Cephalothin. Its in vitro antibacterial spectrum as studied in a diagnostic laboratory.

Of 655 bacterial strains isolated in a routine clinical diagnostic laboratory, 564 (86 per cent) were found sensitive to cephalothin by disk sensitivity test. However, the fraction of sensitive Gram-negative bacilli and enterococci declined sharply at concentrations approximating those obtained in serum after the usual recommended dosages. Gram-positive bacteria remained sensitive throughout the entire range of testing concentrations. Unless very high dosages are used, cephalothin cannot be considered a broad-spectrum antibacterial agent for use while cultures are pending. Even with very high dosages all strains of Pseudomonas spp. and significant fractions of other Gram-negative bacillary species remain resistant to cephalothin. If used alone, the high potency cephalothin disk may yield inadequate information and should be supplemented by a lower potency disk, standardized measurements of zone diameters, or a screening plate.

Policies, laws, and regulations pertaining to antibiotics: report of Task Force 3.

A questionnaire survey-based study of 41 countries was conducted to determine the nature and extent of policies, laws, and regulations governing the prescription, sale, and use of antibacterial agents and the enforcement of and compliance with such policies, laws, and regulations. On the basis of responses adequate for analysis that were received from 35 countries, three groups were identified according to the availability of antibacterial agents to the general public in a country. In one group, access by the general public to these agents was severely restricted, and restrictions were effectively enforced. In the second group, restrictions were less stringent, and enforcement was less effective. In the third group, access was unrestricted. Still to be determined is whether this grouping bears any relation to patterns of antibacterial use, resistance to antibacterial agents, and other factors pertaining to the use of these drugs.

[A community referral system for the permanent program of universal vaccination].

This article describes a community referral system for the permanent immunization program, tested in Tijuana, Baja California, Mexico, by the Regional Nucleus for Health Systems Development (NUREDESS-Norte). The model was designed to facilitate the participation of the intermediate organizations that make up the community in urban settings. Through appropriate technology, health counselors identify with precision, ease and rapidity the specific immunization needs of pre-school age children. The counselors also help diminish the barriers in the way to service access, and follow-up the children at highest risk.
