Correlation of neuropsychological, morphological and functional (regional cerebral blood flow and glucose utilization) findings in cerebral microangiopathy.

UNLABELLED: Cerebral microangiopathy, indicated in MRI by lacunar infarctions (LIs) and deep white matter lesions (DWMLs), is said to be accompanied by vascular dementia, which is reportedly caused by LI and DWML. METHODS: To confirm this assumption, 57 patients with cerebral microangiopathy were assessed for changes in regional cerebral blood flow (rCBF) and glucose utilization (rMRGlu) in both white matter and cortex, and these findings were correlated to the results of extensive neuropsychological testing (cognitive, mnestic and attentiveness tests), as well as to MRI findings. A special head holder ensured reproducibility of positioning during measurement of rCBF (99mTc-HMPAO SPECT) and rMRGlu (18F-FDG PET) and MRI. White matter and cortex were quantified with regions of interest defined on MRI and superimposed to corresponding PET/SPECT slices. The rMRGlu was calculated according to Sokoloff, and rCBF was determined from normalization to the cerebellum. LI and DWML were graded by number and extent. Brain atrophy was classified as no to slight inner and/or outer atrophy (Group A) or moderate-to-severe inner and outer atrophy (Group B). RESULTS: Even in severe DWMLs and in multiple LIs, rCBFs and rMRGlu values were not reduced. Analysis of variance identified atrophy and neuropsychological deficits as the main determinants for reduced rCBF and rMRGlu values (p < 0.05). However, 60% of patients (19 of 31) with neuropsychological deficits in attentiveness tests and 61% of patients (23 of 38) with mnestic deficits belonged to Group A and revealed decreased rCBF and rMRGlu values. Neuropsychological deficits correlated well with decreased rCBF and rMRGlu, whereas MRI patterns, such as LI and DWML, did not. CONCLUSION: We conclude that LI and DWML are epiphenomena that morphologically characterize cerebral microangiopathy. Dementia or neuropsychological deficits, however, are exclusively reflected by functional criteria (rCBF and rMRGlu), as long as cerebral atrophy does not occur.

Blood-flow velocities of the extraocular vessels in patients with high-tension and normal-tension primary open-angle glaucoma.

PURPOSE: To evaluate blood-flow parameters in three different groups of patients with primary open-angle glaucoma. METHODS: Hemodynamic parameters in the ophthalmic artery, central retinal artery, central retinal vein, and lateral and medial short posterior ciliary arteries were evaluated by color Doppler imaging in 237 patients with primary open-angle glaucoma and 124 age-matched normal control subjects. Group A consisted of 56 patients with primary open-angle glaucoma with treated intraocular pressure higher than 20 mm Hg; group B, of 103 patients with primary open-angle glaucoma with progression of glaucomatous damage despite intraocular pressure of 21 mm Hg or less; and group C, of 78 patients with normal-tension glaucoma. RESULTS: All patients showed a significant decrease in end-diastolic velocities (P < .01) and a significant increase in resistivity index (P < .05) in all arteries measured. Peak-systolic velocities were normal in the ophthalmic artery in all three groups. In the central retinal artery and the short posterior ciliary arteries, however, patients in groups B and C had significantly reduced peak-systolic velocity (P < .05) compared with normal control subjects. Peak-systolic velocity in group A did not differ significantly from that of normal control subjects. Maximal and minimal blood-flow velocities in the central retinal vein were significantly lower in groups B and C (P < .001) compared with normal control subjects. In group A, only minimal blood-flow velocity was significantly reduced (P < .05). CONCLUSIONS: Hemodynamic parameters in the extraocular vessels are altered in patients with glaucoma. Reduced blood-flow velocities may be secondary as well as contributory to glaucomatous damage.

Blood flow velocity in the extraocular vessels in chronic smokers.

