Leakages after endovascular repair of aortic aneurysms: classification based on findings at CT, angiography, and radiography.

PURPOSE: To ascertain whether the configuration and location of leakages identified at computed tomography (CT) could provide evidence of their angiographically and fluoroscopically confirmed causes. MATERIALS AND METHODS: Fifty patients aged 26-79 years underwent endovascular repair of traumatic (n = 4) or arteriosclerotic (n = 46) aortic aneurysms (four thoracic, 46 infrarenal). Radiographic examinations in three planes and helical CT were performed 1 week after implantation and every 3 months thereafter. Angiography was performed when there was evidence of a leakage at CT. RESULTS: CT demonstrated evidence of leakages in 13 patients. Broad-based leakages immediately adjacent to the prosthesis were termed "perigraft leakages." If the area most affected by the leakage lay along the border of the aneurysm, then retrograde leakages were apparent at angiography. If the leakage was ventral to the prosthesis, then its source was the inferior mesenteric artery; if it was dorsolateral, then it was supplied by either the lumbar arteries or the median sacral artery through the hypogastric artery. One circumferential leakage could not be evaluated adequately at CT or angiography. Radiography depicted a rupture of the stent mesh in the middle of the prosthesis. Selective angiography demonstrated all types of leakages and permitted CT classification. CONCLUSION: The cause of a leakage can be determined with CT on the basis of its configuration and location in the majority of cases.

[Preoperative assessment of asymptomatic adnexal tumors by positron emission tomography and F 18 fluorodeoxyglucose].

AIM: To evaluate use of F-18-FDG-PET in assessment of dignity of asymptomatic adnexal masses. METHODS: 85 asymptomatic patients with suspicious, asymptomatic adnexal masses were evaluated. Static FDG-PET (Exact HR+ or ECAT 931) imaging of the abdomen was performed following application of 222-555 MBq F-18-FDG. Iterative reconstruction was applied. PET data were analysed visually, at first without and second together with MRT images. Final diagnosis was made by histopathology. RESULTS: FDG-PET allowed correct identification of 4 of 8 malignant adnexal tumors. False negative results were obtained in 2 adenocarcinomas stage pT1a and 2 borderline-tumors. In 60 out of 77 benign adnexal masses malignancy could be excluded. False positive FDG-uptake, partly because of misinterpretation of gastrointestinal activity, was found in 3 inflammatory processes, 1 teratoma, 1 benign schwannoma, 1 dermoid cyst, 1 benign thecoma, 1 serous cyst, 1 serous cystadenoma, 2 mucinous cystadenomas, 2 corpus luteum cysts, 3 endometriosic cysts and 1 sactosalpinx. The overall sensitivity and specificity of FDG-PET alone were 50% and 78%. Evaluation together with MRT images showed a sensitivity of 50% and a specificity of 86%. CONCLUSION: Sensitivity of FDG-PET in detection of borderline-tumors and early stage ovarian cancer seems to be limited. Low incidence of malignant ovarian tumors requires for assessment of dignity a procedure of high specificity, that is not reached by FDG-PET neither without nor together with MRT images for topographic orientation. Therefore use of FDG-PET for assessment of dignity in suspicious, asymptomatic ovarian tumors is limited.

[MRI of the abdomen combined with enteroclysis in Crohn disease using oral and intravenous Gd-DTPA].

In spite of the improved MR-diagnosis of the abdomen, MRI is not used as a routine method for the diagnosis of inflammatory small bowel disease. The aim of this study was--after optimazation of the bowel opacification--the correlation of the findings obtained with enteroclysis and MRI in patients with known Crohns' disease. 60 patients between 17 and 72 years of age were investigated. First, an enteroclysis was performed in typical manner. The applicated methylcellulosis was blended with positive oral MR contrast media (Magnevist oral, Schering). After enteroclysis, MRI of the abdomen was performed using T1- and T2-weighted breathhold sequences (Flash 2D pre- and postcontrast and TSE) in axial and coronal planes. The length of the affected bowel and the stenosis seen with enteroclysis correlated well with the visible thickening of the small bowel wall and the stenosis seen in MRI. Using MRI, additional findings could be obtained in 28 patients, such as fistulas, abscesses or a hydronephrosis, or a better assessment of the stenosis was possible with MRI, because of the avoidance of overshadowing of the affected bowel loop with MRI. A brilliant MR-tomographic imaging of the small bowel is possible under the condition, that the small bowel contrast is optimal. The main prerequisite is a large filling volume of the small bowel to reach a homogeneous contrast and a good distension of the small bowel lumen.

