[Accidental drowning with extreme hypothermia--rewarming with extracorporeal circulation].

A 3 1/2 year old boy had fallen into a fishpond. After about one hour the boy was brought to our hospital. He was cyanotic and bloated, the rectal temperature was 18.4 degrees C. There was no heart beat, the pupils were wide without reaction to light. Via a sternotomy the heart lung machine was connected and core rewarming was achieved. After 7 days of artificial respiration the boy could be extubated, after 16 days the boy left our hospital without neurological consequences.

Association of minimal change nephrotic syndrome (MCNS) with HLA-B8 an B13.

HLA antigens of 45 children with idiopathic nephrotic syndrome were typed for 26 phenotypic specificities. A significant association of MCNS with HLA-B 8 and B 13 was found. There was also a significantly increased frequency of the antigen combination HLA-A 1/B 8 (most presumably the haplo-type). The association with HLA-B 8 provides further evidence of an important role of immune mechanisms in the pathogenesis of MCNS.

[Regional service for dialysis and kidney-transplantation in children (author's transl)].

Children with renal failure should be treated in specialized pediatric centres. In October 1977 we initiated a pediatric dialysis unit at the university of Essen, as there were no special facilities for this area (Ruhr). In 18 months 24 children with end-stage renal failure and 15 children with acute renal failure or severe poisoning had to be treated. We performed 2300 hemodialyses and 400 peritonealdialyses; 12 children got a transplant. The pediatric dialysis unit of Essen has all facilities for treatment of children with acute and chronic renal failure. The documented results were possible by cooperation with all specialists of our hospital and with the neighbouring pediatric dialysis centres.

Acute Kidney Injury

[Clinico-pathological data in infants with different types of nephrotic syndrome (author's transl)].

Case reports including light- and electron microscopy about 4 children with infantile nephrotic syndrome are given. Histologic lesions were microcystic renal disease ("Finnish type"), diffuse glomerular mesangial sclerosis, glomerular mesangial proliferation and minimal changes in the 4 patients, respectively. Knowledge of the individual histologic lesion is essential for adequate therapy and the prognosis in all infants with nephrotic syndrome, except in cases with data proving the Finnish type; in some cases, the identification of individual histologic type will lead to adequate genetic counselling of the family.

[Hypertensive emergencies in children with renal hypertension (author's transl)].

Hypertensive emergencies of 10 children with renal hypertension were analysed. Cause of renal disease are chronic renal failure in three, acute renal failure in three, hemolytic uremic syndrome in two, acute post streptococcal glomerulonephritis in one, and renal arterial stenosis in a further patient. Therapy should be started early in the course of the hypertensive emergency, first symptoms are headache and vomiting. Drug of first choice is diazoxide (3-5-(8)mg/kg i.v.). Three patients developed transitory hyperglycemia after repeated injections of diazoxide.

Acute Disease

[BCG-osteomyelitis and BCG-arthritis after vaccination with BCG (author's transl)].

After vaccination with BCG (type Goeteborg) in the newborn period two children developed a BCG-osteomyelitis 12 and 19 months respectively later. The symptoms were only small. X-ray showed bone lesions in the distal metaphysis of the femur. The bacteriological examination demonstrated BCG-etiology. Disturbances of growth or of joint motility are seldom; they are possible, if the bone lesions are severe and if the diagnosis is late. In these cases the treatment must be intensive.

Cortisone cataract in children with nephrotic syndrome.

In a group of 16 children with idiopathic nephrotic syndrome treated with corticosteroids for longer than 12 months, 9 developed a posterior subcapsular cataract (PSC). No correlation between the frequency of PSC and the duration of treatment and the total dose of treatment with steroids was demonstrable. However, the patients with PSC had received considerably higher average daily doses than those without PSC. Two patients with normal ophthalmologic findings at the end of treatment showed PSC 6 and 9 months later respectively. Only one patient acquired a significant impairment of visus.

Adolescent

[Urinary calculi in children (author's transl)].

From a total of 274 children with urinary calculi, 192 cases were analyzed. 63% of these were male. In 19% a familiar disposition was noticed. Phosphate containing stones were found in 81%. Most of these had an urinary tract infection caused by urea splitting organisms. There was no correlation between the chemical composition of the stones and infection with coli and enterococci. One-third of the analyzed cases exhibited malformations of the urinary tract. Of these, 92% had urinary infections due primarily to urea splitting bacteria. This infection rate was markedly higher than in the children without combined malformations of the urinary tract. The peak incidence was around 2 years of age. In the last group symptoms were atypical but more severe than in those cases without malformations of the urinary tract.

[Measurements of bone mineral concentration ("hydroxylapatite-volume values") and of bone density in vitro and in vivo with a densitometric method using beams of two different energies (author's transl)].

The densitometric method of Rassow-Strüter (1969) using beams of two energies permits separate estimations of bone mineral and connective tissue concentrations in bone; their sum indicates bone density. The value of the method has been examined and the early results of in-vitro and in-vivo measurements are quoted. The former were obtained from pairs of macerated calcanei and vertebral bodies embedded in resin blocks. The in-vivo estimations were concerned with obtaining normal values in healthy children aged four to fifteen years and adults aged 18 to 54 years. Standard deviation, obtained from measurements of four different points of both calcanei in adults was KM 12% for Hydroxylapatite-volume values, and for bone density 14%. The average value for KM for the whole group is 198 mg/cm3 with 17% standard deviation of the single measurements compared with the average. The following-up measurements for children with renal disease do not yet allow final conclusions about the correlation of the clinical aspects of case to the measured bone parameters and their value as independent criteria. For 16 nursing mothers a significantly lower average HA volume value KM = 149 mg/cm3 +/- 21% was found.