AIMS: To determine blood flow velocity in the extraocular vessels in healthy, chronic smokers and to compare these blood flow velocities with those of healthy non-smokers. METHODS: In 46 healthy chronic smokers and 189 healthy non-smokers, peak systolic velocity (PSV), end diastolic velocity (EDV), and the resistivity index (RI) were measured in the ophthalmic artery (OA), central retinal artery (CRA), lateral short posterior ciliary artery (LPCA), and medial short posterior ciliary artery (MPCA) by means of a colour Doppler device, Siemens Quantum 2000. The maximal (max) and minimal (min) velocities were measured in the central retinal vein (CRV). Only one eye was measured in each subject, and right and left eyes were chosen randomly. Blood flow velocities were compared with one way MANOVA and t tests. The influence of age, sex, systolic and diastolic blood pressure, as well as heart rate on blood flow velocity and RI were evaluated by an analysis of covariance. The potential differences of the influence of the covariables on blood flow variables in smokers and non-smokers were tested by calculating the interactions. RESULTS: In the majority of measured vessels blood flow velocity was higher in smokers than in non-smokers. This difference was statistically significant in the OA, CRV, and LPCA. The RI indices were equal or slightly lower in smokers. Furthermore, smokers had significantly lower systolic and diastolic blood pressure. Heart rate was higher in smokers but this difference did not reach statistical significance. CONCLUSIONS: Colour Doppler measurements may differ significantly in smokers compared with non-smokers. Therefore, smoking habits should be considered when interpreting colour Doppler imaging results, and comparing different groups of diseased or healthy subjects.

Regional cerebral blood flow and negative/positive symptoms in 24 drug-naive schizophrenics.

UNLABELLED: SPECT/PET studies in schizophrenia revealed inconsistent changes of regional cerebral blood flow (rCBF). Frontal hyperperfusion as well as hypoperfusion are described. This study was undertaken to investigate the relations between rCBF, psychopathology according to PANSS and effects of neuroleptic therapy. METHODS: Twenty-four drug-naive acute patients with a first manifestation of schizophrenia were examined with 99mTc-HMPAO brain SPECT and assessed according to PANSS. Of these, 22 were controlled again after neuroleptic treatment. Following attenuation correction, region-to-cerebellar count ratios were obtained from 98 irregular regions of interest drawn in all slices (6.25 mm). The ratios were compared to 20 control subjects, and changes lying outside of 2 s.d. were considered abnormal. RESULTS: In different drug-naive patients, hyperperfusion as well as hypoperfused patterns were found. In drug-naive patients, the seven subscores of positive symptoms (pos 1-7) in PANSS showed different correlations to rCBF: Formal thought disorders (pos 2) and grandiosity (pos 5) were positively correlated to bifrontal and bitemporal rCBF (r = +0.59 to +0.70). Delusional ideas (pos 1), hallucinatory behavior (pos 3) and suspiciousness (pos 6) demonstrated a negative correlation to bifrontal, cingulate, left temporal and left thalamic rCBF (r = -0.59 to -0.66). Stereotyped ideas (neg 7) as a negative symptom showed a negative correlation to left frontal, left temporal and left parietal rCBF (r = -0.59 to -0.65). No correlations were found between residual positive symptoms and rCBF after neuroleptic treatment and clinical improvement, but all negative symptoms (neg 1-7) had a negative correlation to bifrontal, bitemporal, cingulate, basal ganglia and thalamic rCBF (r = -0.59 to -0.74). CONCLUSION: Our results illustrate that different positive symptoms are accompanied by different rCBF values: some induce hyperperfusion, others hypoperfusion. After therapy (and reduction of positive symptoms), only negative symptoms correlate exclusively to hypoperfusion. This may be the crucial factor in explaining inconsistencies of past results in perfusion pattern in drug-naive schizophrenic patients.

[A method for combined scatter and attenuation correction without transmission measurement for myocardial SPECT with 99m-Tc binding].

