one year. The patient was found to have sperm within his ejaculate. However, the ultimate goal of pregnancy was not achieved by the microfertilization technique. To evaluate the possible significance of this new treatment, further basic research will be needed to clarify the relationship between mast cell proliferation and impaired testicular function.

Successful treatment of hemorrhagic cystitis secondary to cyclophosphamide chemotherapy with intravesical instillation of prostaglandin F2 alpha.

The treatment of cyclophosphamide-induced hemorrhagic cystitis is difficult. We report a successful case of severe cyclophosphamide-induced hemorrhagic cystitis treated with intravesical instillation of prostaglandin F2 alpha. A 32-year-old woman underwent high-dose cyclophosphamide conditioning before the autologous bone marrow transplantation. She developed clot retention which required continuous irrigation with normal saline. The patient had failed to respond to continuous bladder irrigation with saline and intravesical administration of 1% alum. Fifty ml of prostaglandin F2 alpha solution (1 mg in 100 ml normal saline) was instilled into the bladder, with a dwelling time of 60 minutes, three times a day for 5 days. The hematuria cleared completely 3 days after therapy. The only adverse effect was bladder spasm which was controlled with oxybutynin chloride. The success of this therapy suggests that prostaglandin F2 alpha is a safe and useful therapy for hemorrhagic cystitis secondary to cyclophosphamide chemotherapy.

Intraluminal content is required for the maintenance of antigrade proluminal movement of 3H-androgens into rat caput epididymal tubules.

The present study was undertaken to determine whether or not antigrade, proluminal 3H-androgen movement in the caput epididymis occurs in the absence of native lumen content. A single tubule was perfused with artificial caput fluid containing no androgen-binding protein for 30 min and subsequently tubules were perifused with Minimum Essential Medium perifusion fluid containing 26.7 microCi/ml 3H-testosterone and 1.3 microCi/ml 14C-polyethyleneglycol for 1 h. Radioactivity of isotopes in perifusion and intraluminal fluids was determined at 1 h after sustaining perifusion, and the percentage of peritubular isotopes appearing in the intraluminal fluid was determined. Net entry of 3H-androgen into the epididymal tubules in the presence of native intraluminal content was approximately 323%. In contrast, intraluminal 3H-androgen concentrations in the epididymal fluid in the absence of native lumen content were significantly reduced, to 100% of those in the peritubular fluid. Antigrade, proluminal movement of 3H-androgen in the caput epididymis does not occur in the absence of native lumen content. Androgen-binding protein in the epididymal lumen may be required to maintain uphill proluminal movement of 3H-androgen into the tubules.

Effects of protein synthesis inhibitor and antimicrotubular agent on transepithelial movement of 3H-androgens in the rat caput epididymis.

The effects of protein synthesis inhibition and disassembly of microtubules in the epididymal epithelia on proluminal movement of 3H-androgens were investigated by using in vivo microperifusion of 3H-testosterone and subsequent micropuncture to obtain peritubular and intraluminal fluids of caput epididymal tubules. Cycloheximide (100 micrograms/ml) was used as protein synthesis inhibitor. Nocodazole (3 micrograms/ml) was used to depolymerize microtubules in the cell. The perifusion fluid was Minimum Essential Medium containing 26.7 microCi/ml 3H-testosterone and 1.3 microCi/ml 14C-polyethyleneglycol (14C-PEG), or the same fluid supplemented with cycloheximide or nocodazole. Radioactivity of 3H-androgen and 14C-PEG in perifusion and intraluminal fluids was determined at one hour after initiation of the sustaining perifusion, and the percentage of radioactivity of 3H-androgen and 14C-PEG appearing in the intraluminal fluid to that in the peritubular fluid was determined. Proluminal movement of 3H-androgens into the caput epididymal tubules in the control rats was 323.4 +/- 73.2%. This value was significantly reduced to 121.8 +/- 13% by addition of cycloheximide to the perifusion fluid (p < 0.01). Transepithelial movement of 3H-androgen in the caput epididymis was significantly decreased to 86.6 +/- 5.3% by exposure of the epididymal tubules to nocodazole (p < 0.01). Inhibition of protein synthesis and disassembly of microtubules in the epididymal epithelial cells completely eliminated antigrade proluminal movement of 3H-androgen into the tubules. Study of the incorporation of 35S-Methionine into epididymal tissue protein revealed significant reduction of the quantity of radiolabeled proteins in the perifused tissue with fluid containing cycloheximide (p < 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of albumin on proluminal movement of 3H-androgen into seminiferous and epididymal tubules and androgen binding in the interstitium of the testis and epididymis after perifusion with fluid containing albumin.

