Identification of xenobiotic-degrading isolates from the beta subclass of the Proteobacteria by a polyphasic approach including 16S rRNA partial sequencing.

Nineteen gram-negative, aerobic, biodegradative isolates were identified by using a polyphasic taxonomic approach. The presence of the specific polyamine 2-hydroxyputrescine and the presence of a ubiquinone with eight isoprenoid units in the side chain (ubiquinone Q-8) allowed allocation of these organisms to the beta subclass of the Proteobacteria. On the basis of the results of additional characterization experiments (i.e., API 20NE tests, determinations of soluble protein patterns, and DNA-DNA hybridization experiments), we classified six isolates as either Comamonas testosteroni, Comamonas acidovorans, or Alcaligenes xylosoxidans subsp. denitrificans. By using the same criteria we allocated two additional isolates to the genus Alcaligenes. A comparison of a 16S rRNA fragment (positions 1220 to 1377; Escherichia coli nomenclature) indicated that the remaining isolates should be allocated as follows: one is a member of C. testosteroni and one is a member of Acidovorax facilis, as confirmed by the results of additional DNA-DNA hybridizations; two others probably belong to the family Alcaligenaceae; six are related to "Alcaligenes eutrophus"; and one, strain NRRL 12228, occupies an isolated position.

Bacterial Proteins

The bactericidal action of streptomycin: membrane permeabilization caused by the insertion of mistranslated proteins into the cytoplasmic membrane of Escherichia coli and subsequent caging of the antibiotic inside the cells due to degradation of these proteins.

The mechanism by which the aminoglycoside antibiotic streptomycin permeabilizes the cytoplasmic membrane of Escherichia coli cells was reinvestigated. For this purpose, the extent of streptomycin-induced K+ loss from cells growing at low external K+ concentrations was taken as a measure of membrane permeabilization. Experiments with different K(+)-uptake mutants showed that the antibiotic specifically increased the passive permeability of the cell membrane to K+ and other ions. These permeability changes were small and the membrane potential of the treated cells remained high. The membrane permeabilization was not due to a direct interaction of the antibiotic with the cell membrane, since cells that carry an rpsL mutation and synthesize proteins in a streptomycin-insensitive way did not lose K+ after the addition of the antibiotic. Due to misreading and premature termination of translation the cells synthesized aberrant proteins under the conditions where membrane permeabilization occurred. Two conditions are described under which the cells both degraded these mistranslated proteins rapidly and reaccumulated K+, lending support to the hypothesis that membrane permeabilization is due to the presence of the mistranslated proteins in the cell membrane. Evidence is presented that the irreversibility of (dihydro)streptomycin uptake by cells washed free from the antibiotic might also be due to rapid degradation of the mistranslated proteins, leading to 'caging' of the antibiotic inside the cells.

Analysis of the polyphosphate-accumulating microflora in phosphorus-eliminating, anaerobic-aerobic activated sludge systems by using diaminopropane as a biomarker for rapid estimation of Acinetobacter spp.

Polyphosphate-accumulating gram-negative bacteria were isolated from different anaerobic-aerobic activated sludge systems with diverse processes for enhanced biological phosphorus (P) elimination. Of 22 isolates, 10 were allocated to the genus Acinetobacter by using multiple-test systems and soluble protein and polyamine patterns. As diaminopropane (DAP) appears to be the characteristic main polyamine compound produced by Acinetobacter spp., it was used as a biomarker for the genus. The high DAP contents of representative samples from municipal wastes with enhanced biological P elimination indicated that Acinetobacter spp. can be dominant organisms in sewage treatment plants with low organic loading and nitrification and denitrification steps. Contrary to accepted opinion, sludge from treatment plants with efficient P removal and high organic loading had a low DAP content, indicating that bacteria other than Acinetobacter spp. are responsible for enhanced biological P elimination in these plants.

Acinetobacter

[Ultrasound-controlled cutting biopsy for collecting histologic specimens].