AIM: A new strategy is introduced for a double-head gamma-camera to calculate and to correct for attenuation distribution from count rates of backscattered radiation of 99mTc in the human thorax/myocardium setting. The intention was to gain segmented and calibrated images of distribution employing the structures of lungs and other tissues, so that attenuation may be subsequently corrected by the method of Chang METHODS: Acquisition was done simultaneously in three energy window for 99mTc to receive both emitted and scattered radiation. The acquire projections were used to calculate the attenuation distribution and to execute a scatter correction by the method of Ogawa et. al. By measuring phantoms and evaluating a set of patient studies, quality for both of the approaches towards attenuated distribution and the quality of attenuation correction was tested and an optimal setting for parameters involved was selected. RESULTS: The procedure adequately approaches the correction for both emitted and scattered radiation in SPECT, as experienced in phantom and patient studies. Clinical usefulness was preliminary documented in selected cases. In normal perfusion, distribution was more homogeneous. Perfusion defects appeared more definitive. Problems occurred in high adjacent non-target activity (e.g., in the gallbladder) and with low count rate statistics. CONCLUSION: Myocardial 99mTc MIBI SPECT may be effectively corrected for both attenuation a scatter without an additional transmission device.

Studies of the degradation products of nisin, a peptide antibiotic, using capillary electrophoresis with off-line mass spectrometry.

The utility of capillary electrophoresis (CE) for assessing the purity and stability of pharmaceutical peptides is investigated. The degradation of nisin, a pentacyclic peptide antibiotic, depends upon sample preparation and storage conditions and is followed by CE. With conventional UV detection, peaks are not identified and unresolved components are not detected. Matrix-assisted laser desorption ionization time-of-flight mass spectrometry (MALDI-TOFMS) of isolated CE fractions provides molecular mass information that aids in identification of the nisin degradants and determination of peak purity. The purity of pure and degraded nisin in the absence of any separation is also determined using electrospray ionization mass spectrometry (ESI-MS) and MALDI-TOFMS.

The frequency distribution of blood-flow velocities in the extraocular vessels.

PURPOSE: The study was carried out in order to assess the distribution of normal values of the blood-flow velocity in the extraocular vessels. METHODS: In 240 healthy visitors to a public fair, blood-flow characteristics in the extraocular vessels were measured, and the resistivity index was calculated. Blood-flow velocity was measured with a color Doppler imaging device, using a 7.5-mHz linear-array transducer. Peak-systolic and end-diastolic blood-flow velocities in the arteries were measured, and the resistivity index was calculated. In the central retinal vein the minimal and maximal blood-flow velocities were measured. The statistical analysis of the 14 measured and calculated variables included descriptive statistics, frequency distribution, and quantile plots. RESULTS: The quantile plots of the cumulative frequency showed that none of these 14 variables are normally distributed. Also, no normal distribution could be achieved by adjustment of the data by age. CONCLUSIONS: The blood-flow velocities in the extraocular vessels measured are not distributed normally. Therefore, nonparametric tests are to be used for statistical analysis if the sample size is small. The estimation of tolerance intervals has to be based on distribution-free assumptions.

Early institution of bromocriptine in Parkinson's disease inhibits the emergence of levodopa-associated motor side effects. Long-term results of the PRADO study.