Effect of albumin on proluminal androgen movement from the peritubular space to the intratubular fluids of the adult rat testis and epididymis was examined by using in vivo microperifusion and subsequent micro-puncture of the seminiferous tubules and caput epididymal tubules. Tubules were perfused with four different fluids: (1) Minimum Essential Medium (MEM) containing 3H-testosterone and 14C-polyethyleneglycol (PEG) alone; (2) MEM + 8 mg/ml Bovine Serum Albumin (BSA) containing the same radiolabeled compounds as above; (3) MEM + 80 mg/ml albumin containing the same radiolabeled compounds as above; and (4) Testosterone-free rat serum containing the same radiolabeled compounds as above. Bound 3H-androgens in the interstitial fluids of the testis and epididymis after one-hour perifusion with the four different fluids above were measured by charcoal assay. In the testis, proluminal 3H-androgen movement was not significantly altered by addition of albumin to the perifusion fluid (p = 0.08). Bound 3H-androgens in the interstitial fluid after perifusion were significantly increased with increasing albumin concentrations in the perifusion fluid. In the caput epididymis, proluminal 3H-androgen movement was significantly decreased with increasing albumin concentration in the perifusion fluid. Bound 3H-androgens in the interstitial fluid after perifusion were significantly increased with increasing albumin concentrations in the perifusion fluid (p < 0.05). These findings suggest that proluminal transepithelial movement of 3H-androgens in the reproductive tract could be influenced by the presence of albumin, androgen-binding protein or some other binding protein in the peritubular space.

The effect of varicocele ligation on oocyte fertilization and pregnancy after failure of fertilization in in vitro fertilization-embryo transfer.

Although varicocelectomy is generally believed to improve semen quality and the hamster egg penetration assay, these parameters do not correlate well with in vitro fertilization (IVF) success. The effect of varicocelectomy on IVF of human oocytes was examined on thirteen couples with normal female fertility, but severe oligozoospermia and the presence varicocele in the males with failure of fertilization preoperatively in in vitro fertilization-embryo of transfer (IVF-ET) attempts. The couples were readmitted for the IVF-ET procedure following varicocelectomy. A 31% pregnancy rate was achieved after the operation, while no pregnancies occurred before surgery. Sperm density and motility improved significantly after operation, resulting in a coinciding improved fertilization and pregnancy rates.

Prostate-specific antigen levels in acute and chronic bacterial prostatitis.

Prostate-specific antigen (PSA) is now widely accepted as a useful tumor marker for the diagnosis and follow-up of prostatic cancer. An elevated level of PSA has been asserted to be highly specific for prostate cancer, although patients with large benign prostate glands and those with bacterial prostatitis may also have slightly elevated levels. We measured the serum PSA level in the patients with acute and chronic bacterial prostatitis and consecutively monitored the PSA level in 6 patients who had acute prostatitis and an elevated PSA level. The PSA level was found to be elevated during the acute phase of prostatic inflammation, and the elevated, PSA level in the patients with acute prostatitis returned to the normal level within 14 days after initiation of antimicrobial therapy in all 6 patients. In one patient with chronic prostatitis the elevated PSA level persisted after antibiotic treatment. He was found to have adenocarcinoma by transrectal ultrasonography and biopsy. A markedly elevated serum PSA level in bacterial prostatitis can cause confusion in the diagnosis of prostatic carcinoma. Therefore, PSA determination should be obtained after complete clinical resolution of inflammation to exclude prostatic malignant involvement.

[Chlamydia trachomatis infection in young men with acute epididymitis and their sexual partners].

