Percutaneous embolization of ruptured splanchnic artery pseudoaneurysms.

The usefulness of emergent embolotherapy was evaluated in 17 patients with life-threatening hemorrhage from 18 ruptured splanchnic artery pseudoaneurysms. Complete hemostasis was obtained in 16 out of 17 patients by embolotherapy. Of the 12 initial embolization procedures with permanent embolic materials including stainless steel coils, microcoils, and Ivalon, complete cessation of bleeding was obtained in 11. On the other hand, 6 out of 9 initial embolization procedures with Gelfoam particles failed to halt bleeding, and additional embolization with permanent embolic materials was required. Emergent embolization with permanent embolic materials using superselective catheterization should be considered the initial treatment of choice for ruptured splanchnic artery pseudoaneurysms.

[Successful emergency superselective embolization for hemobilia with a new coaxial catheter and steerable guidewire].

Hemobilia occurred in a 70 year old female from the left hepatic artery pseudoaneurysm induced by PTBD. A Tracker-18 catheter introduced into the left lateral posterior hepatic artery superselectively and embolization was performed with both sponge gel and Ivalon. This new coaxial catheter system allowed selective occlusion of the hepatic artery pseudoaneurysms with minimal loss of liver function.

[Transcatheter arterial embolization in the management of duodenal bleeding].

Emergency transcatheter arterial embolization was performed in 6 patients with massive duodenal bleeding. Three out of 6 patients were bleeding from duodenal ulcer and erosion, 2 from aneurysm of the gastroduodenal artery (GDA) and 1 from duodenal tumor. In 4 cases, complete and permanent cessation of bleeding was obtained by GDA embolization alone and additional pancreatico-duodenal artery embolization were necessary for hemostasis in 2 patients with GDA pseudoaneurysm. One patient maintained by hemodialysis had complication of duodenal infarction and pancreas necrosis following this procedure. Emergency embolization represents a useful alternative to the operative management of massive duodenal bleeding and should be the initial treatment of choice in those patients.

[Cystic meningioma associated with intratumor and subarachnoid hemorrhage during embolization--a case report].

We present a case of a cystic meningioma accompanied with hemorrhage in a cyst and adjacent subarachnoid space that occurred while preoperative embolization in feeders for the tumor was being applied. A 73-year-old male patient was admitted for a complaint of convulsion. Under CT examination, a tumor was observed at the left frontal convexity and found to be fed by the middle cerebral artery shown in the left cerebral angiograms. The tumor was diagnosed meningioma. After removing the tumor, we conducted histological study. Gross findings of its cross section had an atypical bleeding pattern which was resulted from the ruptures of the criss-crossing internal and external carotid arteries in the tumor. The findings indicated that the tumor was an angiomatous meningioma and contained meningotheliomatous components. The hemorrhage was occurred in the portion of the angiomatous meningioma. The possible reason for the hemorrhage was thought that sudden dynamic changes in blood flow which were triggered by the embolization. The sudden changes could have caused multiple ruptures on pathologic small vessels. Therefore, we strongly recommend that preoperative embolization should be proceeded with caution for exercising a cyst meningioma, since it could increase the possibility of bleeding from pathologic weak vessels.
