Pieces of the puzzle: steps toward affordable health care.

This essay proposes a multiple-part solution to the health care affordability crisis. Solution elements include: changing the most commonly held health insurance product to a plan with high-deductible design, and reinstituting community-based health planning. It is proposed that the federal tax code be used to create incentives to change the most commonly held benefit. Further, it is suggested that "capital licenses" be provided to support health planning.

Fringe mode reflectance laser Doppler microscope system.

A reflectance fringe mode Laser Doppler microscope anemometer (LDMA) has been designed and tested. A novel beam splitter permits adjustment of the direction and separation of the two laser beams which form the fringe sensing volume, and furnishes the control needed to compensate for chromatic aberration and variation in optical quality. Tests conducted in a narrow 28:1 aspect rectangular flow channel filled with water showed close agreement between the predicted and measured velocity profiles. Velocity measurements, made alternately in the reflectance and transmittance modes using the same sensing volume in the channel, revealed deviations between the modes of less than 1.1% for water and 2.3% for whole blood. The deviation for the in vivo experiment was better than 2.8%. Velocity profiles were measured alternately in the same sensing volume, (dimensions were 10 x 12 microns) by reflectance and transmittance LDMA in 40 hamster cheek-pouch arterioles, ranging from 65 to 140 microns in diameter. These showed close agreement throughout the range of measured vessels. Reflectance LDMA makes non-invasive, high resolution velocity measurements in the range of 0.03 to 40 cm/s possible in small blood vessels near the surface of opaque organs and tissues. The accuracy and reproducibility of the readings were calculated to be better than 1.1%. Reflectance fringe mode LDMA greatly extends the applicability of LDMA because sites where transmittance LDMA can be applied are limited.

Fringe mode transmittance laser Doppler microscope anemometer: its adaptation for measurement in the microcirculation.

Blood flow analysis in the microcirculation requires accurate measurement of velocity, volume flow and shear-rate versus shear-stress relationships. The resolution of most anemometers is too limited to obtain useful measurements, especially near the blood vessel wall and at branches and bifurcations. To make such measurements possible with a noninvasive, high resolution, accurate technique, we have developed a fringe mode, transmittance laser Doppler microscope anemometer (LDMA). This system has an intrinsically high spatial resolution (10 x 12 microns), and does not require a high concentration (10(6)/cm3) of scatters or red blood cells (RBC) as in our application. Preliminary measurements of water flow in a rectangular channel were conducted to ascertain the reliability and accuracy of velocity measurements using the LDMA. Velocity profiles were then measured by the LDMA system in arterioles 38-135 microns in diameter, in the transparent, everted cheek pouch of the anaesthetized hamster. The extremely high resolution of the optical system, and the ultra-fine traversing mechanism of the microscope stage, made velocity readings larger than 0.02 mm/s with accuracy and reproducibility better than 1%, possible near the wall to within 7-10 microns.

Alternatives to psychiatric hospitalization for children.

The impetus for the dramatic increase in the number of treatment alternatives for children has come from changes in the theoretical conceptualization of treatment, social and political pressures, and financial considerations. This article reviews the literature on alternatives to hospitalization, appraising the available data on the effectiveness of psychiatric hospitalization and its alternatives and considers future research needs and the development of services in this area.

Spreading of vascular endothelial cells in culture: spatial reorganization of cytoplasmic fibers and organelles.

The three-dimensional organization and fine structure of cytoplasmic components within whole non-embedded bovine aortic endothelial cells were examined during their attachment and spreading in tissue culture. Cells were cultured directly on Formvar-coated gold grids, fixed in glutaraldehyde and osmium tetroxide, critical point dried and examined by transmission electron microscopy (TEM) using stereoscopic methods, and by scanning electron microscopy (SEM). Reorganization of cytoplasmic structures during cell spreading occurred in four sequential stages: (1) spreading of the plasma membrane and unstructured cytoplasmic matrix; (2) spreading of cytoplasmic fiber systems (microtubules, microfilament bundles and microtrabecular system); (3) alignment of microfilament bundles and formation of radial tracts of microtubules; and (4) centripetal movement of organelles along radial tracts. These stages observed by TEM correlated with progressive degrees of cell flattening as visualized by SEM. These studies demonstrate that a characteristic reorganization of intracellular fiber systems and organelles accompanies the spreading of endothelial cells in culture.