[High-resolution virtual laryngoscopy based on spiral CT data].

This pilot study investigated the feasibility and clinical value of high-resolution virtual real-time laryngoscopy based on helical CT data sets. Nine patients with laryngeal pathology (three with tumors of the vocal cords, two laryngeal carcinomas, one with invasion of the larynx by thyroid carcinoma and six subglottic stenoses) underwent examination by helical CT at a collimation of 1 mm. Following acquisition, the images were processed at a workstation with standard visualization software, such that virtual endoscopy (VE) in real time was possible. The images were then compared with the findings of conventional endoscopy. Because of swallowing artifacts, reconstruction failed in 2 of 12 patients. None of the carcinomas of the vocal cords was recognized at VE or in the cross-sectional CT images. VE provided the correct diagnosis in 8 of 12 cases (laryngeal tumors, subglottic stenoses). Virtual laryngoscopy is capable of simulating the visual findings of endoscopy in cases of laryngeal tumors and subglottic stenoses. Small tumors of the vocal cords are not adequately visualized. The major problem affecting results is motion artifacts resulting from involuntary swallowing.

MRI follow-up of pisiform bone transposition for treatment of lunatomalacia.

OBJECTIVE: Transposition of the pisiform bone is an operative treatment for lunatomalacia. The postoperative viability of the transposed pisiform bone is difficult to assess. The purpose of the study was to evaluate the utility of MRI for postoperative assessment of viability of the pisiform and lunate bones. DESIGN AND PATIENTS: Six patients who underwent transposition of the pisiform for treatment of lunatomalacia, were assessed pre- and postoperatively with conventional radiographs (including tomography), CT and MRI. RESULTS: Conventional radiographs, conventional tomograms and CT were all useful in demonstrating the location of the transposed pisiform. CT showed the transposed pisiform without superposition. However, neither CT nor conventional techniques provided information regarding viability of the pisiform. In all patients investigated in the first year following surgery, T1-weighted MR images showed high signal intensity in the transposed bones. In all patients investigated after 1 year, the signal intensity decreased to an intermediate level on T1-weighted images. Enhancement following contrast medium administration in the transposed pisiform and the lunate was noted in all patients, indicative of viability. CONCLUSION: Contrast-enhanced MRI is able to provide important information regarding the viability of the transposed pisiform and the remaining parts of the lunate. Thus, contrast-enhanced MRI provides an improved means of postoperative assessment regarding short-term follow-up following pisiform transposition. In the long-term follow-up conventional radiography and CT may be equal to MRI in showing increasing sclerosis and/or fragmentation.

Magnetic resonance epidurography with gadolinium-DTPA.

The aim of this study was to evaluate and describe MRI epidurography as a new imaging tool. Five volunteers and one patient were investigated with MR epidurography after injection of 20 ml Gd-DPTA solution (1:250/1 ml Gd-DPTA/250 ml normal saline). Magnetic resonance epidurography is possible. With fat-suppression techniques, the contrast between Gd-DPTA solution in the epidural space and surrounding soft tissue proved adequate. Using the multiplanar capability of MRI with MR epidurography coronal and sagittal projections similar to conventional epidurography, axial slices comparable to CT epidurography can be obtained. Magnetic resonance epidurography is superior to conventional and CT epidurography. Presently, due to high costs as compared with conventional and CT epidurography, MRI is not suitable for the routine monitoring of peridural catheters, but it may have a place in the future with decreasing costs for MRI and for the evaluation of patients with spine pathology, especially in describing epidural processes.

[Value of dynamic MRI in the diagnosis of hypophyseal microadenomas].

PURPOSE: To evaluate dynamic MR imaging of the pituitary gland. MATERIAL AND METHODS: 19 patients with suspected mass lesions of the pituitary gland were examined at 1.5 Tesla with dynamic and standard MRI using a Turbo-FLASH sequence (1 image/s for 40 s). RESULTS: In 13/19 patients microadenomas were detected. One of the 13 microadenomas was detected using dynamic imaging and was not seen on standard MRI. The remaining 12 microadenomas were diagnosed with standard MRI. CONCLUSION: Dynamic imaging of the pituitary gland is a time-consuming and costly diagnostic technique. If laboratory results suggest the presence of a microadenoma and conventional MRI is unable to localise it, dynamic imaging should be performed.