Long-term levodopa treatment in Parkinson's disease is typically associated with "motor side effects" consisting in dyskinesias and/or fluctuations in motility referred to as the on-off phenomena. The main objective of this prospective, randomized, multi-centre study was to determine to what extent the development of such complications could be prevented by partial substitution of levodopa monotherapy (L-DOPA/benserazide) by bromocriptine in patients with early symptoms of the disease. The basic trial population included 674 newly diagnosed Parkinsonian patients that were randomly allocated to monotherapy with levodopa or a combination therapy based upon a nearly 40% replacement of levodopa by bromocriptine. The two target regimens had to be consistently maintained for 42 months. Parkinsonian symptoms were assessed by means of the Webster rating scale, the Hoehn and Yahr scale, and the Zung Self-Rating Depression scale. Motor side effects and adverse events were recorded at each regular clinic visit. Neurological symptoms improved and stabilized in a similar manner during treatment with both regimens throughout the study period. Motor side effects were observed in more patients on levodopa alone than on combination therapy (28.8 vs 20%; p = 0.008). According to Kaplan-Meier estimates the cumulative probability of experiencing motor side effects was 0.43 on monotherapy, compared to 0.28 on combination therapy, which was equal to a one third reduction of risk (p = 0.025). In regard to motor side effects, the degree of substitution of levodopa proved relevant: patients with > 50% substitution by bromocriptine exhibited half the risk observed in those with < 30% (p = 0.045). The overall burden of motor side effects, as reflected by a sum score based upon the relevance, the severity and the extent of motor dysfunction, was also significantly less on combination therapy (p = 0.046). In conclusion, partial substitution of levodopa by bromocriptine (> 30%) as first-line treatment of Parkinson's disease proves active in the prophylaxis of levodopa associated motor side effects. Early combination therapy therefore extends the period of optimal disease control.

Blood-flow velocities in the extraocular vessels in normal volunteers.

PURPOSE: To determine normal values of blood-flow velocities in extraocular vessels. METHODS: In one eye each in 189 healthy adult volunteers, blood-flow velocities in the ophthalmic artery (OA), central retinal artery (CRA), central retinal vein (CRV), short lateral posterior ciliary artery (LPCA), and short medial posterior ciliary artery (MPCA) were measured by color Doppler imaging. In the arteries, peak systolic velocity (PSV), end diastolic velocity (EDV), and resistivity index (RI) were calculated. In the CRV, maximal and minimal blood-flow velocities were measured. Influence of age, gender, blood pressure, and heart rate on blood-flow velocities and the resistivity index were analyzed. RESULTS: Mean outcomes +/- S.D. cm/sec were as follows: in the OA, PSV was 39.2 +/- 5.3, EDV was 9.1 +/- 2.5, and RI was 0.77 +/- 0.05. In the CRA, PSV was 11.0 +/- 1.8, EDV was 3.3 +/- 0.9, and RI was 0.71 +/- 0.05. In the short LPCA, PSV was 11.2 +/- 1.7, EDV was 3.7 +/- 1.0, and RI was 0.68 +/- 0.06. In the short MPCA, PSV was 11.2 +/- 11.7, EDV was 3.6 +/- 0.9, and RI was 0.68 +/- 0.05. In the CRV, mean maximal velocity was 4.5 +/- 0.9, and mean minimal velocity was 3.3 +/- 0.7. Age, gender, systolic blood pressure, diastolic blood pressure, and heart rate had no consistent statistically significant influence on the measured and calculated variables. CONCLUSION: Normal values for blood-flow velocities in the extraocular vessels serve as a basis in deciding whether a measured value of a patient is normal or abnormal.

Imaging of beta-oxidation by static PET with 14(R,S)-[18F]-fluoro-6-thiaheptadecanoic acid (FTHA) in patients with advanced coronary heart disease: a comparison with 18FDG-PET and 99Tcm-MIBI SPET.

14(R,S)-[18F]-fluoro-6-thiaheptadecanoic acid (FTHA) has been proposed as a PET tracer of the beta-oxidation pathway. The aim of this study was to investigate the diagnostic value of FTHA using static PET imaging in patients with ischaemically reduced left ventricular function. Twenty-one patients with angiographically proven advanced coronary heart disease were examined. All patients underwent SPET with 400 MBq [99Tcm]-2-methoxy-isobutyl-isonitrile (MIBI) for perfusion assessment and PET with 250 MBq FTHA under fasting conditions and with 150 MBq 2-[18F]-fluoro-2-deoxyglucose (FDG) following an oral glucose load. The uptake of FTHA and FDG was analysed quantitatively in 33 regions. Regional uptake was normalized to the region with highest MIBI uptake and expressed as a percentage. FTHA uptake paralleled MIBI uptake (r = 0.80) but not FDG uptake (r = 0.57). Mean FTHA uptake (38.1 +/- 16.3%) in 190 regions with severely reduced perfusion (MIBI uptake < 50%, mean uptake 36.8 +/- 9.4%) was significantly lower compared to FDG uptake (54.6 +/- 25.0%). FTHA uptake was preserved (> or = 70%) in 8 of 52 (13%) regions only with severely reduced perfusion but preserved glucose metabolism (FDG uptake > or = 70%). The similarity between FTHA and MIBI uptake suggests that static PET imaging with FTHA is of limited value when distinguishing between ischaemic or hibernating myocardium and scar. The underestimation of viability may be caused both by the dependence of uptake on flow and the suppression of beta-oxidation in regional chronic ischaemia under fasting conditions.