The fine-needle puncture under ultrasound guidance has the disadvantage that the material obtained can only be evaluated by cytology. The cutting biopsy cannula represents a compromise between the fine and the Menghini needle. In this way, small tissue cylinders can be attained. The aim of the present study was to investigate to what extent the sampled specimen could be assessed by cytology or histology. Fine-needle puncture resulted in 22 (69%) out of 32 cases in material suitable for cytology. Cutting biopsy led in 34 out of 36 cases (94%) to specimen valid for histology. Thus, cutting biopsy clearly extends our diagnostic tools. It is not yet clear at present whether procedures with a higher risk - such as the Menghini puncture - can be in part replaced by alternative ones.

[Coronarography findings following systemic short-term lysis of acute myocardial infarct].

In 50 patients with acute infarction who were admitted within 6 hours after the beginning of the complaints randomizedly an intravenous streptokinase short-term lysis or a conventional heparin phenprocoumon treatment was performed and 4 weeks later the results were objectified by coronarography and ventriculography. The patients were subdivided into 3 groups: 1. control group (without fibrinolysis), 2. early lysis (within 4 hours) and 3. late lysis (4-6 hours). Four weeks after the infarction the proportions of the open infarction vessels are approximately of the same size in all three groups, the functional results are best (end-diastolic pressure and kinetics of the cardiac wall) in the group of patients who underwent an early lysis. Successfully lysed patients with remaining residual stenosis must be regarded as particularly threatened by infarction and remain under special control (residual angina, ergometrically objectified exercise tolerance, perhaps coronarography) in the phase of rehabilitation.

[Rational procedures in the differential diagnosis of jaundice].

In jaundice after exclusions of prehepatic and functional hepatogenic hyperbilirubinaemias the sonography should pre-eminently be used as a riskless, economical and qualified investigation method, taking into consideration clinical and laboratory-chemical data. If sonographically the findings of an intrahepatic cholostatis are shown, in therapeutic relevance the histological clarification must follow. Only in unequivocal focal changes of the liver (perhaps thin needle puncture) further investigations are unnecessary. If there are findings of an extrahepatic cholostasis, in a not unequivocal sonographic result or before a surgical intervention further aimed investigations, such as ERCP, PTC and CT, are necessary for the exact clarification of the cause of the obstruction of the bile ducts.

[Results of ultrasound tomography in the diagnosis of liver diseases].

In a prospective study the impact of ultrasound tomography in the recognition and exclusion, respectively, of liver diseases was investigated. 646 patients with a suspected liver disease were sonographed in real-time procedure. The investigator was familiar with the history as well as with clinical and lab findings before the examination was started. The sonography was performed before a laparoscopy, liver blind punktion, scintigraphy, angiography, computerized tomography. The diagnoses were confirmed by sufficient, comparable methods (see above) or operatively; they were supported by a follow-up for one year. While liver cysts, cystic livers, liver abscesses, haematomas, metastases (with a diameter of more than 10-20 mm), stasis liver, cirrhoses with portal hypertension and fatty livers could be diagnosed with a high rate of confidence by sonography, healthy livers and the following disorders could not exactly be separated by ultrasound: acute hepatitis, reactive hepatitis hepatoses, chronically persisting hepatitis, chronic active hepatitis, and livers with a low incorporation of fat.

[Use of B-sonography in the diagnosis of liver diseases].

The criteria important for the sonographic judgment of the liver and the value of the sonography in the strategy of the diagnostics of liver diseases are described. Since none of the symptoms mentioned, such as size of the liver, ventral contour, lower margin of the liver, demarcability, sound conduction as well size, density and distribution of the structural reflexes, is typical for a certain disease, an approximate diagnosis or the exclusion of certain diseases is a possible only taking into consideration several symptoms and also extrahepatic, sonographically probable ones. On account of the simplicity and of the undangerousness of the examination it is obvious to use the sonography immediately after the examination and thus to organize the selection of further diagnostic measures more rational.

Cysts

[Possibilities of ultrasonic diagnosis of thyroid and gastrointestinal diseases].

