[Naftidrofuryl in arterial occlusive disease. Controlled multicenter double-blind study with oral administration].

The efficacy of naftidrofuryl ( Dusodril ) for treatment of stage II arterial occlusive disease was evaluated in a controlled multi-centre study in a total of 104 out-patients with angiographically documented localization of occlusion. The therapeutic effect was assessed over three months by measurements of walking distance using standardized treadmill conditions. Further parameters were venoocclusive plethysmography and Doppler ultrasonography measurement of pressures. The complaint-free walking distance increased significantly during daily application of 600 mg naftidrofuryl orally (n = 54) during the 12-week assessment period when compared to the placebo group (n = 50). Taking the intraindividual variability of 17.2 m in assessment of walking distance into account, the increase of painless walking of 93 m after treatment for 12 weeks in the active-drug group is considered the result of treatment-induced increased performance.

Noninvasive beat-by-beat registration of ventricular late potentials using high resolution electrocardiography.

We have developed a new high resolution ECG equipment for recording cardiac microvolt potentials from the body surface. Noise reduction has been achieved by specially designed suction electrodes, by spatial averaging of the electrocardiograms from four electrode pairs, using extremely low noise amplifiers, by performing registrations within a Faraday cage, and by teaching the patient to relax during end expiratory breath holding. Fourteen young males (controls) and 30 patients with various cardiac diseases (27 with CHD) were studied. In normals ventricular late potentials were not seen, but in 12/30 patients clearcut diastolic potentials were found. In 7/12 patients with positive findings, late potentials appeared beat-by-beat, in 5/12 patients those signals occurred intermittently, in 11/30 patients questionably, and in the remaining 5/30 patients no late potentials were recorded. One patient with the Romano-Ward syndrome revealed phases with stable beat-by-beat and intermittently occurring ventricular late potentials. These results demonstrate the feasibility of continuous non-invasive recording of ventricular late potentials, whose clinical and prognostic significance remains to be established.

Naftidrofuryl in chronic arterial disease. Results of a controlled multicenter study.

In a double blind, randomized multicenter study naftidrofuryl, a vasoactive substance, was compared with placebo in the treatment of 104 patients with chronic arterial occlusive disease. After a run-in period of four weeks the patients received either naftidrofuryl (600 mg daily) or placebo over 12 weeks. The pain-free and the total walking distances improved significantly in both groups. However, the difference in the improvement of the pain-free walking distance was significant (p less than 0.02) in favour of naftidrofuryl. There also was a difference in the improvement of the total walking distance in favour of naftidrofuryl which was not significant. The results indicate that naftidrofuryl has a beneficial effect on the symptoms and lengthens the painfree walking distance in patients with arterial occlusive disease.

[Perioperative plasma antithrombin activity with "low-dose" heparin prophylaxis. Perioperatively acquired antithrombin III deficiency as a cause for the failure of heparin prophylaxis?].

Antithrombin III activity was estimated using a chromogenic substrate perioperatively in the plasma of 200 patients undergoing major elective abdominal surgery during low-dose heparin prophylaxis. With an initial value of 10.9 +/- 2 IU/ml there was a mean postoperative lowering of plasma antithrombin activity in all patients with a minimum on the second postoperative day. In patients with empirically established increased risk of thromboembolism the postoperative depression of antithrombin was significantly more pronounced (P less than 0.01) than in the control groups. An increased risk prevailed particularly in patients with malignant diseases and in major surgery, i.e. of prolonged duration. In such cases with increased risk of thromboembolism routine assessment of antithrombin III is recommended.

[Pseudofollicles in chronic lymphatic leukemia].