[Assessment of vision disorders using color duplex ultrasonography].

Disturbed circulation is a common cause of visual impairment which time course and severity can vary. Prompt diagnosis and therapy are mandatory to restore visual function in cases of acute drop of vision. Color Doppler imaging is a noninvasive method to investigate blood-flow velocity. During the past years this method has been introduced into ophthalmology. For the first time it is possible to measure the blood-flow velocity in the ophthalmic artery, the central retinal artery and vein as well as the ciliary arteries to quantify the extent of impaired ocular circulation.

[Assessment of myocardial viability using 201TL SPECT at rest to predict the reversibility of left ventricular wall motion abnormalities].

AIM: The aim of the present study was to predict preoperatively the reversibility of left ventricular wall motion abnormalities using TI-201 SPECT at rest. METHODS: 19 patients with advanced coronary artery disease and regional wall motion abnormalities were examined at rest with TI 201 SPECT (acquisition 15 minutes and 3 hours post injection). Successfully revascularized, proven by a second coronary angiography three month after revascularisation. Wall motion was again evaluated by cineventriculography. RESULTS: The preoperative a- or dyskinetic segments were best separated by the minimal relative TI-201 uptake in the defect 3 hrs p.i. in (i) postinterventionally improved wall motion (TI-201 uptake 67 +/- 14%, viable) and in (ii) without recovery (TI-201 uptake 46 +/- 12%, p < 0.001, non viable). A threshold at a TI-201 uptake of more than 50% yielded a positive predictive value of 0.73 and a negative predictive value of 0.86. If the segments with wall motion abnormalities (hypokinetic included) were evaluated as one group, no such threshold was obtained all segments which occurred a TI-201 uptake of more than 80% showed a functional recovery, wall motion did not improve in any segment with a TI-201 uptake of less than 40%. In the present study a considerable influence of the defect localisation with regard to the posterior wall was not observed. The values of TI-201 redistribution under resting condition were not useful to predict functional recovery. CONCLUSION: TI-201 SPECT in rest predicts preoperatively the reversibility in regions with severe wall motion abnormalities only and indicates myocardial viability in these cases. Thus, the method is basically useful to determine myocardial viability. In hypokinetic segments, however, a wide range of TL-201 uptake values exists without definite evidence to functional recovery.

Reproducibility of color Doppler imaging in orbital vessels.

The purpose of this study was to evaluate the short-term reproducibility, the long-term reproducibility, and the reproducibility of the measurements made between two different examiners of blood-flow velocity in extraocular vessels using color Doppler imaging (CDI). In a group of 10 healthy volunteers, measurements of the peak-systolic velocity (PSV), end-diastolic velocity (EDV), and resistivity index (RI) in the ophthalmic artery and the central retinal artery as well as one lateral and one medial short posterior ciliary artery were performed. The best intraindividual reproducibility, expressed as the relative error, was found for the PSV and RI measured in the ophthalmic artery (OA; PSV 5.9%, RI 3.1%) and the central retinal artery (CRA; PSV 7.7%, RI 4.7%). The PSV and RI determined in the posterior ciliary arteries were less reproducible but as much so as the EDV measured in the OA (11.8%) and CRA (19.9%). No systematic trend could be found between the first and second measurements. However, the long-term fluctuation was considerable high. Measurements of PSVs showed good concurrence between the two observers in all four vessels measured. The EDVs and RIs differed statistically significantly between the two observers. This difference was most pronounced in the posterior ciliary arteries.

