Inflammatory epidermolysis bullosa acquisita with coexistent IgA antibodies to plectin.

We present a case of inflammatory epidermolysis bullosa acquisita (EBA) with IgA antibodies to plectin. Analysis of lesional skin biopsies by electron microscopy revealed the split level to be in the sublamina densa zone, corresponding to the diagnosis of EBA. Direct immunofluorescence of perilesional skin demonstrated u-serrated depositions of IgG and IgA that under immunoelectron microscopy were shown to be located in the sublamina densa. In contrast, indirect immunofluorescence on salt-split skin revealed circulating IgA antibodies that stained the roof rather than the floor of the blister. Immunoblotting showed these serum antibodies to be directed to the cytoplasmic hemidesmosomal antigen plectin. The antiplectin specificity of these antibodies was confirmed by 'knockout' immunofluorescence analysis; the serum IgA did not bind to skin sections of a patient with plectin-deficient epidermolysis bullosa. To our knowledge, this case demonstrates for the first time the existence of IgA antibodies against plectin.

Identification of a new polyphosphoinositide in plants, phosphatidylinositol 5-monophosphate (PtdIns5P), and its accumulation upon osmotic stress.

Polyphosphoinositides play an important role in membrane trafficking and cell signalling. In plants, two PtdInsP isomers have been described, PtdIns3P and PtdIns4P. Here we report the identification of a third, PtdIns5P. Evidence is based on the conversion of the endogenous PtdInsP pool into PtdIns(4,5)P(2) by a specific PtdIns5P 4-OH kinase, and on in vivo (32)P-labelling studies coupled to HPLC head-group analysis. In Chlamydomonas, 3-8% of the PtdInsP pool was PtdIns5P, 10-15% was PtdIns3P and the rest was PtdIns4P. In seedlings of Vicia faba and suspension-cultured tomato cells, the level of PtdIns5P was about 18%, indicating that PtdIns5P is a general plant lipid that represents a significant proportion of the PtdInsP pool. Activating phospholipase C (PLC) signalling in Chlamydomonas cells with mastoparan increased the turnover of PtdIns(4,5)P(2) at the cost of PtdIns4P, but did not affect the level of PtdIns5P. This indicates that PtdIns(4,5)P(2) is synthesized from PtdIns4P rather than from PtdIns5P during PLC signalling. However, when cells were subjected to hyperosmotic stress, PtdIns5P levels rapidly increased, suggesting a role in osmotic-stress signalling. The potential pathways of PtdIns5P formation are discussed.

Osmotic stress activates distinct lipid and MAPK signalling pathways in plants.

Plants are continuously exposed to all kinds of water stress such as drought and salinity. In order to survive and adapt, they have developed survival strategies that have been well studied, but little is known about the early mechanisms by which the osmotic stress is perceived and transduced into these responses. During the last few years, however, a variety of reports suggest that specific lipid and MAPK pathways are involved. This review briefly summarises them and presents a model showing that osmotic stress is transmitted by multiple signalling pathways.

MAP Kinase Signaling System↗

Hyperosmotic stress rapidly generates lyso-phosphatidic acid in Chlamydomonas.

Plant cells are continuously exposed to environmental stresses such as hyper-osmolarity, and have to respond in order to survive. When 32P-labelled Chlamydomonas moewusii cells were challenged with NaCl, the formation of a new radiolabelled phospholipid was stimulated, which was barely detectable before stimulation. The phospholipid was identified as lyso-phosphatidic acid (LPA), and was the only lyso-phospholipid to be accumulated. The increase in LPA was dose- and time-dependent. When other osmotically active compounds were used, the formation of LPA was also induced with similar kinetics, although salts were better inducers than non-salts. At least part of the LPA was generated by phospholipase A2 (PLA2) hydrolysing phosphatidic acid (PA). This claim is based on PA formation preceding LPA production, and PLA2 inhibitors decreasing the accumulation of LPA and promoting the conversion of PA to diacylglycerol pyrophosphate. The latter is another metabolic derivative of PA that is implicated in cell signalling. The involvement of multiple lipid-signalling pathways in hyperosmotic stress responses is discussed.

Implant-retained mandibular overdentures: 6-year results of a multicenter clinical trial on 3 different implant systems.