In chronic lymphatic leukemia lymph nodes show very often nodular aggregates of cells that are somewhat larger than small lymphocytes and that are less hyperchromatic and less closely packed. These aggregates appear histologically as light areas that are called pseudofollicles, pseudonodules or proliferation centers. However, it has not been substantiated as yet that a more active proliferation of cells indeed takes place within these areas. Therefore, in 10 cases of chronic lymphatic leukemia the DNA-content, the size of the nuclei, and the frequency of mitoses within and outside the pseudofollicles were assessed. It could be shown that the cells of the pseudofollicles are indeed more actively proliferating populations than the cells outside the pseudofollicles. This proves that pseudofollicles are proliferation centers.

[Continuous registration of micropotentials of the human heart. Initial experiences with a new high resolution ECG amplifier system].

A new ECG-amplifier system for recording cardiac microvolt potentials from the body surface is described. The improvement in signal-to-noise ratio was achieved by using specially designed suction electrodes, which were isolated from each other; by applying parallel signal averaging from four electrode pairs via four low-noise amplifiers; and by conducting the registration in Faraday cage. in 14 normal subjects, 12 patients with coronary heart disease and one patient with surgically corrected ventricular septal defect and pulmonary stenosis, pre-P-potentials (possible sinus node activity), His bundle potentials and ventricular late potentials were recorded with differing degrees of success. Variations of the time intervals to the preceding QRS complex were observed within the S-T segment in six of nine patients with demonstrable ventricular late ventricular late potentials. The advantage of such continuously recording ECG system lies in the highly accurate registration of cardiac micropotentials, particularly with ventricular late potentials that are changing in time, whereas the signal-averaging technique does not provide such possibilities.

[The correlation between evidence of breast-cancer in the nipple and the axillary lymph nodes in cancer of the breast in women (author's transl)].

In a random sample of 1,000 mastectomies from microscopically proven cancer of the breast the concomitant evidence of cancer in the nipple and the axillary lymph nodes were studied. Cases with evidence of cancer in the nipple showed axillary metastases twice as often as cases without evidence of cancer in the nipple. In cases with diffuse or lymphangetic evidence of cancer in the nipple the incidence of metastases in the axillary lymph nodes was twice as high in cases with intra-ductal or paget type evidence of carcinoma in the nipple. In cases with evidence of cancer in the nipple, the incidence of metastases in the axillary lymph nodes decreases more with age than in cases without evidence of breast cancer in the nipple. With increasing tumour size the evidence of breast cancer in the axillary lymph nodes was higher in cases without evidence of cancer in the nipple than in cases with evidence of cancer in the nipple. The microscopic differentiation and the grade of malignancy also had an influence on the incidence of axillary metastases in correlation to evidence of cancer in the nipple.

[Prophylactic and therapeutic application of anti-inflammatory agents on the test method of carrageenin paw edema in the rat (author's transl)].

A number of anti-inflammatory substances and cyclophosphamide were tested for their anti-inflammatory activity on a modified carrageenin-tragacanth-induced oedema of the rat paw. The oral application of the substances occurred before or simultaneously with the induction of the inflammation (prophylactic) as well as 2, 6 and 24 h post-irritation (therapeutic). The paw volume was measured plethysmometrically after application of the substances at 3 and 5 h, respectively (short-term test) and 24 and 25 h, respectively (long-term test). Acetylsalicylic acid and phenylbutazone showed a good anti-inflammatory activity specially in the short-term test when given prophylactically. In contrast, flufenamic acid and indomethacin up to 6 h post-application showed both prophylactic and therapeutic activity in both short- and long-term test. The steroidal compound prednisolone was effective when used therapeutically in short-term test up to 24 h post-application as well as in long-term test up to 6 h post-application. Azapropazone and cyclophosphamide were ineffective, among other factors, because of too low a dosage.

[ROC-analysis in testing and training the interpretation of radiographs with mammography serving as an example (author's transl)].

ROC (receiver operating characteristics) curves taken from the results obtained in evaluating the performance of ten different viewers with different experience in the interpretation of 120 xeromammograms and 120 conventional film mammograms each, serve as a basis to discuss the value of ROC-analysis in evaluating and improving the interpretation skill of individuals reading radiographs.