Endothelin-1 plasma levels in normal-tension glaucoma: abnormal response to postural changes.

BACKGROUND: Endothelin-1 (ET-1) is a potent vasoconstrictor peptide produced by vascular endothelial cells. ET-1 may have a role in the pathogenesis of various vascular diseases. There are reports in the literature that ET-1 plasma levels are raised in normal-tension glaucoma (NTG) patients. METHODS: ET-1 concentration, plasma renin activity, and 24-h blood pressure were measured in 21 high-tension glaucoma (HTG) patients, 19 NTG patients, and 20 non-glaucomatous controls in supine and upright positions. RESULTS: ET-1 plasma levels tended to be higher in NTG patients (3.2 +/- 2.2 pg/ml) than in HTG patients (2.2 +/- 0.6 pg/ml) and controls (2.6 +/- 0.7 pg/ml). The differences, however, were not statistically significant. The individual scatter was significantly greater in the NTG group, indicating that our NTG patients are a heterogeneous population. The physiological increase in ET-1 plasma level after changing from the supine to the upright position was absent in NTG patients. Plasma renin activities tended to be lower in NTG patients (1.2 +/- 1.2 ng/ml/h) than in HTG patients (1.3 +/- 0.8 ng/ml/h) and controls (2.0 +/- 1.7 ng/ml/h). This may explain why NTG patients had relatively low blood pressure despite high ET-1 levels. CONCLUSIONS: Our data support the hypothesis that vascular dysfunction may be involved in the pathogenesis of optic nerve damage in normal-tension glaucoma.

[Automated perimetry in ptosis and blepharochalasis].

BACKGROUND: First, we were looking if a correlation between the extent of ptosis and superior visual field defect exists. In a second step visual fields in blepharochalasis were compared with those in ptosis. PATIENTS AND METHODS: Pre- and postoperative visual fields in 20 patients with blepharochalasis, 6 patients with congenital and 9 with acquired ptosis were tested using program 7 of Octopus 500 automated perimeter. The degree of ptosis was separated in mild, medium and severe. According to the extent of visual field defect the same parameters as in ptosis were used also grading blepharochalasis in mild, medium and severe. RESULTS: We found that the amount of upper visual field defect clearly correlated with the extent of ptosis. In addition, patients with congenital ptosis showed less visual field defect compared to those with acquired ptosis. Comparable to ptosis surgery blepharoplasty also lead in severe and medium cases to a clear gain of upper visual field. CONCLUSION: Using perimetry and the same grading system as for ptosis makes it possible to study the functional defect occurring to the blepharochalasis patient.

Influence of posture on the visual field in glaucoma patients and controls.

The intra-ocular pressure rises when body position is changed from sitting to supine. This phenomenon occurs to the same extent in healthy people and glaucoma patients. The purpose of this preliminary study was to determine the effect of posture on the visual field. Visual fields of 15 eyes of 12 normal-tension glaucoma (NTG) patients, 16 eyes of 11 high-tension glaucoma (HTG) patients and 30 eyes of 26 healthy controls were measured in the upright and supine positions with program G1X on an Octopus swivel-arm perimeter. The delta mean defect (delta MD), that is MDupright-MDsupine, was calculated for each patient and compared among study groups. Whereas visual fields improved in healthy controls (delta MD = +0.45 dB), glaucoma patients showed a slight deterioration of the visual field after changing position from supine to upright (HTG: delta MD = -0.25 dB, NTG: delta MD = -0.24 dB). The difference between HTG and controls, as well as between NTG and controls, was statistically significant (p < 0.002, Student's t test). These findings indicate that in some glaucoma patients the regulatory mechanisms for blood supply in the optic nerve head are insufficient.