PURPOSE: The goal of this multicenter prospective clinical trial was to compare clinical outcome and post-treatment care and patient satisfaction with different implant systems used for mandibular overdenture treatment during a 6-year follow-up period. PATIENTS AND METHODS: A total of 87 edentulous patients with a severely resorbed mandible (bone height, 8 to 15 mm) received treatment involving either 2 Intra Mobiele Zylinder implants (IMZ group, n = 41) (Friedrichsfeld AG, Mannheim, Germany), 2 Brånemark implants (Brå group, n = 17) (Nobel Biocare AB, Gothenburg, Sweden) or a Transmandibular Implant (TMI; Krijnen Medical, Beesd, The Netherlands) (TMI group, n = 29). The evaluation involved clinical parameters, radiographs, surgical and prosthetic post-treatment care, clinical implant performance (CIP scale), and patient satisfaction. RESULTS: After 6 years, there was a significant difference in the mean plaque index:the mean plaque index of the TMI group was significantly higher than that of the IMZ and Brå groups. Other clinical parameters showed no significant differences. The implant survival rate of 97.5% was noted in the IMZ group, 97.1% in the Brå group, and 72.0% in the TMI group. The CIP scale were significantly worse for the TMI group. CONCLUSION: It was concluded that the IMZ and the Brånemark implant systems have a better survival rate and clinical implant performance than the TMI system. Based on these data, these systems should be the choice for the edentulous mandible with a height between 8 and 15 mm.

Use of short endosseous implants and an overdenture in the extremely resorbed mandible: a five-year retrospective study.

PURPOSE: A retrospective study was performed to assess the success of short endosseous implants in combination with an implant-retained overdenture in the extremely resorbed mandible. PATIENTS AND METHODS: Patients included had a mandible not exceeding a median height of 12 mm measured on a standardized lateral cephalometric radiograph. Seventeen patients (14 women, 3 men; mean age, 65 years) with 68 endosseous implants were clinically and radiographically evaluated. RESULTS: During the follow-up period (mean, 77 months; range, 60 to 97 months), 8 implants were lost, bringing the cumulative survival rate to 88%. The peri-implant tissues were in good condition, and the bone loss around the implants was minimal. CONCLUSION: Because of the relative simplicity and low morbidity of this treatment strategy, it is a justified treatment option.

The oblique radiographic technique for bone height measurements on edentulous mandibles: a preclinical study and an introduction to the clinical use.

OBJECTIVE: The aim of this study was to quantify the effect of mandibular angulation, position, and shape of the edentulous mandible on the distortion of its image on lateral oblique radiographs in a preclinical study. Suggestions for the clinical use of this technique are made. STUDY DESIGN: Five edentulous dry mandibles were used, varying in size from small to wide and equipped with metal bars in and on top of the mandible. The mandibles were radiographed at 9 different positions by tilting the mandible around the intercondylar axis and by using the oblique projection technique in a cephalostat. RESULTS: The length of the images of the metal bars varied significantly when we tilted the mandibles from 20 degrees to -20 degrees. As a result of the effects of magnification and distortion, the image size varied from between 0.96% and 1.06% of the real size. Neither the size of the mandible nor the position of the bars was related to the degree of magnification. CONCLUSION: The oblique projection technique has limited geometric errors in depicting the edentulous mandible. For evaluative bone height measurements in patients with extremely resorbed mandibles treated with dental implants, it is a promising technique if the patient (specifically the mandible) can be positioned reproducibly in the apparatus.

Horizontal osteotomy for reconstruction of the narrow edentulous mandible.

The aim of this retrospective study was to evaluate the applicability of a horizontal osteotomy procedure for reconstruction of a narrow edentulous mandible in order to enable insertion of implants. In 7 edentulous patients a narrow inferior alveolar ridge was reconstructed with autogenous bone grafts harvested by a horizontal osteotomy procedure (mean width of the top of the alveolar crest before grafting 1.3 +/- 0.3 mm, after grafting 5.6 +/- 0.6 mm). Insertion of endosseous implants (10 Brånemark implants, 8 ITI Bonefit implants) was performed 3 months after the grafting procedure. Implant supported overdentures were fabricated 3 months after implantation. At regular time intervals a standardized clinical and a radiographic evaluation was performed. All grafted sites showed sufficient bone volume for insertion of the implants at the moment of implantation. At time of evaluation (mean 37 +/- 14.6 months, range 14-68 months) all implants and overdentures functioned well. None of the patients showed a disturbed sensitivity of the lip or chin region. It is concluded that reconstruction of a narrow edentulous mandible with autogenous bone grafts harvested by an intraoral horizontal osteotomy was shown to be a reliable preimplantology procedure with potential applicability in this group of patients.

Effectiveness of three treatment modalities for the edentulous mandible. A five-year randomized clinical trial.

