[Surgical treatment of peripheral nerve injuries. Evaluation of a multicenter study].

In a retrospective study 674 nerve injuries of the upper limb were analysed, which were treated by operative management in 10 hospitals of the GDR. The median nerve (226 cases) and the ulnar nerve (209 cases) most were frequently concerned. 427 injured nerves were reconstructed primary or early secondary. In 46.3% epineural nerve suture was performed and in 23% interfascicular nerve grafting. Good or very good results occurred in 71% (motor recovery) and 70.7% (sensory recovery) according to the scheme of Highet. The best results occurred after primary reconstruction independent of operative technique. The results get worse with increasing age.

[Ligament injuries of the metacarpophalangeal joint of the thumb].

When it comes to injuries of the collateral ligament in the metacarpal-phalangeal joint of the thumb, the best results have proved to be obtainable from primary surgery. Secondary ligament surgery according to Strandel, in its modification by Bäuerle and Reill, has its clear-cut indication and ensures restoration of thumb functionality. Arthrodesis is an alternative that should be chosen with generosity.

[Intraneural ganglia of the common peroneal nerve].

Intraneural ganglia (pseudocysts) of the peronaeus communis nerve are rare diseases. A palpable tumour in the popliteal fossa, paraesthesia, and myasthenia, followed by sensorial and locomotor failures, are the clinical symptoms. Symptoms are mild at the beginning, which makes early detection difficult. Microsurgical extirpation of the ganglia, without touching the nerve fibres, is the therapeutic approach. Three of the authors' own cases are described in some detail.

[Microsurgical treatment of brachial plexus injuries].

Injuries of the brachial plexus are found to occur primarily to juvenile patients as a consequence of motorcycle accidents. While it is a severe injury, its prognosis has been substantively improved by the availability of microsurgical treatment, using long nerve transplants. Further improvement of results will be possible by early operation, a desirable objective. 44 patients with brachial palsy received treatment, including surgery in 41 cases. An account is given in this paper of intraoperative findings, surgical techniques, and results from follow-up checks. The need is underlined for long-term intensive after-care for the purpose of occupational reintegration.