[Transjugular intrahepatic portosystemic shunt in children. Initial clinical experiences and literature review].

PURPOSE: To present special methodical and clinical findings of transjugular intrahepatic portosystemic shunts (TIPSS) in children and to discuss potential indications. PATIENTS AND METHODS: Between 1993 and 1996, 6 children aged 2-13 years were treated by TIPSS-insertion. In four cases, the underlying disease was extrahepatic biliary atresia (EHBA) and in two cases liver fibrosis secondary to treatment of neoplasms during early childhood. Indications for TIPSS insertion were variceal bleeding resistant to other treatment modalities in three patients, hypersplenism in one patient and both bleeding and hypersplenism in two. Portal vein punctures were performed using 16-gauge needles, because 19-gauge fine-needles showed insufficient stiffness. The mean follow-up was 24.5 months. RESULTS: Shunt insertion succeeded in all children with a mean procedure time of 5.2 hours. Periportal fibrosis associated with EHBA, atypical course of hepatic veins and small diameters and distances of vessels were conditions making the procedure difficult. Bleeding ceased in all patients, peripheral platelet counts rose by a mean value of 58%. Procedure-related complications were minor extrahepatic bleeding in one child and temporary haemolysis in another child. Restenosis resulted in three patients and were treated successfully by means of transjugular interventions. 5 children remain free of symptoms to this day, one child underwent successful orthotopic liver transplantation 8 months after TIPSS. CONCLUSIONS: TIPSS insertion is technically more difficult in children and has to consider child growth and possible subsequent liver transplantation. Potential indications are recurrent variceal bleeding, also of intestinal origin, resistant to standard treatment and clinically significant hypersplenism.

[Temperature stress for patients in routine magnetic resonance examinations].

PURPOSE: To determine energy exposure and temperature changes in routine magnetic resonance imaging practice. MATERIALS AND METHODS: Body core and skin temperatures were compared in 155 persons (143 patients, 12 volunteers) undergoing routine magnetic resonance examinations with a 1.5 T field-strength magnetic resonance tomography unit using a fluoroptic temperature measurement system. RESULTS: Average applied energy was 0.3 W/kg for whole body and 1.92 W/kg for spatially localized SAR. The maximum whole-body SAR was 1.43 W/kg spatially localized. Body core temperatures differed from those of the control group by a median 0.1 degree C and only a few patients (16.8%) exceeded the limit (+/- 0.5%) at which regulatory mechanisms set in. All patients remained within the normal physiological circadian temperature range (+/- 1 degree C). Skin temperature rose a median 0.49 degree C, with a maximal increase of 5.31 degrees C, which may be considered to be within the limits of physiological temperature change. CONCLUSIONS: Clinically relevant warming of the body is unlikely in routine magnetic resonance imaging practice.

Diagnostic value of spiral-CT angiography in comparison with digital subtraction angiography before and after peripheral vascular intervention.

To evaluate spiral-computed tomography (CT) angiography in primary diagnosis and/or in noninvasive follow-up after vascular intervention, we compared spiral-CT angiography and conventional angiography before and after vascular intervention. Helical-CT examinations before and after percutaneous transluminal angioplasty (PTA) or stent implantation were performed in 10 patients (mean age 63 years) with symptomatic peripheral arteriosclerotic disease. Stenoses were located in the iliac, femoral, or popliteal artery. CT examinations were done with a spiral-CT in double detector technique (CT Twin, Elscint). The parameters were as follows: slice thickness: 5.5 mm, increment: 2.7 mm, pitch: 1.5, contrast medium: 150 mL, flow rate: 2.5 mL/second, delay: 30 seconds. For evaluation, transverse planes as well as maximum intensity projections and 3-D reconstructions were used. The possible scan length reached from the aortic bifurcation down to about 10 cm below the ankle trifurcation. Preinterventional digital subtraction angiography (DSA) was superior to CT angiography (CTA: 94%, maximum intensity projection [MIP] alone: 65%), although high-grade stenoses were detected by both methods. After intervention, a resolved stenosis and improved peripheral flow could be detected by helical-CT as well as by intraarterial angiography in every patient (100%). In the primary diagnosis of vascular changes, intraarterial DSA remains the method of choice. Nevertheless, spiral-CT angiography shows comparable results after percutaneous intervention and becomes a noninvasive alternative in the postinterventional follow-up.