Currently, 3 treatment options are available for patients with denture complaints and an edentulous mandible with a height of at least 15 mm: meticulous construction of a new set of dentures (CD), construction of a new set of dentures following preprosthetic surgery to enlarge the denture-bearing area (PPS), and construction of an implant-retained mandibular overdenture (IRO). The aim of this study was to evaluate patient satisfaction and subjective chewing ability of edentulous patients treated with one of these treatment modalities. Ninety edentulous patients (Cawood class IV and V, mean mandibular height 20.7 mm) were randomly assigned to receive 1 of these 3 groups. Denture satisfaction and chewing ability were assessed using questionnaires focusing on denture-related complaints and problems chewing different types of food were assessed before treatment, and 1- and 5-years after treatment. At the 1-year evaluation, significantly better scores were observed in the 2 surgical groups (IRO, PPS) than in the CD group. At 5-year evaluation the "complaints of the lower denture" showed a significantly better score in the IRO group when compared to the PPS and CD groups. No significant differences were observed between the PPS and CD group. From this study it is concluded that both in the short and long term denture satisfaction appears most favourable in the IRO group when compared to the PPS and CD groups. Implant-retained overdentures are therefore a satisfactory treatment modality for edentulous patients with problems with their lower denture, even in cases of not severely resorbed mandibles.

A controlled clinical trial of implant-retained mandibular overdentures; five-years' results of clinical aspects and aftercare of IMZ implants and Brånemark implants.

The aim of this prospective randomized controlled clinical trial was to evaluate a set of clinical items and prosthetic aftercare of edentulous patients with a mandibular overdenture retained by 2 IMZ implants or 2 Brånemark implants during a 5-years' period. Patients were allocated to the IMZ group (n = 29) or the Brånemark group (n = 32) by a computerized balancing method. In the IMZ group 4 implants were lost during the 5-years' follow-up (survival rate: 93%). In the Brånemark group 9 implants were lost (survival rate: 86%). All patients were re-operated successfully. Multiple prosthetic revisions were necessary in both groups, especially the precision attachment system in the overdenture was subject to frequent fracture or loosening. From this study can be concluded that there is no difference in clinical state, radiographical state, survival rate and clinical implant performance between the IMZ implant system and the Brånemark implant system supporting an overdenture on 2 implants after 5 years of follow-up.

Hyperosmotic stress stimulates phospholipase D activity and elevates the levels of phosphatidic acid and diacylglycerol pyrophosphate.

In mammalian cells, phospholipase D (PLD) and its product phosphatidic acid (PA) are involved in a number of signalling cascades, including cell proliferation, membrane trafficking and defence responses. In plant cells a signalling role for PLD and PA is also emerging. Plants have the extra ability to phosphorylate PA to produce diacylglycerol pyrophosphate (DGPP), a newly discovered phospholipid whose formation attenuates PA levels, but which could itself be a second messenger. Here we report that increases in PA and its conversion to DGPP are common stress responses to water deficit. Increases occur within minutes of treatment and are dependent on the level of stress. Part of the PA produced is due to PLD activity as measured by the in vivo transphosphatidylation of 1-butanol, and part is due to diacylglycerol kinase activity as monitored via 32P-PA formation in a differential labelling protocol. Increases in PA and DGPP are found not only in the green alga Chlamydomonas moewusii and cell-suspension cultures of tomato and alfalfa when subjected to hyperosmotic stress, but also in dehydrated leaves of the resurrection plant Craterostigma plantagineum. These results provide further evidence that PLD and PA play a role in plant signalling, and provide the first demonstration that DGPP is formed during physiological conditions that evoke PA synthesis.

Implant-retained mandibular overdentures compared with complete dentures; a 5-years' follow-up study of clinical aspects and patient satisfaction.

The aim of this prospective randomized controlled clinical trial was to evaluate and compare clinical aspects and satisfaction during the first year following treatment and consecutively the change in treatment during the next 4 years of follow-up. Patients were allocated to one of the following treatment modalities: an implant-retained overdenture (IRO-group, 2 endosseous implants, n = 61) or a complete denture (CD-group, n = 60). One year after placement of the denture, unsatisfied patients of the CD-group got the opportunity for a retreatment including an implant-retained overdenture. In the IRO-group 4 implants were lost during the first year and again 4 implants were lost during the next 4 years (survival rate: 93%). All patients could be re-operated successfully. In the CD-group 14 patients (23%) chose an implant-retained overdenture after 1 year. Patients of the IRO-group were significantly more satisfied than patients of the CD-group after 1 year (satisfaction score 8.3 versus 6.6, scale 1-10) and after 5 years (7.4 versus 6.4). From this study it can be concluded that endosseous implants have a high survival rate after 5-years' follow-up. Satisfaction score of the IRO-group is diminishing in time, probably because patients get used to an improved situation. After 5 years, the mean satisfaction score of the CD-group (including patients who got implants) was still lower than of the IRO-group, in spite of the opportunity to a retreatment and have implant-retained overdentures.

Alveolar Bone Loss↗

[Dental implants: a better anchor for dentures].

