Any reasonable evaluation presupposes a precise question. Furthermore it is necessary to eliminate all factors of disturbance, which are able either to simulate or to mask a result of evaluation. The most important of these factors is the psychiatrist himself. Therefore, experimental designs have to taken in account the factor psychiatrist. Criteria of the effects of any drug must be relevant with regard to comparison of two or more drug effects. The reproduciability of criteria is very desirable, but a wish-fulfilment under the most circumstances. Because criteria should be independent and mutually exclusive, all scales like the Hamiltons one are not suitable for evaluation. Finally one should use as few criteria as possible. Models of evaluation are described for the most frequent questions in connection with clinical trials in psychopharmacology.

An example stresses the point of well established routines, which are usual in comparative pharmacokinetics today. It seems to be necessary to analyse the problem carefully, before starting any pharmacokinetical trial.
[Effect of intramuscular dihydroergotamine on blood circulation in the early postoperative phase. A clinical study].
The effects of Dihydergot (1,0 mg i.m.) and a placebo on orthostatic regulation disturbances were examined during the mobilisation in the first post-operative phase. According to a modified Schellong-test, 100 fresh-operated patients with hyper- rsp. hypodiastolic regulation disturbances were checked on the first post-operative day. In comparison with the placebo, Dihydergot showed a stabilisation of the systolic blood pressure and a decrease of the pulse rate during orthostatic conditions. Neither Dihydergot nor placebo influenced the diastolic blood pressure. Orthostatically caused side effects were observed less frequent within Dihydergot - therapy than under placebo.

[Clinical studies on the effect of drugs used in the treatment of angina].
The report is based on a four-centre study on 25 patients with coronary insufficiency (ECG signs). In random sequence the following were given to each patient: a nitrate (isosorbide dinitrate), carbocromene, practolol, and oxyphedrine. The initial criterion for inclusion was S-T depression of 0.2 mV on stepwise exercise (bicycle ergometry). S-T depression, systolic blood pressure and heart rate changes were used as criteria. For each drug there was an acute test and a long-term test of 12 days. The Friedman test was used for the statistical evaluation of the results. Isosorbide dinitrate decreased the extent of S-T depression in the acute tests while practolol did so in the long-term test. Practolol reduced systolic pressure on long-term administration, with no difference in the acute experiment. Heart rate was reduced by practolol both acutely and long-term. These results give no indication for any possible differential treatment of coronary insufficiency, but this may be due to the selection of patients in this study.

Statistical methods seem to confirm the impression that a connection exists between the administration of neuroleptic drugs and a lowered cancer mortality rate. However, further analysis shows that the neuroleptic group and the controls represent two distinct populations which cannot be compared with each other.

When giving details of the frequency of Sympathetic Ophthalmia a clear distinction must be made between this disorder and the Phacogenic Ophthalmia also affecting the second eye. The latter is found with lens damage, whereas genuine Sympathetic Ophthalmia in relation to enucleation, eye damage and eye opening surgery at Heidelberg University between 1937 and 1973 is less than 1%, a figure markedly below figures quoted in the earlier literature. In two out of three patients with recent genuine Sympathetic Ophthalmia we found an augmented stimulation index in the LTT with retina antigen, not choroid antigen, as compared to control groups. This can be regarded as due to an individually augmented immunologic reactivity towards eye specific tissue antigens.
