Assessment of the Florida Pharmacist Self-Care Consultant Law using patient profile and prescription audit methods.

This article examines pharmacist nd physician prescribing patterns during the first two years of the Florida Pharmacist Self-Care Consultant Law. Nineteen pharmacies were audited. The mandatory patient profiles were audited for a two-year period, and the prescription files were audited for a five-year period. For the nine pharmacies maintaining patient profiles, the prescribing activities of the pharmacists almost double from the first year to the second year. However, these prescriptions made up a very small portion of the total. Pharmacist prescribing appeared to be limited to three primary drug categories: topical pediculicides (lindane shampoo), oral analgesics, and otic analgesics; these comprised 82 percent of all pharmacist-generated prescriptions.

The pharmacist as prescriber.

During the past few years, patients have taken more active roles to provide for their own health needs and to find more economical ways to meet their overall medical care needs. This has created a variety of demands on the health care system. Community pharmacists have responded to these demands by expanding the services and product lines available to the public (e.g. home health care supplies, medical durable equipment, diagnostic home kits, health screening programmes, and public health educational programmes). In Florida, the public pressure for more economical ways to provide health care resulted in the passage of the Pharmacy Self Care Consultant Law. The intent of the law is to provide the consumer with access to effective medications at relatively lower cost and less inconvenience than is involved in seeing a physician for minor self-limiting medical conditions. At the same time, the law has effectively created a third class of drugs which the pharmacist can prescribe. This has given the pharmacist another opportunity to meet the health care needs of the public more effectively.

Distribution and cost of drugs in a pharmaceutically decentralized health maintenance organization.

A private, free-standing health maintenance organization, Group Health of Arizona, was established with a decentralized pharmacy distribution system. A closed formulary and monthly peer review by pharmacists and physicians influence the number of prescriptions written as well as drug cost. A study was conducted to determine: the 50 most-prescribed items by number and cost; drug cost according to certain therapeutic categories; the average drug cost per prescription; the number of prescriptions dispensed per enrollee; and the cost of drugs dispensed as a percentage of wholesale cost. The objectives of the study were achieved but additional questions requiring future studies were raised.