Loss of teeth results in resorption of the jaws. As a consequence the denture bearing area progressively reduces, eventually causing loss of retention and stability of the conventional denture. In three patients, two women aged 36 and 37 and a man aged 46, suffering from severe problems with their conventional dentures endosteal implants were inserted in the jawbone. After the healing period the dentist fabricated an implant-retained overdenture. The patients were very satisfied with the final result. Endosteal implants are, especially in the mandible, a reliable treatment modality for a stable implant-retained denture in case of patients suffering from retention problems with their conventional dentures.

Reconstruction of the extremely resorbed mandible with interposed bone grafts and placement of endosseous implants. A preliminary report on outcome of treatment and patients' satisfaction.

During recent decades many surgical techniques have been developed to enlarge the denture-bearing area of the mandible. Most of these techniques improved retention and stability of the lower dentures only temporarily. Since the advent of endosseous implants to stabilize overdentures, combinations of augmentation procedures and placement of endosseous implants have been introduced to restore the severely resorbed mandible. In this study we describe the preliminary results of such a combined approach using sandwich osteotomy with an autogenous bone graft (iliac crest) followed by placement of four endosseous implants in the interforaminal region in 10 women. After a mean follow-up period of 31 months (range 19-57) several variables were measured including condition of the peri-implant tissues, radiographic bone changes and patient satisfaction. The first results indicate that the technique described offers a solid base for implant-stabilized overdentures: no implants were lost, the peri-implant tissues were in good condition, bone loss was limited, and patients were satisfied. Future studies will evaluate the permanence of these results.

The mandibular staple bone plate: a long-term retrospective evaluation.

PURPOSE: Clinical and radiographic parameters and denture satisfaction were evaluated in a long-term retrospective study of patients treated with the mandibular staple bone plate. PATIENTS AND METHODS: Fifty-six edentulous patients were treated with the mandibular staple bone plate to stabilize their lower denture. The mean evaluation period was 103 months (range, 84 to 139 months). Peri-implant mucosa and bone height were scored, together with quality of the prosthesis and prosthodontic maintenance care. Denture satisfaction was assessed by using two questionnaires. RESULTS: Four staple bone plates were removed during the evaluation period, and one appeared to be fractured. The remaining 51 staple bone plates were functional without any signs of major complications (survival rate, 91%). No further alveolar resorption in the interforaminal region of the mandible took place during the evaluation period. Patients were very satisfied with the prosthetic construction. CONCLUSIONS: The mandibular staple bone plate is a good modality to stabilize the lower denture. However, endosseous implant systems are preferred because of comparable success rates with a more simple operative procedure.

Mandibular overdentures supported by two Brånemark, IMZ or ITI implants. A prospective comparative preliminary study: one-year results.

The aim of this prospective comparative study was to evaluate the condition of the peri-implant tissues of three different implant systems supporting a mandibular overdenture. Ninety edentulous patients (Cawood class V-VI) participated in this study. After randomization, 30 patients were treated with 2 Brånemark implants, 30 patients with 2 IMZ implants and 30 patients with 2 ITI implants. The implants were inserted in the canine region of the mandible. After 3 months overdentures were fabricated supported by a round bar and clip attachment. A standardized clinical and radiographic evaluation was performed 0,6 and 12 months after insertion of the denture. The intraoral radiographs were made, using the long-cone technique with an aiming device. Two implants were lost (1 Brånemark, 1 IMZ) during the healing period. None of the patients showed any sensory change in lip or chin region. The pocket depth in the Brånemark group decreased significantly whereas the mucosa recession increased significantly in both the Brånemark as well as in the IMZ group. After 12 months, there was significantly less bone loss in the ITI group. From our study it was concluded that 2 (Brånemark, IMZ or ITI) implants placed in the interforaminal region connected with a bar supply a proper base for the support of a mandibular overdenture in the (Cawood V-VI) edentulous patient. The ITI implant appears to be the implant of choice for mandibular overdenture therapy, because only one operation is required for a comparable result.

Radiographic assessment of changes in marginal bone around endosseous implants supporting mandibular overdentures.

OBJECTIVES: To evaluate the reliability of a new method for quantification of radiographic changes in the marginal bone around implants. METHODS: Three groups of seven patients treated with an overdenture supported by two endosseous implants (Brnemark, n = 7 patients; IMZ, n = 7 patients; ITI Bonefit, n = 7 patients) were selected. Six weeks after loading the implants, radiographs were obtained using the long cone technique and an aiming device, scanned and digitized. Two observers measured the height and area of the peri-implant bone defects twice. The difference between measurements was statistically analysed using paired t-tests. RESULTS: There were small differences in interobserver error for the Brnemark implant system (P < 0.05), but no significant differences for the IMZ and ITI Bonefit. There were no significant difference in intraobserver error with any of the three implant systems (P > 0.05). CONCLUSIONS: The newly developed technique by measuring the area of the defect overcomes some drawbacks of existing techniques for quantification of peri-implant bone loss in the mandible.