Virtual cystoscopy based on helical CT scan datasets: perspectives and limitations.

The purpose of the study was to simulate cystoscopy based on three-dimensional helical CT scan datasets in real-time in patients with tumours of the urinary bladder. A helical CT scan with double detector technology was carried out pre-operatively in 11 patients with histologically confirmed carcinoma of the urinary bladder and one patient with chronic cystitis. A non-enhanced scan was first performed, followed by an examination in the early phase of contrast medium enhancement. Further images were acquired after adequate filling of the bladder with contrast medium, approximately 30 min after injection. These data were transferred to a separate graphic computer workstation and reconstructed. The results were then compared with the cystoscopic and histopathological findings. All tumours of the urinary bladder identified at fibreoptic cystoscopy were shown on virtual cystoscopy. The best reconstruction results were obtained from data acquired 30 min after injection of contrast medium. The ureteric orifices were not visualized at virtual cystoscopy. These data lead us to conclude that, at present, virtual cystoscopy has not reached the quality of fibreoptic examination and remains restricted to use in specific cases, for example patients with urethral strictures.

Metastases to the pancreas.

Disparity in prognosis and management between primary and secondary pancreatic tumours makes recognition of metastases to the pancreas on CT and MRI an important goal. Three different patterns of secondary pancreatic tumours may be seen: localized, multifocal, or diffuse enlargement. CT findings include hypodense lesions, which show rim enhancement following intravenous contrast medium. On MR examination, the lesions are usually hypointense on T1 weighted and hyperintense on T2 weighted images.

Mechanical thrombolysis of acute occlusion of both the superficial and the deep femoral arteries using a thrombectomy device.

OBJECTIVE: Our objective was to evaluate the efficacy of the Amplatz thrombectomy device for recanalization of acute occlusions of both the superficial and the deep femoral arteries. MATERIALS AND METHODS: Eighteen patients with acute occlusions of the femoral arteries (eight male, 10 female; 10-87 years old) were treated using the Amplatz thrombectomy clot macerator. The duration of occlusion was 16 +/- 8 hr. Eighteen patients underwent treatment of the deep femoral artery, and 16 patients had additional involvement of the superficial femoral artery. After primary recanalization of the deep femoral artery, the superficial femoral artery was also recanalized using the Amplatz thrombectomy device. Nine patients required additional aspiration thrombectomy of the tibial arteries, five patients required additional aspiration thrombectomy of side branches of the deep femoral artery, and 12 patients required additional local thrombolysis with urokinase. RESULTS: In 14 (78%) of 18 patients, recanalization of the deep femoral artery was complete without demonstrable residual thrombi. Arterial spasms were observed in five patients (28%). The rate of limb salvage was 94% at a mean follow-up interval of 8.9 +/- 4.1 months. In the 18 patients, the ankle-brachial pressure index went from a median value of 0.56 before therapy to a median value of 0.91 after therapy. No severe complications occurred. CONCLUSION: Mechanical thrombolysis in the deep femoral artery with the Amplatz thrombectomy device is an effective, rapid method of treatment and is rarely associated with complications. In cases of concomitant occlusion of the tibial arteries, recanalization should always be attempted because the deep femoral artery may provide a functionally decisive collateral artery between the iliac and tibial vasculature.

[MRCP (magnetic resonance cholangiopancreatography)--an assessment of current status].

For the first time, magnetic resonance cholangiopancretography (MRCP) provides images of the biliary and pancreatic ducts based on 3D data similar to those we are used to from endoscopic retrograde cholangiopancreatography (ERCP) and percutaneous transhepatic cholangiography (PTC). Extraordinarily impressing is the fact that no contrast media have to be applied. The following manuscript shows technical basics and the development of this technique from gradient echo sequences to the recent single-shot techniques. Furthermore possibilities and limitations of MRCP will be discussed with respect to certain illnesses.