Untreated infection of female partners by men with chlamydial epididymitis may have serious effects on the partners' fertility. To assess the need for detailed microbiological investigation, 32 patients, 35 years old or younger, with epididymitis and their sexual partners were examined. The patients underwent thorough evaluations, including chlamydia isolation, microscopy of urethral swab, bacterial culture, and chlamydia serologic testing. An infective cause was identified in 56% of the patients. The most common microorganism was Chlamydia trachomatis. This microorganism was identified from urethral swabs in 11 patients (34%). A total of 18 sexual partners were traced and investigated for chlamydia antigen by cervical swab, urinary bacterial culture, and chlamydia serologic testing. Of the 18 female sexual partners screened, 9 were partners of patients with chlamydial epididymitis; 78% of these women had the same infection. Young men with epididymitis, as well as their partners, should undergo full microbiological evaluation including Chlamydia trachomatis for adequate treatment of this infection.

[Two cases of continent cystoileostomy using nipple valve method].

We performed continent cystoileostomy in which the technique of Kock's efferent nipple was utilized, on two female patients with urethral cancer and vulvar cancer. We recommend continent cystoileostomy for the patient who needs urethrectomy but whose bladder function can be preserved.

Etiology of asymptomatic microscopic hematuria in adults.

Asymptomatic microscopic hematuria is a common finding that demands urologic evaluation. Of the 236 patients over the age of 40, 6.8% were found to have a genitourinary cancer, while 27.5% had other significant urologic disease. However, 52.1% of the patients had unknown etiology of microscopic hematuria and 13.6% had insignificant urologic lesions. Of the 72 patients under the age of 40 a positive diagnosis was made in 16 patients (22.2%). Cystoscopic examination was of diagnostic value in only 1 patient. Therefore, cystoscopy is of little diagnostic value in young patients. Once asymptomatic microscopic hematuria is established and no etiological cause is identified, we follow the patient by urinalysis and cytology every three months and reevaluate the patient in whom urological symptoms develop.

A case of spontaneous recanalization following renal infarction.

A 30-year-old-male was admitted to our hospital with a tentative diagnosis of appendicitis. We found no signs of peritonitis, and therefore suspected urinary tract calculi. Intravenous pyelography revealed his right kidney to be nonfunctional, while retrograde pyelography was normal. Angiography revealed right renal infarction. Renovascular hypertension was present, and treatment with captopril was prescribed. Two years later without medication he had normal blood pressure and plasma renin activity. Intravenous pyelography revealed a right kidney functioning well, and digital subtraction angiography (DSA) revealed spontaneous recanalization of his right renal artery.

[Clinical and bacteriological study of sparfloxacin on bacterial prostatitis].

Sparfloxacin (SPFX), a new oral quinolone antimicrobial, was studied for the bacterial response, pharmacokinetics and clinical efficacy in the treatment of bacterial prostatitis. The minimum inhibitory concentration (MIC) values for 48 strains isolated from expressed prostatic secretion were measured. The values for 8 out of 12 strains of S. epidermidis were below 0.05 microgram/ml and those for all 6 strains of E. coli were below 0.025 microgram/ml. The SPFX concentrations in prostatic fluid (PF) were 0.33 to 0.49 microgram/ml at 1 to 3 hours after oral administration of 200 mg, the PF/serum ratio being 1.15 to 1.47. SPFX was administered at a dose of 200 to 400 mg daily for an average of 14.1 days to 14 patients with prostatitis (5, acute: 9, chronic). The clinical efficacy judged by physician in charge was effective in 12 cases with an efficacy rate of 85.7%. The bacterial eradication rate was 93.3% (14/15 strains), and eradication was complete in all 7 cases infected with gram-negative rods. SPFX-related abnormal laboratory values were observed in one case with transient increase of glutamic-oxalacetic transaminase, glutamic-pyruvic transaminase and alkaliphosphatase activities, and decrease of platelet. As side effects, one case with gastrointestinal symptoms and the other case with photosensitivity skin rash accompanied by sensory abnormality of palms were observed. The abnormal values or side effects in these patients recovered to normal or disappeared after completion of the treatment without any treatments. In view of the higher concentrations in PF than the MIC values with long remaining in the tissues, SPFX is considered to be effective in the treatment of bacterial prostatitis.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical statistics of the patients admitted to the Department of Urology Shizuoka Red Cross Hospital between 1981 and 1990].