[Comparative clinical study on the accuracy of conventional and electronic blood pressure self-measuring devices (author's transl)].

19 conventional and 19 electronic blood pressure measuring devices were tested on a average of 23 patients each. The patients were classified according to age, sex, and experience in blood pressure measurement. Altogether, the accuracy of the electronic devices was inferior to that of the conventional apparatus. Especially the diastolic blood pressures were estimated considerably too low by the electronic devices. Furthermore, these instruments were more sensitive to a false position of the microphone membrane than the conventional devices and they showed, in the average, greater differences between two exemplars of the same instrument. Blood pressure readings with conventional devices were significantly influenced by the patients' experience (systolic blood pressure) and age (diastolic blood pressure), whereas the readings with electronic machines were significantly affected by the patients' sex (systolic and diastolic blood pressure) and age (diastolic blood pressure). The result of the technical development of blood pressure devices for self-measurement is rather disappointing, especially with respect to costs and benefits.

[Pharmacological effects on the motor activity of Oddi's sphincter. Postoperative electromanometric measurements of the bile ducts].

Besides the high diagnostical value of the electromanometric measurement, this method is especially useful for the registration of pharmacological effects upon the sphincter of Oddi. In this study the influence of hymecromon on sphincter function was tested in 138 patients. Hymecromon induced a significant decrease of pressure by prolonging the opening time (p < 0.001). In clinical experiments it could be shown that the increase of pressure induced by morphine in the common bile duct could be equalized by hymecromon within a short time. Depending significantly on the dosage (p < 0.01) there were identical results on the intravenous or intraductal application of hymecromon. An increase of activity of the liver-specific enzymes could not be seen after hymecromon treatment.

Extracellular K+ concentration and K+ balance of the gastrocnemius muscle of the dog during exercise.

Exercise-induced changes of the extracellular and the venous K+ concentrations ([K+]e, [K+]ven) were measured continuously in autoperfused dog gastrocnemii using PVC liquid membrane electrodes. O2 consumption (VO2), blood flow and performance of the muscles were also determined. The muscles were stimulated indirectly and isotonic tetanic contractions (0.2 s) were produced every 0.7 s for more than 45 min. 1. [K+]e started to increase immediately after the onset of exercise and reached a maximal value of 7.4 meq/l after 4 min of exercise, thereafter it decreased, reaching 5.3 meq/1 at the end of the 12 min exercise. After 6 min of recovery [K+]e became lower than the control level. [K+]ven reached maximal values of 5.0 meq/l after 30 s of exercise and thereafter decreased slowly to 3.8 meq/l at the end of exercise. Thus, a K+ gradient of up to 3 meq/l was observed between the interstitial space and the venous blood. About 30 s after the end of exercise a net K+ uptake of up to 0.3 mu eq per min per g was observed. 2. The K+ loss of the muscle fibres reached maximal values of 1.5 mu eq per min per g at 1.5 min after the onset of exercise. Total K+ loss was 7% during 12 min of exercise and 12.5% of intracellular K+ during 45 min of exercise. 3. The changes of [K+]e correlated closely with performance and VO2 during exercise but not during recovery. On the other hand, blood flow correlated with the changes of [K+]e during both exercise and recovery. These results support the hypothesis that changes of [K+]e of the working muscle might be an important factor regulating exercise hyperemia.

[The significance of the nipple in carcinoma of the female breast (author's transl)].

In 1,000 specimens of mastectomy with microscopically proven carcinoma of the breast, cancer was found in the nipple in approximately every fifth case. Extension of the carcinoma into the nipple is more frequent in the higher age group of women. Extension into the nipple is more common with larger tumor size. The microscopic growth pattern of the carcinoma has an influence on the presence of tumor in the nipple whereas the microscopic grading for malignancy according to WHO has no clear cut correlation to tumor in the nipple. The frequency of extension of carcinoma of the breast in the nipple in our series and in most series in the literature is in our opinion a contra-indication to conservative types of operation.