Issuing from the physical principles of the ultrasound technique and the explanation of the different investigation methods, the possibilities of the ultrasonic diagnosis are demonstrated on own results in 1,166 cases of struma nodosa and 154 palpatorically and radiologically, respectively, localised enlargements of the organs or demands of space in the abdominal region. The establishment of 46 cysts in 357 scintigraphically warm nodes of the thyroid gland allows the conclusion that a relatively high percentage of the warm nodes contains solid cold regions. In several cases the ultrasonic diagnosis makes a clear diagnosis, in other cases it is an important supplementation to the other diagnostic possibilities. The A-picture-apparatuses produced in the GDR are suitable for the differential diagnosis of palpatorically or radiologically localised enlargements of the organs or space demands. In diffuse diseases of the organs and for the search of disturbances in extended regions B-picture-apparatuses with improved resolving power are necessary.

Bile Ducts

[Value of sonography in the differential diagnosis of icterus].

In early use the sonography may facilitate and accelerate the differential diagnosis of the jaundice. In parenchymal hepatic icterus frequently pathologic changes of the liver may be proved sonographically. The typical findings of dilated and thus sonographically demonstrable intra- and extrahepatic biliary ducts speak for obstructive jaundice. Characteristic constellations of dilated segments of the bile duct allow conclusions to the localisation of the occlusion. Changes which cannot be explained by sonography are a cause for further examinations.

[Internal medicine and dactyloscopic examinations of mothers with malformed children].

A retrospective study concerning anamnestic data as well as an internal and dactyloscopic examination were caried out on 75 woman who were delivered from a deformed child. Apart from the well-known causes of a deformity the chronic arterial hypertension is emphasized as teratogenetic factor. The dactyloscopic examination did not result in an accumulation of stigmata which refer to a genetic condition of the deformities. Differences concerning the distribution of patterns on the finger-tips need further investigations; at present they are not yet to be explained.

[Hepatic coma--causes, recognition, treatment].

The name of the disease coma hepaticum is a collective notion for etiologically, pathologo-anatomically and biochemically different kinds of a failure of the function of the liver. A demarcation of different forms of the liver coma is necessary above all for prognostic and therapeutic reasons: hepatic disintegration coma, coma in hepatic failure, mixed coma, false hepatic coma, ferment block coma. Despite certain progress the therapeutic possibilities in hepatic coma cannot satisfy in recent years. Also the use of different, partly very expensive methods of the so-called liver substitute (exchange function and plasmapheresis, heterologous and homologous extracorporal liver perfusion, parasymbiotic crossed circulation, haemoperfusion with encapsulated activated charcoal, liver transplantation) up to now did not lead to a decisive improvement of the nearly infaust prognosis of the hepatic disintegration coma. Therefore preventing measures and careful control of endangered patients deserve particular attention.

Anti-Bacterial Agents

[Discriminatory function of serum proteins in liver and biliary tract diseases].

In 104 patients with acute virus hepatitis, chronic hepatitides, cirrhoses, fatty livers and biliary diseases with partial and complete obstructive jaundice, respectively, IgG, IgA, IgD, beta1A- and beta1E-globulin, cholinesterase, total protein, and albumin, in 45 of these patients additionally prealbumin, retinol binding protein, thymol turbidity test were determined as well as an electrophoretic separation of the serum was performed. 11 persons with healthy liver served as control group. According to the results of univariate and multivariate variance analyses with following test of redundance (test for indispensability) and analysis of discriminance with calculations of reclassification IgD, beta1E-globulin and retinol binding protein were identified as not evident or redundant. Electrophoresis and thymol turbidity test give sufficient basis informations and can further be recommended for orienting examinations. Immune globulinogrammes from IgB, IgA and IgM are suitable as so-called mesenchyma tests particularly for controls of the course. Prealbumin and cholinesterase prove to be the most sensitive parameter of synthesis, whereas albumin and beta1A-globulin possess a high prognostic evidence.

[Cellular immunoreaction in alcohol-induced liver damage].

Of 231 patients with alcohol-induced liver diseases of different degrees of severity 170 patients had a pathological result of the migration inhibition test, when autologous homogenate of the liver was used as antigen. Of them in 25 cases a stimulation, in the other patients an inhibition of the migration of leucocytes was present. With increasing degree of severity of the liver lesion the number of cases with migration inhibition increased. These findings refer to a cellular immune reactivity in alcohol induced liver diseases.