A statistic survey was made on the patients, diseases and operations experienced at the Urological ward of Shizuoka Red Cross Hospital between 1981 and 1991. The total number of inpatients was 2,830 and the male to female ratio was 3.5 to 1. The most frequent diseases among the inpatients were obstructive uropathy (25.5%), malignant neoplasia (23.7%), non-specific infection (16.4%) urolithiasis (16.1%). The number of operations was 1922. Endoscopic surgery was the most frequent form of operation (55.2%).

[Clinical study of cefclidin on bacterial prostatitis].

Cefclidin (CFCL), a new injectable cephem antibiotic, was used in the treatment of bacterial prostatitis. Concentration in prostatic fluid (PF): One hour after the i.v. administration of 1 g of CFCL the concentration of PF was 0.84 +/- 0.72 micrograms/ml (n = 4), the ratio of PF/serum being 2.24%. CFCL was administered at a dose of 1-2 g a day for 6.5 days on average. In acute bacterial prostatitis, the efficacy was evaluated as excellent or moderate in all 7 cases (100%), but in chronic cases, the effectiveness rate was as low as one out of 4 cases (25%). In safety profile, transient skin eruption was seen in one patient, but did not require further treatment. Besides this case, no side reactions or abnormal clinical values were encountered. In conclusion, CFCL was evaluated to be very useful in the treatment of acute bacterial prostatitis, caused by gram negative rods.

[Inclination and area of occlusal facets in natural dentition].

The characteristics of occlusal facets are important criteria used in the functional assessment of the stomatognathic system. A new system of optically quantifying the facets using a graphic image-processor has been developed, which makes it possible to measure the area and the inclination angles of each facet in the frontal and sagittal sections. In this study, the occlusal facets were investigated in 5 cases with normal dentition concerning the inclinations, areas, locations and occlusal contacts. The facets which made contact with their opposing ones in the intercuspal and lateral occlusal positions were selectively analyzed. The frontal inclination angles of the facets increased or decreased according to their mesio-distal locations in the working-facet or balancing-facet series, respectively. The sagittal ones indicated that most of the facets on the lateral excursion belonged to either the mesial or distal slopes of the cusps in each case. The angles made by the upper and lower facets which made contact were greater in the lateral occlusal positions than in the intercuspal position. Most of the areas of the upper facets were larger than those of the lower ones, which fact was remarkable on the facets related to the occlusal contacts between the intercuspal position and the cusp-to-cusp positions of the canines.

[Newly devised subcutaneous needle electrodes for EMG recording].

Subcutaneous needle electrodes made of stainless steel were newly devised for ease of handling and perfect insulation. This needle (phi 0.20 mm) is used for acupuncture. The electrodes had the capability to record the EMG activity easily from a certain muscle. Then, the EMG activities of the masseter muscle were recorded with both these needle electrodes and surface electrodes and the results were compared. 1. Insertion of the electrodes into the subcutaneous tissue was easily performed because of the application of the acupuncture needle and their lightness of 0.2g was effective in fixing the electrodes on the skin without causing any tension. 2. After the insertion of the needle electrodes, the impedance showed below 5 kohm immediately, and the EMG recordings during biting could be simply achieved with great stability. 3. The action potential from the needle electrodes was less than that from the surface ones. The former reacted more sensitively to the change in the distance between the electrodes, compared with the latter.

[A case of metastasis of lung cancer to renal cell carcinoma].

A 62-year-old male was admitted with abnormal shadow in chest X-P. CT and other examinations were done, and he was diagnosed left renal cell carcinoma with metastatic lung cancer. He rejected operation and was discharged. We gave him alpha-interferon injection every day. About 5 months later, he complained of fever and dyspnea, and was admitted. On the 10th day after admission, he died suddenly with massive hemoptysis. This hemoptysis was from the pulmonary artery, which was surrounded by tumors and ruptured into the trachea. Pathological diagnosis was double cancer, such a case is very rare with a primary lung cancer (oat cell carcinoma) which has metastasized into a renal cell carcinoma (common type, clear cell subtype).
